Analysis of blood protein markers for early detection and diagnosis of advanced adenomatous and colorectal cancer

ABSTRACT: This study constructed a CRC early screening model using blood proteomics, achieving high accuracy and sensitivity in detecting early CRC and AA. We explored colorectal cancer (CRC)-related protein biomarkers by proteome-wide differential expression analysis based on a case-control design. The blood samples form 150 colorectal cancer (CRC) cases and 50 controls were collected. The blood samples from 50 individuals with advanced adenoma were also collected and analyzed in another subproject study. Plasma protein measurements were generated using the iodo Tandem Mass Tags (TMT)-6plex quantitative proteomics. Plasma proteomics data from all samples (150 CRC cases, 50 individuals with advanced adenoma, and 50 controls) were submitted.

Real-time quantitative miRNA PCR analysis of human plasma samples; circulating miRNAs miR-34a and miR-150 associated with colorectal cancer progression

Project description:Background: Screening for the early detection of colorectal cancer is important to improve patient survival. The aim of this study was to investigate the potential of circulating cell-free miRNAs as biomarkers of CRC, and their efficiency at delineating patients with polyps and benign adenomas from normal and cancer patient groups. Methods: The expression of 667 miRNAs was assessed in a discovery set of 48 plasma samples comprising normal, polyp, adenoma, and early and advanced cancer samples. Three miRNAs (miR-34a, miR-150, and miR-923) were further examined in a validation cohort of 97 subjects divided into the same five groups, and in an independent public dataset of 40 CRC samples and paired normal tissues. Results: High levels of circulating miR-34a and low miR-150 levels distinguished groups of patients with polyps from those with advanced cancer (AUC=0.904), and low circulating miR-150 levels separated patients with adenomas from those with advanced cancer (AUC=0.875). In addition, the altered expression of miR-34a and miR-150 in an independent public dataset of forty CRC samples and paired normal tissues was confirmed. Conclusion: We identified two circulating miRNAs capable of distinguishing patient groups with different diseases of the colon from each other, and patients with advanced cancer from benign disease groups.

Project description:A better understanding of the biology of colorectal cancer (CRC) would help identifying specific markers of the disease that could be used as diagnostic and/or prognostic biomarkers or become potential therapeutic targets of intervention. In this work, we aimed to analyze the differential protein expression of paraffin-embedded tissue colorectal adenocarcinoma in comparison to the surrounded original adenoma and healthy adjacent tissue by quantitative proteomics to identify early dysregulated key proteins in the disease. Two independent quantitative 10-plex tandem mass tag (TMT) experiments were performed using separately paired FFPE paired tissue samples from six different CRC patients. After protein extraction, trypsin digestion and labeling, proteins were identified and quantified by mass spectrometry using a Q Exactive. Data analysis and bioinformatics was then performed with MaxQuant and the R program to identify proteins differentially expressed in the adenoma to adenocarcinoma transition. More than 2500 proteins were identified and quantified, with 277 and 283 significantly upregulated or downregulated proteins (expression ratio ≥ 1.5 or ≤0.67, p-value ≤ 0.05), respectively, in adenoma and/or adenocarcinoma in comparison to healthy tissue. Ten altered proteins were then selected for further analyze their role in CRC by western blot, immunohistochemistry, tissue microarrays, and ELISA using tissue and plasma samples from CRC patients, individuals with premalignant lesions (low and high grade adenomas) and healthy individuals (controls), and by loss-of-function in vitro cell-based assays using three CRC cell lines with different metastatic properties. Among them, we found SLC8A1 and TXNDC17 associated to CRC survival and liver metastasis, and whose protein levels in plasma possessed diagnostic ability of early stages of the disease.

Project description:We explored the differential methylation patterns found in cfDNA between no neoplasia (NN; individuals with no colorectal findings, benign pathologies and non-advanced adenomas) and patients with advanced neoplasia (AN; advanced adenomas and colorectal cancer) using pooled samples, for the discovery of non-invasive methylation biomarkers for CRC screening. cfDNA was extracted from serum samples and methylation measurements were assessed with the Infinium MethylationEPIC BeadChip. Data was mainly preprocessed and analyzed with R/Bioconductor packages.

Phosphorylated filamin-A at Serine 1459 from plasma-derived small extracellular vesicles as a promising biomarker for high-risk adenoma and colorectal cancer

Project description:Parallel proteomic and phosphoproteomic analyses were conducted on plasma-derived small extracellular vesicles (sEVs) obtained from healthy individuals with negative colonoscopy (N), patients with advanced precancerous polyps (high-risk adenoma; HRA), and patients with colorectal cancer (CRC). In the discovery cohort, plasma samples were processed for sEV isolation using size-exclusion chromatography. The sEV isolates were digested with mass spectrometry-grade trypsin, and the resulting peptides were quantified using the Pierce Quantitative Colorimetric Peptide Assay. Peptides from each group (n = 10) were pooled, and 100 µg of the pooled peptides were aliquoted for phosphopeptide enrichment using the ProteoExtract Phosphopeptide Enrichment TiO2 Kit. Both the enriched phosphopeptides and corresponding unenriched peptides were analyzed in parallel using label-free quantitative mass spectrometry in three technical replicates. Findings were validated in individual samples (n = 50) through immunoblot analysis in the validation cohort.

This profiling is based on the analysis of 4 donors who underwent curative large bowel resection for CRC from 1 to 19 years before (M-CRC samples), 4 disease-free carriers of mutations in the mismatch repair system genes, who are predisposed to develop HNPCC (HNPCC samples) and 4 endoscopy-negative, asymptomatic individuals (NOR samples)

Modulation of Serpin family levels detected by proteomics to monitor colorectal adenoma to carcinoma progression

Project description:Colorectal cancer (CRC) remains a major cause of cancer related-death in developed countries. The risk of death is correlated with the stage of CRC determined at the primary diagnosis and early diagnosis is associated with enhanced survival rate. Consequently, there is an interest in using proteomics technologies to identify specific markers of adenomatous polyps as well as advanced stages of CRC.This study supports the concept that serum proteins can discriminate adenoma and CRC patients from unaffected patients and highlights the value of the SERPIN family as potential biomarkers of CRC.

Identification of an intermediate signature that marks the initial phases of colorectal adenoma-carcinoma transition

Project description:The colorectal adenoma-carcinoma sequence describes the stepwise progression from normal to dysplastic epithelium and then to carcinoma; only a small proportion of colorectal adenoma (CRA) progresses to colorectal carcinoma (CRC). Presently, endoscopic intervention is used on patients with CRAs of high grade dysplasia, diameters > 1 cm, or villous components > 25% who are at higher risk than other CRA sufferers. During the process, biopsy samples were taken for conventional histological diagnosis, but poor pathomorphological sensitivity and specificity greatly limit the diagnostic accuracy. Unfortunately, there are no reliable molecular criteria available that can predict the potential development of CRA to CRC. In present study, we use microarrays to detail the global programme of gene expression underlying the gradual progress of colorectal adenoma-carcinoma sequence.

Uncovering Stage-Specific Biomarkers in Colorectal Cancer: RNA-Seq Analysis of Adenoma and Carcinoma Progression

Project description:Colorectal cancer (CRC) progresses through distinct molecular mechanisms, tumor stage playing a pivotal role in prognosis and treatment outcomes. Despite advances in CRC treatment, the lack of stage-specific biomarkers limits the potential for reliable diagnostic and treatment strategies. This study aims to identify stage-specific gene expression profiles and the molecular mechanisms through comparative transcriptomic analysis of adenoma (n = 10), carcinoma in situ (n = 8) and adenocarcinoma (n = 11) samples. Utilizing RNA-seq from FFPE samples processed with absGSEA, we identified significant cellular pathways influenced from each CRC stage. In adenocarcinoma, pathways related to transcriptional co-regulatory mechanisms and protein kinase functional activity, critical for tumor growth and metastasis, were significantly enriched compared to adenoma. While apoptotic processes and Wnt signaling pathways were prominent in carcinoma in situ compared to adenoma samples. Extracellular matrix organization pathway was significantly enriched between adenocarcinoma and carcinoma in situ. Transcriptomics data and bioinformatics analysis revealed that in adenocarcinoma cohorts, genes such as COL1A2, CEBPZ, MED10, and PAWR were overexpressed, highlighting their potential roles in advanced disease progression. Conversely, in adenoma samples, the overexpression of genes such as ARRB1, CTBP1, and CTBP2 underscores their involvement in early tumorigenesis. COL1A2 and CEBPZ were implicated in extracellular matrix remodeling and transcriptional regulation respectively, while and ARRB1 were associated with early tumorigenesis. These findings highlight the complexity of gene regulation in CRC and provide a foundation for developing stage-specific diagnostic and prognostic biomarkers, and to ultimately improve therapeutic strategies.
