Discovery of novel protein biomarkers in urine for diagnosis of prediabetes using label-free proteomics

ABSTRACT: Type 2 diabetes (T2D) often develops insidiously, and many individuals with prediabetes (preDM) remain undiagnosed. While current diagnostic methods rely on blood sampling, urine-based proteomic biomarkers offer a promising non-invasive alternative for early risk stratification. In this study, we applied label-free nano-LC-MS/MS-based proteomics to identify urinary protein biomarkers capable of distinguishing individuals with preDM from normoglycemic controls.

Robustness of urinary extracellular vesicle-derived miRNA profiles over multiple days and the impact of urine concentration

Project description:Urinary extracellular vesicle (EV)-derived microRNAs (miRNAs) have emerged as promising noninvasive biomarkers for disease prediction. However, fluctuations in urine concentration throughout the day may influence miRNA abundance and profiles, potentially limiting their clinical utility. To ensure the broader applicability of urinary miRNAs in disease prediction, it is essential that their profiles remain stable regardless of collection time. In this study, we systematically examined factors influencing urinary miRNA abundance and assessed the stability of miRNA profiles. We collected serial urine samples from healthy individuals over three days, and performed small RNA sequencing to analyze urinary miRNA profiles in relation to various urine concentration parameters. Total miRNA counts were negatively correlated with urine volume and positively correlated with urine concentration indicators, including specific gravity and creatinine levels. miRNA profiles remained stable across different times and days when samples with low miRNA counts were excluded. These findings indicate that urine dilution—primarily due to fluid intake—is a major cause of variation in urinary miRNA abundance. Our results highlight the importance of collecting adequately concentrated samples and provide foundational insights for developing standardized urine collection protocols to enhance the reliability of urinary miRNAs as biomarkers.

Size matters: Biomolecular compositions of small and large extracellular vesicles in the urine of glioblastoma patients

Project description:Glioblastoma is a fatal and devastating primary brain tumour that poses significant challenges to accurate tumour monitoring and patient treatment. Substantial efforts are underway to develop accurate non-invasive liquid biopsy methods that can routinely assess glioblastoma markers in patient body fluids. We have recently shown that patient urine offers ready access to glioblastoma biomarkers within small extracellular vesicle populations (EVs; <200 nm membrane particles) and can accurately diagnose glioblastoma and reflect changes in glioblastoma pathophysiology. However, little is known about other urinary-EV populations in glioblastoma, and their capacity as biomarkers. In this study, we compared the biochemical properties of small (< 200 nm) and large (200-1000 nm) urinary-EVs using fourier-transform infrared spectroscopy and data independent acquisition mass spectrometry and identified significantly changing molecular distributions across the two EV populations in glioblastoma states. We also determined putative glioblastoma biomarkers associated with large urinary-EV biomarkers by ranking significant proteins with a logistic regression model. Our analysis revealed seven proteins capable of perfectly distinguishing all glioblastoma patients from healthy controls (AUC=1) and accurately classifying 96.7% of all cases as glioblastomas. Many significant large urinary-EV proteins also displayed consistent trends at different glioblastoma clinical timepoints, with protein abundance shifts post-surgery that return to pre-surgical levels at recurrence. Our findings demonstrate that both EV populations facilitate easy access to glioblastoma biomarkers for clinical diagnostics. Putative urinary-EV biomarkers described here merit further validation using comprehensive sets of longitudinal cohorts of glioblastoma urine.

Quantitative proteomics and phosphoproteomics of urinary extracellular vesicles defines putative diagnosis biosignatures for Parkinson's Disease

Project description:Background: Mutations in the leucine-rich repeat kinase 2 (LRRK2) gene have been recognized as genetic risk factors for Parkinson’s disease (PD). However, compared to cancer, fewer genetic mutations contribute to the cause of PD, propelling the search for protein biomarkers for early detection of the disease. Methods: Utilizing 138 urine samples from four groups, healthy individuals (control), healthy individuals with G2019S mutation in the LRRK2 gene (non-manifesting carrier/NMC), PD individuals without G2019S mutation (idiopathic PD/iPD), and PD individuals with G2019S mutation (LRRK2 PD), we applied a proteomics strategy to determine potential diagnostic biomarkers for PD from urinary extracellular vesicles (EVs). Results: After efficient isolation of urinary EVs through chemical affinity followed by mass spectrometric analyses of EV peptides and enriched phosphopeptides, we identify and quantify 4476 unique proteins and 2680 unique phosphoproteins. We detect multiple proteins and phosphoproteins elevated in PD EVs that are known to be involved in important PD pathways, in particular the autophagy pathway, as well as neuronal cell death, neuroinflammation, and formation of amyloid fibrils. We establish a panel of proteins and phosphoproteins as novel candidates for disease biomarkers and substantiate the biomarkers using machine learning, ROC, clinical correlation, and in-depth network analysis. Several putative disease biomarkers are further partially validated in patients with PD using parallel reaction monitoring (PRM) and immunoassay for targeted quantitation. Conclusions: These findings demonstrate a general strategy of utilizing biofluid EV proteome/phosphoproteome as an outstanding and non-invasive source for a wide range of disease exploration.

Specific expression of novel long non-coding RNAs in high-hyperdiploid childhood ALL

Project description:Pre-B cell childhood acute lymphoblastic leukemia (pre-B cALL) is a heterogeneous disease involving many subtypes typically stratified using a combination of cytogenetic and molecular-based assays. These methods, although widely used rely on the presence of known chromosomal translocations, which is a limiting factor. There is therefore a need for robust, sensitive, and specific molecular biomarkers unaffected by such limitations that would allow better risk stratification and consequently better clinical outcome. In this study we performed a transcriptome analysis of 56 pre-B cALL patients to identify expression signatures in different subtypes. In both protein-coding and long non-coding RNAs (lncRNA), we identified subtype-specific gene signatures distinguishing pre-B cALL subtypes, particularly in t(12;21) and hyperdiploid cases. The genes up-regulated in pre-B cALL subtypes were enriched in bivalent chromatin marks in their promoters. LncRNAs is a new and under-studied class of transcripts. The subtype-specific nature of lncRNAs suggests they may be suitable clinical biomarkers to guide risk stratification and targeted therapies in pre-B cALL patients.
