Project description:Even though autoimmune diseases are heterogeneous, believed to result from the interaction between genetic and environmental components, patients with these disorders exhibit reproducible patterns of gene expression in their peripheral blood mononuclear cells. A portion of this gene expression profile reflects family resemblance rather than the actual presence of an autoimmune disease. Here we wanted to identify that portion of this gene expression pattern that is independent of family resemblance and determine if it is a product of disease duration, disease onset, or other factors. By increasing the number of autoimmune samples in our analysis and employing supervised clustering algorithms, we identified 100 genes whose expression profiles are shared in individuals with various autoimmune diseases but are not shared by first-degree relatives of individuals with autoimmune disease or by controls. Project description:Pro-inflammatory autoantigen-specific CD4+ T helper (auto-Th) cells are central orchestrators of autoimmune diseases (AIDs). We aimed to characterize these cells in human AIDs with defined autoantigens by combining human leukocyte antigen (HLA)-tetramer-based and activation-based multidimensional ex vivo analyses. In aquaporin4-antibody-positive neuromyelitis optica spectrum disorder (AQP4-NMOSD) patients, auto-Th cells expressed CD154, but proliferative capacity and pro-inflammatory cytokines were strongly reduced. Instead, exhaustion-associated co-inhibitory receptors were expressed together with FOXP3, the canonical regulatory T cell (Treg) transcription factor. Auto-Th cells responded in vitro to checkpoint inhibition and provided potent B cell help. Cells with the same exhaustion-like (ThEx) phenotype were identified in soluble liver antigen (SLA)-antibody-autoimmune hepatitis and BP180-antibody-positive bullous pemphigoid, AIDs of the liver and skin, respectively. While originally described in cancer and chronic infection, our data point to T cell exhaustion as a common mechanism of adaptation to chronic (self-)stimulation across AID types and link exhausted CD4+ T cells to humoral autoimmune responses, with implications for therapeutic targeting.

Project description:Autoimmune thyroid diseases (AITD) such as Graves' disease (GD) or Hashimoto's thyroiditis (HT) are organ-specific diseases related to complex interactions between distinct components of the thyroid ecosystem. We applied spatial transcriptomics (ST) to explore the molecular architecture and heterogeneity of different cells present in the thyroid tissue, including thyroid follicular cells (TFCs) and stromal cells such as fibroblasts and endothelial cells. We identified damaged antigen-presenting TFCs that had an upregulated CD74/MIF axis in thyroids from AITD patients. Furthermore, we discerned two main fibroblast subpopulations in the connective tissue including ADIRF+ myofibroblasts, mainly upregulated in GD, and inflammatory-associated fibroblasts (IAFs), mainly overexpressed in HT patients. We also demonstrated an increase of fenestrated PLVAP+ vessels in AITD, especially in GD. Our data unveil novel stromal and thyroid epithelial cell subpopulations that could play an essential role in the pathogenesis of AITD.

Project description:Genome-wide association studies (GWAS) have boosted our knowledge of genetic risk variants in autoimmune diseases (AIDs). Most of the risk variants are located within or near genes with immunological functions, and the majority is found to be non-coding, pointing towards a regulatory role. We have performed a cis expression quantitative trait locus (eQTL) screen to investigate whether single nucleotide polymorphisms (SNPs) associated with AIDs influence gene expression in thymus. Genotyping was performed using the Immunochip and 353 AID associated SNPs were tested against expression of surrounding genes (+/- 1 Mb) from human thymic tissue (N=42). We identified eight genes where the expression was associated with AID risk SNPs at a study-wide level of significance (P < 2.57x10-5). Five genes (FCRL3, RNASET2, C2orf74, SIRPG and SYS1) displayed cis eQTL signals also in other tissues, while for two loci (NPIPB8 and LOC388814), the eQTL signal appear to be thymus-specific. Since many AID risk variants from GWAS have been subsequently fine-mapped in recent Immunochip projects, we explored the overlap between these novel AID risk variants and the thymic eQTL regions. Moreover, we examined the functional annotation of the seven expression altering SNPs (eSNPs). Our study reveals autoimmune risk variants that act as eQTLs in thymus. We have highlighted functional variants within these genetic regions that potentially can represent causal autoimmune risk variants. Total RNA from 42 human thymic samples were obtained from children undergoing cardiac surgery.

Project description:Autoimmune hepatitis (AIH) can be challenging to distinguish from other chronic liver diseases such as metabolic dysfunction-associated steatotic liver disease (MASLD). We applied advanced proteomic analysis to paired liver and plasma samples from newly diagnosed AIH patients, MASLD patients, and healthy controls to uncover disease-specific pathways and biomarkers. We quantified 7632 liver and 556 plasma proteins, identifying significant differences in 2521 liver and 227 plasma proteins between AIH and healthy individuals. A machine learning model based on plasma proteomics accurately distinguished AIH from MASLD and healthy controls (AUC = 0.91), with validation in the UK Biobank confirming diagnostic value for several candidate biomarkers. This study highlights distinct metabolic and immune signatures in AIH and supports the utility of plasma proteomics in non-invasive disease classification.
