Staphylococcus and Neisseria Tablets Improving Symptoms and Inflammation of COPD Patients through the Gut-lung Axis

ABSTRACT:

Objective To investigate the effects of Staphylococcus and Neisseria Tablets (Qipian) on the clinical symptoms, systemic inflammation, microbiota, and metabolome in patients with stable chronic obstructive pulmonary disease (COPD). Methods A randomized controlled trial (RCT) was conducted with 40 stable COPD patients (Qipian group: n=22; control group: n=18) over 12 weeks.

Microbiome profiling demonstrated that, compared to the control group, the β-diversity of the gut microbiota in the Qipian group significantly separated after treatment, with an increased relative abundance of beneficial bacteria (Limosilactobacillus and Lactobacillus) and a decrease in pathogenic bacteria (Escherichia-Shigella and Streptococcus). Similarly, the β diversity of the airway microbiota was significantly distinct, showing increased relative abundance of beneficial bacteria such as Veillonella and Prevotella, and decreased abundance of pathogens such as Streptococcus. Metabolomic profiling identified significant upregulation of L-glutamic acid and L-glutamine. Pearson correlation analysis revealed that metabolites altered by Qipian intervention (e.g., L-glutamine) were negatively correlated with the NLRP3/caspase-1 pathway, suggesting a potential mechanism for alleviating systemic inflammation through pathway inhibition. Conclusion Qipian showed a trend towards alleviating clinical symptoms in COPD patients. The underlying mechanism may involve regulating the gut microbiota, increasing the relative abundance of beneficial bacteria and reducing the relative abundance of pathogenic bacteria, thereby reshaping microbe-associated metabolite L-glutamine, and modulating pulmonary microecology via the gut-lung axis, inhibiting the NLRP3/caspase-1 inflammasome pathway, thus reducing airway and systemic inflammation and improving the condition.
