Pre-clinical models for prediction of immunotherapy outcomes and immune evasion mechanisms in genetically heterogeneous multiple myeloma

Project description:Multiple myeloma (MM) is a neoplasia of bone marrow (BM) plasma cells that remains largely incurable with current treatment. Advancing therapeutic discoveries has been hampered by the lack of genetically heterogeneous models of MM. To circumvent this limitation, we engineered fifteen mouse models carrying combinations of eight MM genetic drivers, which fulfilled the pathogenesis of human disease including progression from premalignant states under immune surveillance. Integrative analyses of ⁓500 mice and ⁓1,000 patients revealed a MAPK-MYC genetic pathway that regulates time to progression and immune escape mechanisms across genetically heterogeneous tumors. During progression, MYC-dependent and independent remodeling of the BM microenvironment divided MM into immune categories with predominance of selected T-cell subpopulations, which dictated immunotherapy responses. Experimental targeting of the cytotoxic or immunosuppressive T-cell states observed in refractory MM patients enhanced immunotherapy effectiveness. Our resource enabled characterization of MM cell-intrinsic and immunological traits at unprecedented levels, which will accelerate the translation of personalized immunotherapy.

Ambroxol Induced Myeloma Cell Death by Inhibiting Autophagy

Project description:Recently, newly developed drugs have significantly improved prognosis of patients with multiple myeloma (MM). However, most patients relapsed sooner or later, and thus, MM is still an incurable hematological malignancy. In addition, serious adverse events occasionally hamper to continue treatment. Exploitation of the new drugs that potentiate the anti-tumor activities and alleviate the adverse effects of the existing drugs is needed. Here, we found by drug repositioning that ambroxol hydrochloride (ambroxol) induces apoptosis of MM cells. Interestingly, turnover assay and reporter assay showed that ambroxol inhibits late stage of autophagy. Transmission Electron Microscope (TEM) observation also showed that MM cells treated with ambroxol revealed accumulation of autophagy vesicles in cytoplasm, supporting the result of inhibition of late stage autophagy. Existing anti-MM drugs demonstrate various effects on autophagy; panobinostat, a histone deacetylase inhibitor, induced autophagy while bortezomib and lenalidomide did not. Ambroxol revealed synergistic anti-myeloma effect with panobinostat because ambroxol was considered to inhibit activation of panobinostat-induced autophagy and also downregulated MCL1 expression. In KMS 11-xenograft model, ambroxol alone significantly delayed tumor growth, and synergistically enhanced panobinostat-induced inhibition of tumor growth. Interestingly, concomitant use of ambroxol with panobinostat alleviated panobinostat-induced diarrhea. Gene set enrichment analyses (GSEAs) and pathway analysis also showed that ambroxol increased expression of the genes related to autophagy inhibition and unfolded protein response. These results suggested that autophagy is a promising therapeutic target for multiple myeloma.

Transcription profiling of human hyperdiploid multiple myeloma compared to non hyperdiploid multiple myeloma

Project description:Multiple myeloma is a relatively common B-cell malignancy that is currently incurable. Certain recurrent genetic abnormalities characteristics of different genetic subtypes have been described. Hyperdiploid myeloma characterized by recurrent trisomies is the most common genetic subtypes. However little is know about it's biology. Another common genetic abnormality is chromosome 13 deletion which is also associated with inferior prognosis. This abnormality is already present at the pre-malignant MGUS stage and is clonally selected with disease progression. Although it is biologically and clinically important the molecular consequence of chromosome 13 deletion is unknown. Experiment Overall Design: Hyperdiploid myeloma was identified using FISH. The gene expression profile of hyperdiploid MM is compared to that of non-hyperdiploid myeloma to identify differentially expressed genes. Molecular heterogeneity within H-MM is analyzed using unsupervised techniques. The distinctive subgroups identified are also tested in MGUS/SMM and NH-MM. The clinical relevance of these subtypes of hyperdiploid myeloma is then analyzed by correlating with relevant clinical information. Chromosome 13 deleted and undeleted MM are identified by FISH and their gene expression profile compared to identify molecular signature.

Adipocyte-derived β-hydroxybutyrate confers metabolic resilience in multiple myeloma by stabilizing IRF4 via NAT10-mediated acetylation

Project description:<p> Multiple myeloma (MM) remains an incurable hematologic malignancy despite advances in treatment. While obesity is a well-established risk factor, the underlying metabolic mechanisms by which adipocytes contribute to disease progression remain elusive. Here, we uncover a critical metabolic crosstalk between adipocytes and MM cells that promotes MM cell survival under glucose deprivation. We show that glucose restriction activates AMPK, leading to subcellular relocalization of HSP90 and disruption of its protective interaction with the oncoprotein IRF4, thereby rendering IRF4 susceptible to TRIM21-mediated ubiquitination and proteasomal degradation. Paradoxically, the same metabolic stress stimulates adipocytes to upregulate ketogenesis and produce β-hydroxybutyrate (β-OHB). MM cells utilize β-OHB via OXCT1-mediated ketolysis, fueling NAT10-dependent acetylation of IRF4 at K87. This acetylation event restores IRF4-HSP90 binding, thereby shielding IRF4 from degradation and sustaining tumor cell survival. Genetic ablation of Hmgcs2, the rate-limiting ketogenic enzyme, in adipocytes abrogates this protective effect. Importantly, combining an AMPK activator (metformin) with either an OXCT1 inhibitor (pimozide) or a NAT10 inhibitor (remodelin) results in potent synergistic antitumor activity in vivo. Our study thus positions β-OHB as a critical metabolic adaptor that empowers myeloma cells to circumvent glucose metabolic stress and highlights a promising combination therapeutic strategy that exploits the vulnerability arising from competing metabolic dependencies.</p>
