Project description:Sepsis is defined as a life-threatening organ dysfunction caused by a dysregulated host response to infection. The immune response varies significantly based on the anatomical source of infection. This study investigates the transcriptomic profiles associated with sepsis using bulk RNA sequencing. A total of 207 PBMC samples were analyzed, including those from healthy adults, adult ICU controls, adult sepsis patients, healthy children, pediatric ICU controls, and pediatric sepsis patients. The analysis of this dataset reveals both shared and source-specific gene expression patterns, providing insights into the molecular mechanisms underlying sepsis and informing the development of targeted therapeutic strategies.

Circulating exosomes containing biomarkers and mediators of sepsis have recently received attention, but the progress has been far from optimal.ObjectivesThe present study focuses on the profiles of molecular dynamics in serum exosomes and explores the potential molecular mechanisms on serum exosomes during the process of sepsis.MethodsWe used high-performance liquid chromatography-tandem mass spectrometry and RNA-seq to detect the dynamic profiles of exosome proteins and RNAs (including mRNAs, lncRNAs and miRNAs) in serum exosomes from 3 healthy individuals and 9 septic patients at the different stages. Then integrative multiomics analyses were performed and the results were validated by qRT-PCR, LiquiChip assay and metabolomics analysis on mice subjected to cecal ligation and puncture (CLP) modeling.ResultsA total of 354 proteins, 195 mRNAs, 82 lncRNAs and 55 miRNAs were identified as differentially expressed molecules in serum exosomes from septic patients. Integrative multiomics analysis showed that exosome components were associated with cytokine storm, complement and clotting cascades, the endothelial barrier, 20S proteasome-dependent protein degradation and vitamin metabolism. Importantly, pretreatment with serum exosomes derived from mice subjected to CLP significantly restrained proinflammatory cytokine expression and alleviated tissue injury in septic mice. Further metabolomics analysis demonstrated that pretreatment with septic serum exosomes significantly affected the metabolites associated with vitamin digestion and absorption in CLP mice.ConclusionOur study for the first time describes the landscape of the molecular dynamics of serum exosomes during the development of sepsis and proposes some hypothetical molecular mechanisms by integrative multiomics analysis, which may provide helpful diagnostic and therapeutic insights for the ongoing battle against sepsis.

Project description:Sepsis is a leading cause of global mortality, characterized by organ dysfunction due to a dysregulated immune response to infection. This study aims to delineate the immune landscape in sepsis through a combined analysis of single-cell RNA sequencing (scRNA-seq) with TCR and BCR sequencing. A total of 133 samples were analyzed, including PBMCs from healthy adults, adult ICU controls, adult sepsis patients, healthy children, pediatric ICU controls, and pediatric sepsis patients. The integration of scRNA-seq with TCR and BCR sequencing revealed both common and unique immune cell populations across different infection sources and between adults and children. These findings provide insights into the pathophysiology of sepsis and suggest potential targets for personalized therapeutic approaches.

The sepsis patients are categorized by infections originating from different anatomical sites. The findings aim to support sepsis stratification and guide the development of targeted therapies based on infection sources.

Project description:Blood monocytes serve as the first line of host defense and are equipped to recognize and respond to infection by triggering an immune-inflammatory response. While most information on these cells comes from in vitro studies in humans or in vivo studies in mice, little is known about monocytes under human disease conditions. We investigated the role of monocytes during sepsis and its resolution in humans. A transcriptomal and functional analysis of blood monocytes from patients during gram negative sepsis and at recovery was performed. Monocytes from sepsis patients showed upregulation of a large number of pro-inflammatory genes and cytokines/chemokines, consistent with an ongoing systemic inflammation. However, these cells showed impairment to ex vivo endotoxin (LPS) challenge, displaying a quantitative decrease in the number of LPS-inducible genes. Moreover, they downregulated the expression of several pro-inflammatory cytokine/chemokine genes, activation marker genes and transcription factors associated with monocyte/macrophage activation, upon ex vivo LPS stimulation. Functionally, they downregulated expression of inflammatory cytokines/chemokines and antigen presentation-related molecules and functions. In contrast, genes and functions related to phagocytosis, anti-microbial activity and tissue remodeling where remained unaffected or even enhanced . Collectively, our observations suggest a genetic and functional re-programming of these cells during human sepsis progression. Understanding the molecular mechanisms which regulate this re-programming will allow to devise strategies which could modulate the response of these cells and hence, disease progression. Blood monocytes from gram-negative sepsis patients during sepsis (Sepsis) and following their recovery (Recovery/Basal) as well as healthy donor (control) were isolated. Thereafter, these cells were treated ex vivo with or without LPS for 3h and analysed for transcriptomic study.

Project description:Background: Schizophrenia (SZ) is a debilitating mental illness with uncertain etiology and challenges in early diagnosis and treatment outcomes. For the first time, we applied a multiomics techniques to explore plasma exosomal markers of SZ and underlying molecular mechanisms. Methods: Exosomes were separated and identified from ten drug-naive first-episode SZ patients and ten healthy controls. Then small RNA-seq and high-performance liquid chromatography-tandem mass spectrometry technology were used to detect the profiles of microRNAs (miRNAs) and proteomics, respectively. The integrative multiomics analysis was further performed. Results: A total of 167 differentially expressed miRNAs (DE miRNAs) were identified in plasma exosomes from drug-naive first-episode SZ patients. The potential target genes of DE miRNAs were predicted, and GO and KEGG enrichment analysis showed that they were associated with RNA catabolic process, proteasome-mediated ubiquitin-dependent protein catabolic process, etc. Proteomic analysis identified 274 differentially expressed proteins (DEPs), and DEPs were mainly enriched in immune response and some signaling pathways. The combination of Top 10 DE miRNAs/ DEPs both had good values to diagnose SZ. Importantly, miRNA-protein ceRNA networks were constructed by integrating multiomics, one consisting of 21 downregulated DE miRNAs and 21 upregulated DEPs and the other consisting of 64 upregulated DE miRNAs and 86 downregulated DEPs in SZ patients. Conclusions: Our study for the first time describes the multiomics landscape of plasma exosomes in first-episode drug-naïve of SZ, and provides novel insights into the molecular alterations of SZ. These findings hold promise for advancing diagnostic and therapeutic strategies in SZ management.

Project description:Despite decades of research, sepsis remains one of the most urgent unmet medical needs. Clinical trials in sepsis have mainly focused on targeting the inflammatory pathway, however, recent data indicate that sepsis should also be seen as a metabolic disease. Targeting metabolic dysregulations that take place in sepsis might uncover novel therapeutic opportunities to treat human sepsis patients. The role of PPARα in liver dysfunction during sepsis has recently been described, and restoring PPARα signaling proves to be successful in murine sepsis. To find out whether this therapy might also be helpful in human sepsis patients, we analyzed metabolic perturbations in liver of a porcine fecal peritonitis model. Resuscitation with fluids, antimicrobial therapy and abdominal drainage were applied to the pigs in order to mimic human clinical care. By using RNA-seq, we detected problems with PPARα signaling in the livers of septic pigs and reduced PPARα levels correlated well with disease severity. As PPARα regulates the expression of many genes involved in FA oxidation, reduced expression of these target genes concomitant with increased FFAs in plasma and ectopic lipid deposition in the liver was observed. The results obtained in pigs are in agreement with earlier observations in mice and support the potential of targeting defective PPARα signaling in the clinic.

Project description:Understanding the plasma proteome dynamics in sepsis patients is crucial for improving prognostic and therapeutic strategies, as sepsis remains a leading cause of mortality in intensive care units. In this study, 363 patients with newly diagnosed sepsis were enrolled and plasma was collected on the first and fourth day after diagnosis. Proteome analysis was performed using liquid chromatography–tandem mass spectrometry. The plasma proteome was analyzed via classical statistical analysis and machine learning to identify associations with 30-day survival. Out of 363 sepsis patients, 224 survived and 139 did not survive the 30-day period. Significant differences in the abundance of 87 proteins on day 1 and 95 proteins on day 4 were found between survivors and non-survivors. Additionally, between days 1 and 4, 63 proteins were differentially regulated between both groups. We found that protein regulations within the first days of sepsis were generally associated with a worse outcome. Statistical analysis revealed the underlying proteins to be foremost related to blood coagulation, immune response and complement activation. Complementarily, the machine learning classifier achieved an area under the receiver operating characteristic curve of 0.75 for predicting 30-day survival. The feature importance analysis highlighted additional proteins and substantiated the findings of univariate statistics. This study describes the temporal alterations of the plasma proteome and the underlying protein networks that are associated with survival. These findings enhance the understanding of sepsis pathophysiology and the identified proteins might serve as starting points for further investigations and guide the development of targeted therapeutic strategies.

Project description:This study delves into the pathophysiological underpinnings of sepsis-associated acute kidney injury (SA-AKI), a critical and often fatal complication arising from the systemic inflammatory response and immune dysregulation triggered by sepsis. By employing the cecal ligation and puncture (CLP) method to simulate sepsis-induced AKI in mice, we conducted comprehensive transcriptome profiling using RNA sequencing (RNA-seq) to explore the molecular dynamics and identify potential new mechanisms underlying SA-AKI. The comparative analysis between the CLP-induced AKI model and control subjects aims to unravel the complex interplay of genes and signaling pathways involved in kidney damage and failure during sepsis. Through this approach, our research seeks to contribute to the broader understanding of SA-AKI's molecular basis.
