Deciphering the mechanisms of the synergistic efficacy of Shenqi Fuzheng formula (Codonopsis pilosula and Astragalus membranaceus) against acute myocardial infarction at the single-cell level [RNA-seq]

ABSTRACT: The Shengqi (SQ) formula, comprising Danshen (DS) and Huangqi (HQ), is effective against acute myocardial infarction (AMI). While DS and HQ have distinct chemical profiles, their synergistic mechanisms remain unclear. We integrated RNA sequencing (RNA-seq) and single-cell RNA sequencing (scRNA-seq) to investigate the molecular and cellular basis of DS-HQ synergy in the AMI model. DS and HQ monotherapies restored 43.91% and 49.26% of AMI-associated genes, respectively, whereas SQ rescued 77.49% of these genes. Despite compositional differences, DS and HQ co-regulated angiogenesis-related targets. Single-cell analysis showed that SQ treatment activated microvascular endothelial subpopulations, enhanced reparative Spp1^hi macrophage polarization, and increased VEGFA secretion, thereby amplifying VEGFA–VEGFR signaling. Docking studies indicated that active compounds from DS and HQ bind distinct VEGFA domains, supporting structural synergy. DS and HQ synergistically promote angiogenesis and immune–endothelial crosstalk in AMI, offering new insights into combinational drug mechanisms in cardiovascular disease.
