Project description:The purpose of this project was to elucidate gene expression in the peripheral whole blood of acute ischemic stroke patients to identify a panel of genes for the diagnosis of acute ischemic stroke. Peripheral blood samples were collected in Paxgene Blood RNA tubes from stroke patients who were >18 years of age with MRI diagnosed ischemic stroke and controls who were non-stroke neurologically healthy. The results suggest a panel of genes can be used to diagnose ischemic stroke, and provide information about the biological pathways involved in the response to acute ischemic stroke in humans. Total RNA extracted from whole blood in n=39 ischemic stroke patients compared to n=24 healthy control subjects.

Project description:Ischemic stroke is a brain injury caused by the occlusion of blood vessels, leading to reduced blood flow, excessive oxidative stress, and the cascade of events, including the formation of the glial barrier, fibrotic scar, neuronal death, and motor impairment. Despite extensive research into stroke etiology, the specific initiation trigger for neuronal death is mainly unknown, severely limiting the development of effective treatments. Here, we demonstrate that excessive H2O2 generation is the initiating factor of ischemic stroke in the photo-thrombosis (PT) mouse and non-human primate (NHP) models, with H2O2-induced astrocytic type I collagen (COL1) playing a crucial role in the cascade of stroke pathology. Treatment with KDS12025, an H2O2-decomposing peroxidase enhancer, significantly alleviates these effects by reducing H2O2-induced COL1 in extremely low doses in mouse and NHP stroke models. In cultured astrocytes, H2O2 stimulates the production of astrocytic COL1 through post-transcriptional up-regulation of Col1 mRNA by decreasing microRNA29, leading to the death of neighboring neurons. Using matrix-assisted laser desorption/ionization mass spectrometry imaging (MALDI-MSI), we observe profound alterations in N-glycan profiles, particularly fucosylated N-glycans, which reverted to normal levels in both the KDS12025 treatment and astrocytic Col1 gene-silencing groups. The gene-silencing of astrocytic Col1 or Fut8, a key enzyme responsible for core fucosylation necessary for COL1 secretion, prevents neuronal death and motor impairment. Our findings reveal that H2O2-induced astrocytic COL1 production is critical to ischemic stroke. These findings propose that targeting H2O2, astrocytic COL1, and FUT8 could be effective therapeutic strategies for ischemic stroke, offering new avenues for treatment.

Project description:Physiological mechanisms of the pivotal hyperacute (0-24 hours) stage of stroke remain incompletely understood. Synaptic plasticity has been strongly implicated in early stages of neurodegenerative and neurodevelopmental disorders. Here, we describe distinct region-specific patterns of synaptic remodeling across the brain, arising within 1-4 hours of stroke onset. As synapses in the ischemic core decline profoundly, mildly ischemic penumbra retains synaptic structure albeit with decreased function, whereas the contralateral hemisphere shows an increase in synaptic size and function. Mechanistically, stroke leads to synaptic recruitment of NMDA-type glutamate receptors (NMDARs) in ischemic core and penumbra, and NMDAR signaling blockade triggers synaptic loss in the penumbra and abolishes contralateral synaptic strengthening. Proteomic analysis confirms synaptic enhancement in the contralateral hemisphere and reveals rapid onset of metabolic rearrangement in the penumbra, while RNAseq shows decline of synaptic gene expression in the penumbra. These findings identify brain-wide synaptic rebalancing as a potential mechanism for rapid-response homeostatic compensation in stroke, highlighting an extent of brain resilience to acute functional perturbation.

Project description:Ischemic stroke causes extensive neuronal cell death. Antithrombotic agents and rehabilitation are the primary treatment options but have limited efficacy. Therefore, to achieve radical neurological improvement, practical neuroregenerative technologies must be developed. Recently, direct chemical reprogramming using small molecules to transdifferentiate somatic cells into neurons has garnered much attention. We investigated the possibility of applying this technology for the treatment of ischemic stroke. During the acute phase of ischemic stroke, circulating monocytes increase chemotaxis and accumulate in the infarct area through the disrupted blood-brain barrier, where they differentiate into macrophages involved in inflammation and remodeling. Direct reprogramming of these monocyte-derived macrophages into neuronal cells in vivo is theoretically possible with the administration of small-molecule drugs. However, the efficacy of this neuroregenerative therapy cannot be verified because the optimal chemical compounds that convert macrophages into neurons have not yet been identified.

Project description:The high incidence, mortality, and disability rate of ischemic stroke impose huge economic burdens on patients and social health care systems.N6-methyladenosine (m6A) is one of the most extensive RNA methylation modifications in eukaryotes and participates in the pathogenesis of numerous diseases including ischemic stroke. Peripheral blood neutrophils are forerunners after ischemic brain injury and exert crucial functions.However, the underlying mechanisms of neutrophils in ischemic stroke need to be further clarified. This study aims to explore the transcriptional profiles of m6A modification in neutrophils of patients with ischemic stroke. The Arraystar Human m6A-mRNA&lncRNA Epitranscriptomic microarray analysis was performed on the peripheral blood neutrophils of 3 patients with ischemic stroke and 3 healthy controls, providing the clinical significance of m6A modification on ischemic stroke.

Project description:Prospective cohort studies and meta-analyses examining the relationship between HDL-cholesterol (C) and stroke risk are discordant and question the value of HDL-C as a marker for stroke risk prediction. However, the relationship between other metrics of HDL, function and proteome, and stroke remain unexplored. We investigated changes in HDL protein composition and function -cholesterol efflux capacity (CEC)- after acute ischemic stroke, and their relationship to long-term functional and neuronal recovery after stroke. Plasma samples were collected from healthy controls (N = 35) and from stroke patients either 24 (early, N = 35) or 96 hours (late, N = 20) after stroke onset. ABCA1 mediated CEC was measured using murine macrophages. Stroke recovery was assessed at 3 months after stroke event using the Modified Rankin Scores (MRS) and the NIH Stroke Scale (NIHSS). Proteomic analysis of HDL by parallel reaction monitoring indicated a distinct time-dependent remodeling after stroke. 15 proteins were significantly altered following stroke, with 6 proteins significantly changing between 24 and 96 h post stroke. Inflammation-related proteins (SAA1, SAA2, PON1) were increased at both time points. Macrophage CEC, consistent with inflammatory remodeling of HDL proteome, was reduced by 50% (P<0.0001) in both early and late post-stroke samples compared to the controls. Changes in 6 post-stroke significantly correlated with stroke recovery scores (P<0.05). Further, the multiple linear regression model adjusting for baseline stroke severity confirmed that these proteins that predict the stroke recovery. Changes in HDL associated proteins within the first 96 hours post stroke could be used as markers to predict functional stroke recovery.
