Human transcriptome analysis of acute responses to glucose ingestion reveals a role of leukocytes in hyperglycemia induced inflammation.

Project description:Objective was to examine acute gene expression responses to physiologic oral glucose ingestion in human circulating leukocytes. Microarray study of human circulating leukocytes sampled before, 1 hour after and 2 hours after glucose ingestion was performed. The present study demonstrated 36 genes which showed acute gene expression change in human leukocytes within 1 hour after glucose ingestion and suggest that leukocytes participate in the inflammatory process induced by acute hyperglycemia. A Novel Single Cell RNA-seq Analysis of Circulating Immune Cells in Late Sepsis

Project description:With the successful implementation of guidelines from the Surviving Sepsis Campaign, in-hospital mortality to sepsis continues to decrease. However, this is has not lead to completely improved outcomes after sepsis. Up to 1/3 of sepsis survivors continue to have dismal long-term outcomes and 1-year mortality. Although the pathobiology of the dismal outcomes after sepsis remains undefined, it is thought that late after sepsis individuals enter a state of pathologic myeloid activation, inducing suboptimal lymphopoiesis and erythropoiesis, with many of the downstream immune cells being dysfunctional. The goal of this study was to use single-cell RNA sequencing to perform a detailed transcriptomic analysis of non-myeloid cells to better understand the pathology of late sepsis. Our findings highlight the unique transcriptomic pattern that circulating non-myeloid cells display 14 days after sepsis. Non-myeloid leukocytes in particular reveal an endotype of inflammation, immunosuppression and dysfunction. Immunomodulatory therapies at this late time point in sepsis may be feasible through precision medicine.

Integrative ‘omic analysis of experimental bacteremia identifies a metabolic signature that distinguishes human sepsis from SIRS

Project description:Rationale: Sepsis is a leading cause of morbidity and mortality; early diagnosis and prediction of progression is difficult to determine. The integration of metabolomic and transcriptomic data in an experimental model of sepsis may be a novel method to identify molecular signatures of clinical sepsis. Objectives: Develop a biomarker panel for earlier diagnosis and prognostic characterization of sepsis patients to inform personalized clinical management and improve understanding of the pathophysiology of sepsis progression. Methods: Mild to severe sepsis, lung injury and death was recapitulated in Macaca fascicularis by intravenous inoculation of Escherichia coli. Plasma samples were obtained at time of challenge and at one, three, and five days later or time of euthanasia. Necropsy was performed and blood, lung, kidney and spleen samples were obtained. An integrative analysis of comprehensive metabolomic and transcriptomic datasets was performed to identify and parameterize a biomarker panel. Measurements and Main Results: Pathogen invasion, respiratory distress, lethargy and mortality was dose dependent. Severe infection and death were associated with metabolomic and transcriptomic changes indicative of mitochondrial, peroxisomal and liver dysfunction. Analysis of reciprocal pulmonary transcriptome and plasma metabolome data revealed an integrated host response that suggested dysregulated fatty acid catabolism resulting from peroxisome-proliferator activated receptor signaling. A representative 4-metabolite model effectively diagnosed sepsis in primates (AUC 0.966) and in two human sepsis cohorts (AUC=0.78 and 0.82). Conclusion: A model to guide early management of patients with sepsis was developed by analysis of reciprocal metabolomic and transcriptomic data in primates that diagnosed sepsis in humans.
