Exercise training markedly augments dietary weight loss-induced improvements in multi-organ system metabolic function in people with obesity and prediabetes

Project description:The additional therapeutic effects of regular exercise during a dietary weight loss program in people with obesity and prediabetes are unclear. We evaluated the effect of 10% weight loss, induced within ~5 months by calorie-restriction alone (Diet-ONLY, n=8) or calorie-restriction plus multi-modal exercise training (Diet+EX, n=8), on metabolic function in people with obesity and prediabetes. Whole-body (primarily muscle) and hepatic insulin sensitivity were 2-3 fold greater in the Diet+EX than the Diet-ONLY group, and were accompanied by increased muscle expression of genes involved in mitochondrial biogenesis, energy metabolism and angiogenesis in the Diet+EX group without any change in the Diet-ONLY group. There were no differences between groups in plasma branched-chain amino acids or markers of inflammation, and both interventions caused similar changes in the gut microbiome. These results demonstrate that adding regular exercise to a diet-induced weight loss program has profound metabolic benefits in people with obesity and prediabetes.

Project description:The purpose of this study was to conduct a randomized-controlled trial to evaluate the effect of a high-protein (HP) diet weight loss (WL) on changes in body composition, insulin sensitivity and skeletal muscle gene expression profile in postmenopausal women who were obese and randomized to one of three dietary intervention groups: 1) a weight maitenance group, 2) a weight loss group (normal protein intake, 0.8 g protein/kg body weight per day) and 3) a weight loss group (high protein intake, 1.2 g protein/kg body weight per day) and studies before and after they lost 10% of their body weight (WL groups) or a time-matched weight maintenacne period.

Study of the usefulness of human peripheral blood mononuclear cells for the analysis of metabolic recovery after weight loss (METAHEALTH-TEST)

Project description:The aim of this study is to design and validate a test, METAHEALTH-TEST, based on gene expression analysis in blood cells, to quickly and easily analyse metabolic health. This test will be used to analyse metabolic improvement in overweight/obese individuals and in metabolically obese normal-weight (MONW) individuals after undergoing a weight loss intervention and/or an intervention for improvement in eating habits and lifestyle. Obesity and its medical complications are a serious health problem today. Using peripheral blood mononuclear cells (PBMC) as an easily obtainable source of transcriptomic biomarkers would allow to deepen into the knowledge of adaptations in response to increased adiposity that occur in internal homeostatic tissues, without the need of using invasive biopsies. Moreover, if PBMC were able to reflect lipid metabolism gene expression pattern recovery as a result of weight loss, it would provide valuable information to know the efficacy of therapies aimed at weight loss and, in any case, it would allow to personalize them according to the evolution of obese patients until the desired metabolic recovery is achieved.

Project description:Obesity is associated with insulin resistance and increased intrahepatic triglyceride (IHTG) content, which are key risk factors for diabetes and cardiovascular disease. However, a subset of obese people does not develop these metabolic complications. We tested the hypothesis that MNO, but not MAO, people are protected from the adverse metabolic effects of weight gain. To this end, global transcriptional profile in adipose tissue before and after weight gain was evaluated by microarray analyses. We collected subcutaneous adipose tissue samples from MNO (n=11) and MAO (n=7) subjects before and after moderate (~6%) weight gain (total 36 samples). We evaluated the effects of weight gain on adipose tissue gene expression in both MNO and MNO subjects.We used the GeneChip Human Gene 1.0 ST array (Affymetrix, Santa Clara, CA, USA).

Gene expression profiling of human skeletal muscle in response to stabilized weight loss.

Project description:Background: Diet induced weight reduction promotes a decrease in resting energy expenditure that could partly explain the difficulty to maintain reduced body mass. Whether this reduction remains after stabilized weight loss is still controversial. The molecular mechanisms are unknown. Objective: To investigate the effect of a stabilized 10%-weight loss on resting metabolic rate, body composition and skeletal muscle gene expression profile in obese women. Design: Obese women were successively submitted to a 4-w very low-calorie diet, a 3-6-wk low-calorie diet, and a 4-wk weight maintenance program to achieve a 10% weight loss. Resting energy expenditure, body composition, plasma parameters and skeletal muscle transcriptome were compared before weight loss and during stabilized weight reduction. Results: Energy restriction caused an 11% weight loss. Stabilization to the new weight was accompanied by an 11% decrease of the resting metabolic rate normalized to the body cellular mass which was below that of lean subjects. The range of the changes in the skeletal muscle transcriptome was modest. The main regulated genes were that of slow/oxidative fiber markers which were overexpressed and the gene encoding the glucose metabolism inhibitor PDK4 which was down-regulated. The knowledge based approach, gene set enrichment analysis, identified pathways related to insulin and interleukin 6 and long term calorie restriction adaptations during weight loss. Transcription profiling of human skeletal muscle from morbidly obese patients before and after gastric bypass surgery to identify genes responsive to weight loss.

Project description:Obesity results from a chronic imbalance between energy intake and energy expenditure, with excess calories stored as fat. As such, weight loss has long been considered as a primary goal of treatment for obesity. A surgical treatment of severe obesity such as gastric bypass provides the most dramatic reductions in body weight, and a well-known effect of weight loss is an improvement in insulin sensitivity. However, the molecular mechanism underlying this association remains unclear. Thus, we profiled skeletal muscle of morbidly obese patients before and after gastric bypass surgery. Results from this project will provide global patterns of gene expression with weight loss, which help to understand the pathogenesis of obesity at the molecular level.
