Project description:Objectives: Secretion of extracellular vesicles (EV) and associated micro-RNAs (miR) is altered during cellular stress and may serve as biomarkers of organ injury. We hypothesized that measuring changes in urinary levels of EV and miR will predict the onset of acute kidney injury in cardiac surgery patients. Design: Predictive accuracy biomarker study performed in the cohort of the REVAKI-2 trial Setting: Single center ICU between September 2015 and September 2018 Interventions: Intravenous sildenafil citrate 12.5 mg kg-1 over 150 min or dextrose 5% at the commencement of surgery. Measurements and main results: Urine samples were collected before and 24 hours after the procedure from 93 cardiac surgery patients. Urine EV concentrations and size distribution were assessed using NanoSight. EV derivation and levels were measured using flow cytometry. Samples from 10 selected patients were sequenced to detect differentially expressed miR. Verification was performed with advanced TaqMan assays in samples from all patients. Urine EV concentrations significantly increased in patients with AKI after surgery, with the percentage of EV positive for aquaporin-2 and β1-integrin also increasing. Pre surgery podocalyxin-positive EV were significantly lower, and β1-integrin EV werehigher in patients with AKI. The levels of the former correlated with the severity of theinjury. miR-125a-5p was expressed at higher levels in urine from patients with AKI stage 2/3. Levels of miR-10a-5p decreased after surgery in AKI patients; its levels correlated with the severity of the injury. Preoperative levels of podocalyxin EVs and miR-125a-5p had moderate AKI predictive value and, in a logistic model together with ICU lactate levels, offered good (AUC = 80.9%) AKI prediction. Conclusions: Lower levels of podocalyxin-positive EV at baseline predict the severity of post-surgery AKI. Urine EV concentrations and miR expression offer excellent predictive accuracy when combined with commonly measured biomarkers.

Project description:Urothelial carcinoma (UC) is the most common tumour type in canine bladder cancer. Current diagnostic methods are technically challenging or can lack specificity, hence there is a need for novel biomarkers of UC. To this end, we analysed the microRNA (miRNA) cargo of extracellular vesicles (EVs) from urine samples of dogs with UC to identify miRNAs that could be utilised as biomarkers. Urine was fractionated using ultrafiltration combined with size-exclusion chromatography and small RNA sequencing analysis was performed on both the EV enriched and (EV free) protein fractions. A greater number of candidate miRNA biomarkers were detected in the EV fraction than the protein fraction, and further validation using droplet digital PCR (ddPCR) was performed on the EV enriched fraction of a second cohort of dogs with bladder cancer which validated three miRNAs as candidate biomarkers; miR-182, miR-221 and miR-222 as being significantly overrepresented in dogs with UC when compared with healthy dogs and dogs with urinary tract infections. Pathway analysis confirmed that these three miRNAs are involved in cancer. In addition, their potential downstream gene targets were predicted and PIK3R1, a well-known oncogene is likely to be a shared target between miRNA-182 and miRNA-221/222. In summary, this study highlights the potential of urinary EV-associated miRNAs as a source of biomarkers for the diagnosis of canine UC.

Project description:Urinary extracellular vesicle (EV)-derived microRNAs (miRNAs) have emerged as promising noninvasive biomarkers for disease prediction. However, fluctuations in urine concentration throughout the day may influence miRNA abundance and profiles, potentially limiting their clinical utility. To ensure the broader applicability of urinary miRNAs in disease prediction, it is essential that their profiles remain stable regardless of collection time. In this study, we systematically examined factors influencing urinary miRNA abundance and assessed the stability of miRNA profiles. We collected serial urine samples from healthy individuals over three days, and performed small RNA sequencing to analyze urinary miRNA profiles in relation to various urine concentration parameters. Total miRNA counts were negatively correlated with urine volume and positively correlated with urine concentration indicators, including specific gravity and creatinine levels. miRNA profiles remained stable across different times and days when samples with low miRNA counts were excluded. These findings indicate that urine dilution—primarily due to fluid intake—is a major cause of variation in urinary miRNA abundance. Our results highlight the importance of collecting adequately concentrated samples and provide foundational insights for developing standardized urine collection protocols to enhance the reliability of urinary miRNAs as biomarkers.

Project description:Urine is a remarkably complex fluid, providing a non-invasive snapshot of an individual's overall physiologic state and of the genitourinary tissues through which it passes. Urine includes both secreted proteins and proteins encapsulated in tissue-derived extracellular vesicles (EVs). To better understand the population variability and clinical utility of urine, we comprehensively profiled both the secreted and EV proteomes of 190 men, including prostate cancer patients. We demonstrate a simple protocol to enrich for prostatic proteins in urine, and show that secreted and EV proteins reflect different subcellular compartments. Urinary proteomes are accurate surrogates of tissue proteomes, but proteins secreted from cell lines are not. The urinary proteome is longitudinally stable, and accurately distinguishes malignant from benign prostatic lesions, and can risk-stratify prostate tumors. Taken together, this resource quantifies the complexity of the urinary proteome, and reveals the synergistic value of distinguishing secreted and EV proteomes for biomarker studies.

Project description:Enrolled 24 participants, including twelve renal biopsy-proven T2DN and twelve T2DM, and isolated uEV s by ultracentrifugation to perform adequate parallel microarrays for mRNAs, lncRNAs and circRNAs and sequencing for miRNAs.One hundred mL of first-morning urine was collected.All urine samples were retained and immediately transferred to the laboratory and centrifuged at 2500 x g at 4°C for 15 min using a horizontal rotor (Dynamica V14R Pro, UK) and at 17,000 x g at 4°C for 40 min using a fixed angle rotor (FA15C rotor, Dynamica, UK). Supernatants were filtered using 0.22µm PES filters (Jet Bio-Filtration, Guangzhou, China) and transferred to new centrifuge tubes and stored at -80 degrees. After overnight melting through a 4 degree, ultracentrifuged at 200,000 xg at 4°C for 2h in a fixed angle P45AT rotor (k factor= 130, CP100NX, Hitachi, Tokyo, Japan) and repeat again after resuspending the pellet in PBS. Supernatants were discarded and the pellets were washed in PBS to obtain the uEVs.

Project description:Extracellular vesicles (EVs) are lipid bound vesicles secreted by cells into the extracellular space to enable intercellular communication. Extracellular vesicle surface proteins serve as the fundamental signaling gateways, determining the function of extracellular vesicles to communicate and interact with their environment. The surface proteins of EVs and their post-translational modifications are highly valuable sources of potential disease biomarkers and therapeutic targets, making the exploration of EV surface proteome (surfaceome) in circulating biofluids highly attractive. However, due to the unique features of surface proteins (e.g. hydrophobicity and glycosylation), highly selective and in-depth profiling of EV surfaceome still faces substantial technical challenges. Herein, we developed a strategy for comprehensively profiling the EV surfaceome based on the chemical affinity isolation of EVs and in situ labeling of EV surface glycoproteins for their downstream enrichment and identification. The developed method demonstrated significant efficacy in clinical samples at multiple centers with quantification of over 4,000 unique EV surface peptides from only 1 mL of urine. By characterizing the EV surfaceome from urine samples collected from 172 individuals, including 111 prostate cancer patients and 61 non-cancer controls, a comprehensive dataset of EV surfaceome comprising 1,121 proteins was achieved, representing the largest urine-derived EV surface protein dataset to date. Notably, the changes in the EV surfaceome patterns uncovered alterations of surface protein signatures during prostate cancer development and progression, validated using urine samples from an independent cohort of 196 additional individuals, including 144 prostate cancer patients and 52 non-cancer controls. This study identified cathepsin L and integrin alpha 3 as characteristic proteins with potential as non-invasive biomarkers for the early diagnosis of prostate cancer, while fibrinogen beta chain and tetraspanin-1 emerged as key proteins for prostate cancer grading. The clinical performance underscores the innovation and practicality of this approach, establishing it as a robust tool with potential for widespread clinical application.
