Project description:Risk of hepatocellular carcinoma (HCC) remains after sustained virological response (SVR) in patients with chronic hepatitis C (CHC). Epigenetic abnormalities might be key regulators in the development of HCC. This study aimed to identify the genes involved in hepatocarcinogenesis after SVR. DNA methylation in liver tissues was compared between 21 CHC patients without HCC and 28 CHC patients with HCC, all of whom had achieved SVR. Through additional comparisons with 23 CHC patients before treatment and 10 normal livers, we found that the several genes were methylated and demethylated by HCV infection and the development of HCC after achieving SVR.

Project description:Although several dysregulated miRNAs have been reported in liver diseases of different etiologies, no genome-wide analyses of hepatic miRNAs from patients with chronic hepatitis C (CHC) of HCV genotype 3 have been reported. With the aim of determining miRNAs associated with CHC pathogenesis, we present a comprehensive catalogue of the hepatic miRNAome of CHC-infected and control liver tissues obtained using next-generation sequencing. The study design was divided into discovery and validation phases. In the initial NGS-based discovery phase, 10 liver tissues (CHC-positive: 8, controls: 2) were subjected to miRNA-sequencing, using illumina HiSeq 2000. The expression of selected deregulated miRNAs was validated using qRT-PCR in a validation cohort comprising of 123 treatment-naive CHC patients of the HCV genotype 3 and 60 healthy controls. Furthermore, a comprehensive computational workflow incorporating miRNA–mRNA interaction analysis, was established to determine the functional significance of dysregulated miRNA–mRNA pairs in CHC infection.

Project description:Approximately 50% of patients with chronic hepatitis C (CHC) have a sustained virologic response (SVR) to treatment with pegylated interferon (pegINF)-M-NM-1 and ribavirin. Non-response to treatment is associated with constitutively increased expression of IFN-stimulated genes (ISGs) in the liver. Treatment of patients with acute hepatitis C (AHC) is more effective, with SVR rates >90%. We investigated mechanisms of the different responses of patients with CHC and AHC to pegIFN-M-NM-1 therapy. We analyzed IFN signaling and ISG expression in liver samples from patients with acute hepatitis C (AHC), patients with chronic hepatitis (CHC), and individuals without hepatitis C (controls) using microarray, immunohistochemical, and protein analyses. Findings were compared with those from primary human hepatocytes stimulated with IFN-M-NM-1 or IFN-M-NM-3, as reference sets. Expression levels of 100s of genes, primarily those regulated by IFN-M-NM-3, were altered in liver samples from patients with AHC compared with controls. Expression of IFN-M-NM-3M-bM-^@M-^Sstimulated genes was induced in liver samples from patients with AHC, whereas expression of IFN-M-NM-1M-bM-^@M-^Sstimulated genes was induced in samples from patients with CHC. In an expression analysis of negative regulators of IFN-M-NM-1 signaling, we did not observe differences in expression of SOCS1 or SOCS3 between liver samples from patients with AHC and those with CHC. However, USP18 (another negative regulator of IFN-M-NM-1 signaling), was upregulated in liver samples of patients with CHC that did not respond to therapy, but not in AHC. In conclusion, differences in expression of ISGs might account for the greater response of patients with AHC, compared to those with CHC, to treatment with pegINF-M-NM-1 and ribavirin. Project description:Chronic hepatitis C (CHC) is one of the major risk factor for the progressive development of end stage liver diseases including liver cirrhosis (LC) and HCC worldwide. A deep insight into the molecular mechanism of development and progression of liver fibrosis into cirrhosis and HCC following chronic HCV infection leads to characterization of multiple cellular processes and the underlying regulatory mechanisms. Non-coding RNAs (sncRNAs) including microRNAs (miRNAs) and long non-coding RNAs (lncRNAs) are found to be the prime regulators of multiple cellular pathways. Using such tools several studies have identified myriads of differentially regulated non-coding RNAs and genes in HCV disease progression to HCC. Our study attempted to look into the integrative network of regulatory non-coding RNAs and target genes involved in HCV related HCC and thereby identify a potential diagnostic molecule as well as therapeutic target for HCC surveillance and management.

The current standard therapy for chronic hepatitis C (CHC) consists of a combination of pegylated IFN alpha (pegIFN-alpha) and ribavirin. It achieves a sustained viral clearance in only 50â??60% of patients. To learn more about molecular mechanisms underlying treatment failure, we investigated IFN-induced signaling in paired liver biopsies collected from CHC patients before and after administration of pegIFN-alpha. In patients with a rapid virological response to treatment, pegIFN-alpha induced a strong up-regulation of IFN-stimulated genes (ISGs). As shown previously, nonresponders had high expression levels of ISGs before therapy. Analysis of posttreatment biopsies of these patients revealed that pegIFN-alpha did not induce expression of ISGs above the pretreatment levels. In accordance with ISG expression data, phosphorylation, DNA binding, and nuclear localization of STAT1 indicated that the IFN signaling pathway in nonresponsive patients is preactivated and refractory to further stimulation. Some features characteristic of nonresponders were more accentuated in patients infected with HCV genotypes 1 and 4 compared with genotypes 2 and 3, providing a possible explanation for the poor response of the former group to therapy. Taken together with previous findings, our data support the concept that activation of the endogenous IFN system in CHC not only is ineffective in clearing the infection but also may impede the response to therapy, most likely by inducing a refractory state of the IFN signaling pathway. Project description:Recent studies have demonstrated that micro (mi)RNA molecules can be detected in the circulation and can serve as potential biomarkers of various diseases. This study used microarray analysis to identify aberrantly expressed circulating miRNAs in patients with type 1 autoimmune hepatitis (AIH) compared with healthy controls. Patients with well-documented and untreated AIH were selected from the National Hospital Organization (NHO)-AIH-liver-network database. They underwent blood sampling and liver biopsy with inflammation grading and fibrosis staging before receiving treatment. To further confirm the microarray data, circulating expression levels of miR-21 and miR-122 were quantified by real-time quantitative polymerase chain reaction in 46 AIH patients, 40 patients with chronic hepatitis C (CHC), and 15 healthy controls. Consistent with the microarray data, serum levels of miR-21 were significantly elevated in AIH patients compared with CHC patients and healthy controls. miR-21 and miR-122 serum levels correlated with alanine aminotransferase levels. Circulating levels of miR-21 and miR-122 were significantly reduced in AIH patients with liver cirrhosis, and were inversely correlated with increased stages of fibrosis. By contrast, levels of circulating miR-21 showed a significant correlation with the histological grades of inflammation in AIH. We postulate that aberrantly expressed serum miRNAs are potential biomarkers of AIH and could be implicated in AIH pathogenesis. Project description:miRNAs were purified from frozen plasma from injection drug users in the Baltimore Before and After the Start of Hepatitis (BBAASH) Study before, during and after acute hepatitis C infection. Acute HCV outcome of 'Clear' indicates spontaneous resolution of infection. 'Time 3' in these individuals indicates the last known viremic specimen from these individuals before resolution and 'Time 4' indicates a plasma sample greater than one month after resolution of infection. Acute HCV outcome of 'Persist' indicates progression to chronic infection and 'Time 3' and 'Time 4' samples from these individuals are plasma samples time-matched to those of the clearers with the same name. qPCR miRNA expression profiling. 22 individuals HCV negative at enrollment were profiled for miRNA abundance in plasma, before, during, and after HCV infection.

Project description:Although extensive studies have demonstrated the gene expression patterns of antigen-specific CD4+ and CD8+ T-cells during chronic hepatitis C virus (HCV) infection, the transcriptional profiles of global CD4+ and CD8+ T-cells remains unclear. In this report, we recruited 10 long-term (~20 years) treatment-naïve chronic HCV (CHC) patients and 5 healthy donors (HDs) to investigate differences in global CD4+ and CD8+ T-cells gene expression profile. Global CD4+ and CD8+ T-cells showed unique transcriptional profiles in the expression of apoptosis-related genes. We identified BCL2, PMAIP1, and CASP1 in CD4+ T-cells and IER3 and BCL2A1 in CD8+ T-cells from CHC patients as HCV-specific gene signatures. The unique apoptosis-related gene expression profilesin global CD4+ and CD8+ T-cells programmed by chronic HCV infection seemed to enhance activation-induced apoptosis, which was suffered by global CD4+ and CD8+ T-cells.
