Disturbed flow in the juxta-anastomotic area of an arteriovenous fistula correlates with endothelial loss, acute thrombus formation and neointimal hyperplasia

Project description:Background: Clinical failure of arteriovenous fistulae (AVF) is frequently due to juxta-anastomotic neointimal hyperplasia (JANIH). Although the mouse AVF model recapitulates human AVF maturation, previous studies focused on the outflow vein distal to the anastomosis. We hypothesized that the juxta-anastomotic area (JAA) has increased NIH compared to the outflow vein. Method: AVF were created in C57BL/6 mice without or with chronic kidney disease (CKD). Temporal and spatial changes of the JAA were examined using histology and immunofluorescence. Computational techniques were used to model the AVF. RNA-seq and bioinformatic analyses were performed to compare the JAA with the outflow vein. The jugular vein to carotid artery AVF model was created in Wistar rats. Result: The neointima in the JAA shows increased volume compared to the outflow vein. Computational modeling shows increased volume of disturbed flow at the JAA compared to the outflow vein. Endothelial cells are immediately lost from the wall contralateral to the fistula exit, followed by thrombus formation and JANIH. Gene Ontology (GO) enrichment analysis of the 1862 differentially expressed genes (DEG) between the JANIH and the outflow vein identified 525 overexpressed genes. The rat jugular vein to carotid artery AVF showed changes similar to the mouse AVF. Conclusion: Disturbed flow through the JAA correlates with rapid endothelial cell loss, thrombus formation, and JANIH; late endothelialization of the JAA channel correlates with late AVF patency. Early thrombus formation in the JAA may influence later development of JANIH.

Genes differently expression between human failed arteriovenous fistula (AVF) and the control

Project description:From January 1, 2018 to December 31, 2018, there were 19 patients (from four hemodialysis centers) undergoing failing arteriovenous fistula (AVF) that were resistant to endovascular therapy with extensive fistula aneurysmal change (n=4), diffuse stenosis (n=13) or poor skin condition of the forearm (n=2). While 13 of them manifested good size and quality of the outflow vein (>10 cm long, >6 mm in diameter and <6 mm under the skin, BF >600 ml/min) to create of a new AVF upstream. They have been provided with an end-to-side anastomosis of the outflow vein with the more proximal inflow artery, together with ligation of the failing downstream AVF. During the operation, the small segment adjacent to the distal healthy access was exercised for the control access (CA) sample. Until December 31, 2019, 5 of 13 patients had suffered failing AVF again, followed by the excision of segment of high-grade stenosis for the failing access (FA) samples.

Project description:Arteriovenous fistula (AVF) is the preferred vascular access for patients undergoing hemodialysis and the majority of AVFs fail because of venous intimal hyperplasia (VNH). Results from our lab have shown that adipose tissue-derived mesenchymal stem cell (MSC) transplantation to the adventitia of the outflow vein of AVF attenuates VNH in an immunodeficient mice. in the present study efficacy of AMSCs transplantation in treating AVF failure was tested in a chronic kidney disease (CKD) followed by an AVF created in C57BL/6 mice and porcine models as described in methods. Moreover, a phase 1 trial was performed in small group of endstage renal disease (ESRD) patients who are in hemodialysis. To explore the molecular mechanisms involved in MSCs transplantation improving AVF durability, we performed RNA-seq transcriptome analysis of the outflow veins of AVFs excised from mice and Pigs the after adventitial transplantation of MSCs from the donor inbred C57BL/6 mice or pigs respectively. Whole genome transcriptomics of pre-access veins and hemodialysis arteriovenous fistula (AVF) samples from two-stage AVF patients with different maturation outcomes (matured vs failed)

Project description:We report the RNA expression profiles of native veins used for AVF creation and of remodeled AVF samples obtained 7-16 weeks later at the time of transposition (if vein matured) or salvage procedure (if vein failed). We perfomed RNA-seq on native veins and AVFs with different maturation outcomes (matured vs. failed). The "matured" and "failed" subgroups were similar in terms of demographics (age range, sex distribution, ethnicity distribution), clinical characteristics (proportion of diabetes mellitus and coronary artery disease, previous hemodialysis access history), and time interval between first-stage and second-stage surgeries.
