Adipose tissue gene expression associated with weight gain in kidney transplant recipients

ABSTRACT: The aim of this study was to investigate the association of gene expression profiles in subcutaneous adipose tissue with percent of total body weight change in 26 kidney transplant recipients. Using multivariate linear regression analysis controlled for race and gender, expression levels of 1553 genes were significantly (p<0.05) associated with weight change.

A secondary data analysis was done on a subgroup (n=26) of existing clinical and gene expression data from a larger prospective longitudinal study examining factors contributing to weight gain in transplant recipients.

Project description:Antipsychotic drugs (APs) are used to treat psychiatric disorders but also have the prominent side effect of weight gain resulting in a higher incidence of metabolic disease in this patient group. While the majority of patients gain significant weight in response to APs some patients are relatively resistant to these effects. APs such as clozapine and olanzapine are deemed to be highly efficacious at treating psychiatric conditions yet they have some of the highest weight gain liabilities. Therefore, there is a need to determine which patients are less susceptible to the metabolic side effects of APs and would be good candidates for drugs such as olanzapine, and conversely, identifying patients that should be prescribed alternative APs with less weight gain liabilities.

Subcutaneous adipose tissue (SAT) gene expression patterns between weight control and regaining weight subjects

Project description:Objective: This work aimed at identifying and characterizing differences in subcutaneous adipose tissue (SAT) gene expression patterns between subjects succeeding weight control against subjects regaining weight 6 months after a low calorie diet program. Methods: Weight-reduced obese subjects from 8 European countries were randomized into 4 diets differing in protein and glycemic index. In addition to the anthropometric and plasma parameters, SAT biopsies were taken at the beginning (CID2) and the end (CID3) of the weight maintenance intervention. Pan-genomic cDNA microarrays were used to define SAT gene expression profiles at both CID2 and CID3 of 22 subjects succeeding weight maintenance (successful subjects; mean weight change= -2.6 ± 1.2 kg) and 22 subjects regaining weight (unsuccessful subjects; mean weight change= 3.9 ± 1.3 kg) among the 4 dietary arms. Results: Differences in SAT gene expression patterns between successful and unsuccessful groups were mainly due to weight variations rather to differences in diet macronutrient composition. An ANCOVA analysis with total energy intake at CID3 as covariant led us to the definition of 1339 differential genes. Functional analysis of differential genes showed that cellular growth and proliferation, inflammation, cell death, cancer, cellular function and maintenance were the main biological processes involving these differential genes.

Altered adipose lipid mobilization predicts long-term weight gain and impaired glucose metabolism

Project description:Factors predicting body weight gain and associated disturbed glucose metabolism remain to be established. Here we assessed the role of subcutaneous adipocyte lipid mobilization (lipolysis) in spontaneous long-term (>10 years) body weight changes. In two independent clinical cohorts we found that low stimulated lipolysis at baseline correlated inversely with body mass index changes over time. Disturbed lipolysis gave odds ratios of ≥4.6 for weight gain and ≥3.2 for development of insulin resistance and impaired fasting glucose/type 2 diabetes. Baseline adipose mRNA expression of a set of established lipolysis-regulating genes was lower in weight gainers.

Withdrawal of immunosuppressive therapy in stable liver transplant recipients

Project description:Complications due to long-term administration of immunosuppressive therapy increase the morbidity and mortality of liver transplant recipients. Discontinuation of immunosuppressive drugs in recipients spontaneously developing operational tolerance could substantially lessen this burden. However, this strategy results in the development of rejection in a high proportion of recipients who require lifelong immunosuppression. Thus, there is a need to identify predictive factors of successful drug withdrawal and to define the clinical and histological outcomes of operationally tolerant liver recipients. Methods. We enrolled 102 stable liver transplant recipients in an immunosuppression withdrawal trial in which drugs were gradually discontinued over a 6-9 month period. Patients with stable graft function and no signs of rejection in a liver biopsy conducted 12 months after cessation of immunosuppressive therapy were considered operationally tolerant. Results. Out of the 98 recipients who completed the study, immunosuppression discontinuation was successful in 41 recipients and rejection occurred in 57. Rejection episodes were mild and were resolved in all cases. Development of tolerance was independently associated with time elapsed since transplantation, recipient age, and male gender. No histological damage was apparent in protocol biopsies performed after successful drug withdrawal.
