Ovarian Cancer prognosis: Different methylation in patients with PFS <3 years vs. >3 years

ABSTRACT: Identification of differently methylated regions of CpG islands in epithelial ovarian cancer (EOC) tissue from patients with progression free survival <3 years (worse outcome) vs. patients with PFS >3 years (good outcome, relapse free until last follow up). Patients were homogenous in regard to clinical (FIGO III/IV, serous histology, optimally resected (macroscopically tumor free), platin-taxan chemotherapy) and molecular properties (immunohistochemistry for p16, BRCA1, Ki67 and p53)).

Transcription profiling of ovarium cancer samples from patients with tumors of different stages and sensitivity to platinum chemotherapy treatment

Project description:We investigated whether prognostic information is reflected in the expression patterns of ovarian carcinoma samples. RNA obtained from seven FIGO stage I without recurrence, seven platin-sensitive advanced-stage (III or IV), and six platin-resistant advanced-stage ovarian tumors was hybridized on a complementary DNA microarray with 21,372 spotted clones. The results revealed that a considerable number of genes exhibit nonaccidental differential expression between the different tumor classes. Principal component analysis reflected the differences between the three tumor classes and their order of transition. Using a leave-one-out approach together with least squares support vector machines, we obtained an estimated classification test accuracy of 100% for the distinction between stage I and advanced-stage disease and 76.92% for the distinction between platin-resistant versus platin-sensitive disease in FIGO stage III/IV. These results indicate that gene expression patterns could be useful in clinical management of ovarian cancer.<br> KEYWORDS: clinical, FIGO stage, microarrays, ovarian cancer, platin resistance.

Expression of miRNAs in breast tumor from very youn women (<36) and women above 65 years old

Project description:miRNA expression from breast tumor tissue obtained from two groups of breast cancer (BC) patients: one group formed by women younger than 36 years and the other group by women older than 65 years. We selected breast tumor tissue samples from 21 women under 36 years old and 12 women above 65 years old, without BRCA mutation and without metastases. We selected a pool of samples in which all the subtypes were representated, in young patients as well as in older patients, to avoid expression biass. We add three samples from breast tissue of cancer-free women to minimize the age-related expression biass. Our aim was to compare miRNA expression between young women and older women, considering normal tissue as a reference.

Transcription profiling of ovarium cancer samples from patients with tumors of different stages, sensitivity to platinum chemotherapy and with no disease recurrence, recurrence or progression to evaluate models of clinical behaviour

Project description:Ovarian cancer is the leading cause of death in gynaecological malignancies in women. However, currently there are no clinical or pathologic parameters available that can reliably predict clinical outcome. In a previously published pilot study we explored the performance of microarrays in predicting clinical behaviour of ovarian tumours. For this purpose we performed microarray analysis on 20 patients and estimated that we could predict disease stage with 100% accuracy and the response to platin-based chemotherapy with 76.92% accuracy using leave-one-out cross validation techniques in combination with Least Squares Support Vector Machines (LS-SVMs). In the current study we prospectively evaluate models, built on the pilot data set, on a set of 49 new patients. Principal component analysis showed that the gene expression data from stage I, platin-sensitive advanced stage and platin-resistant advanced stage tumours in the prospective study did not correspond to their respective classes in the pilot study. Additionally, LS-SVM models built using the data from the pilot study, only misclassified one of four stage I tumours and correctly classified all 45 advanced stage tumours but this model was not able to predict resistance to platin-based chemotherapy. We discuss possible reasons for prospective failure of these models and conclude that existing results based on gene expression patterns of ovarian tumours need to be thoroughly scrutinized before this technology could be considered ready for clinical use.

Comprehensive profiling of myxopapillary ependymomas identifies a distinct molecular subtype with relapsing disease

Project description:Myxopapillary ependymoma is a heterogeneous disease with respect to histopathology and clinical outcome. However, there are no diagnostic markers or clinical parameters that reliably predict the progression-free survival of the respective patients. We analyzed 185 tumors that classified as myxopapillary ependymoma based on global DNA methylation. Of these, 46 samples have been previously analyzed in GSE65362, GSE90496 or GSE109381. Using k-means clustering, our series split up into two distinct subtypes: MPE-A occurred in younger patients (median age 27 years) and were significantly enriched with tumors demonstrating papillary morphology and MGMT promoter methylation. Half of these tumors could not be totally resected, and 85% relapsed within 10 years. On the other hand, MPE-B occurred at a median age of 45 years. They included a significantly higher number of tumors with an initial diagnosis of WHO grade II and tanycytic morphology. Importantly, patients within this subgroup had a significantly better outcome with a relapse rate of only 33% in 10 years.

X-linked recessive TLR7 deficiency in 1% of men under 60 years with life-threatening COVID-19

Project description:Autosomal inborn errors of type I IFN immunity and autoantibodies against these cytokines underlie at least 10% of critical COVID-19 pneumonia cases. We report very rare, biochemically deleterious X-linked TLR7 variants in 16 unrelated male individuals aged 7 to 71 years (mean: 36.7 years) from a cohort of 1,202 male patients aged 0.5 to 99 years (mean: 52.9 years) with unexplained critical COVID-19 pneumonia. None of the 331 asymptomatically or mildly infected male individuals aged 1.3 to 102 years (mean: 38.7 years) tested carry such TLR7 variants (p = 3.5 x 10-5). The phenotypes of five hemizygous relatives of index cases infected with SARS-CoV-2 include asymptomatic or mild infection (n=2, 5 and 38 years), or moderate (n=1, 5 years), severe (n=1, 27 years), or critical (n=1, 29 years) pneumonia. Two boys (aged 7 and 12 years) from a cohort of 262 male patients with severe COVID-19 pneumonia (mean: 51.0 years) are hemizygous for a deleterious TLR7 variant. The cumulative allele frequency for deleterious TLR7 variants in the male general population is < 6.5x10-4. We also show that blood B-cell lines and myeloid cell subsets from the patients do not respond to TLR7 stimulation, a phenotype rescued by wild-type TLR7. The patients’ blood plasmacytoid dendritic cells (pDCs) produce low levels of type I IFNs in response to SARS-CoV-2. Overall, X-linked recessive TLR7 deficiency is a highly penetrant genetic etiology of critical COVID-19 pneumonia, in about 1.8% of male patients below the age of 60 years. Human TLR7 and pDCs are essential for protective type I IFN immunity against SARS-CoV-2 in the respiratory tract.

Predicting Outcome in Follicular Lymphoma by Using Interactive Gene Pairs

Project description:Purpose: Follicular lymphoma is a common lymphoma of adults. Although its course is often indolent, a substantial proportion of patients have a poor prognosis, often due to rapid progression or transformation to a more aggressive lymphoma. Currently available clinical prognostic scores, such as the follicular lymphoma international prognostic index, are not able to optimally predict transformation or poor outcome. Experimental Design: Gene expression profiling was done on primary lymphoma biopsy samples. Results: Using a statistically conservative approach, predictive interaction analysis, we have identified pairs of interacting genes that predict poor outcome, measured as death within 5 years of diagnosis. The best gene pair performs >1,000-fold better than any single gene or the follicular lymphoma international prognostic index in our data set. Many gene pairs achieve outcome prediction accuracies exceeding 85% in extensive cross-validation and noise sensitivity computational analyses.Many genes repeatedly appear in top-ranking pairs, suggesting that they reproducibly provide predictive capability. Conclusions:The evidence reported here may provide the basis for an expression-based, multigene test for predicting poor follicular lymphoma outcomes. Keywords: Comparative genomics

Blood transcriptomes of chronic HCV patients four years after therapy-induced viral clearance

Project description:Little is known on the immune status in liver and blood of chronic HCV patients long after therapy-induced viral clearance. In this study, we demonstrate that 4 years after clearance, regulation of HCV-specific immunity in blood by regulatory T-cells (Treg) and the immunosuppressive cytokines IL-10 and TGF-β is still ongoing. Importantly, sampling of the liver 4 years after clearance shows that intrahepatic Treg are still present in all patients, suggesting that liver T-cells remain regulated. Identifying mechanisms that regulate HCV-specific memory T-cell responses after clearance is highly relevant for the development of protective vaccines, especially in patients at high-risk of reinfection. A genome-wide gene expression analysis was performed on a blood cohort of 5 chronic HCV patients. For each patient, blood was collected at 4 years after ending of HCV therapy, and blood transcriptomes was compared to the paired transcriptomes at baseline and 24 weeks after ending HCV therapy (GSE59312).

microRNA alterations associated to clinical response in advanced stage ovarian cancer patients (training set)

Project description:In order to investigate miRNA alterations associated to early relapse in ovarian cancer patients, we analyzed miRNA expression profile in a training set of 55 surgical specimens including 30 early relapsing and 25 late relapsing patients. Patients were selected on the basis of residual disease after primary surgery and time to relapse (TTR) after front-line chemotherapy. For training set, a selection of the outliers concerning TTR was made: as early relapsing were defined optimally debulked (OD) patients with a TTR< 12 months and sub-optimally debulked (SOD) patients with TTR< 6 months; late relapsing were defined OD patients with TTR> 36 months and SOD patients with TTR> 12 months.
