Project description:We report the analysis of nasal curettage cells by RNAseq collected at pre-symptomatic timepoints in healthy adults experimentally challenged with respiratory syncytial virus (RSV). Following inoculation, 57% of participants developed PCR-confirmed infection. Prior to viral challenge, 80 differentially expressed genes were identified that associated with susceptibility to symptomatic infection. At day 3, 87 differentially expressed genes were associated with protection. Thus, we showed that the nasal mucosa at the time of virus exposure and during the incubation phase correlate with susceptibiltiy and protection from respiratory viral infection.

Project description:Assessment of host gene expression is an emerging tool for the diagnosis of human infections. We compared nasal and blood samples for evaluation of the host transcriptomic response in children with acute respiratory syncytial virus (RSV), symptomatic and asymptomatic picornavirus (PV) infection, and virus-negative asymptomatic controls (Ctrls). RNA was extracted from nasal and blood samples and analyzed by microarray. Despite generally lower quality of nasal RNA, the number of genes detected in each sample type was equivalent. Nasal gene expression signal derived mainly from epithelial cells but also included a leukocyte contribution that was higher in samples from symptomatic children. The number of genes with increased expression in virus-infected children was comparable in nasal and blood samples, while nasal samples also had large numbers of genes with decreased expression, including many genes associated with ciliary function and assembly. Compared to symptomatic children, those with asymptomatic PV had fewer genes with increased or decreased expression in both sample types. Genes with increased expression in comparisons of symptomatic children versus Ctrls included genes associated with components of innate immunity and apoptosis. Children with RSV but not PV also had increased expression of genes related to the cell cycle. Using nested leave-one-pair-out cross-validation and supervised principal components analysis, we defined sets of genes whose expression patterns accurately classified subjects, with high area-under-the-curve values in receiver operating characteristic analysis. Our results support use of nasal samples to augment pathogen-based tests to diagnose viral respiratory infection.

Project description:The aim of this investigation was two-fold: i) to describe miRNAs involved in the immune response to Respiratory syncytial virus (RSV) in a clinical setting in order to inform further research of immune system regulation by miRNAs in RSV or other infections; ii) to discover differences in miRNA expression between disease severity groups. We have therefore profiled miRNA in cytology brushings of the nasal mucosa in infants with RSV disease, comparing them to healthy infants. miRNA microarray identified 26 differentially regulated miRNA which were subsequently analyzed by RT-qPCR.

Project description:Diagnosis of acute respiratory viral infection is currently based on clinical symptoms and pathogen detection. Use of host peripheral blood gene expression data to classify individuals with viral respiratory infection represents a novel means of infection diagnosis. We used microarrays to capture peripheral blood gene expression at baseline and time of peak symptoms in healthy volunteers infected intranasally with influenza A H3N2, respiratory syncytial virus or rhinovirus. We determined groups of coexpressed genes that accurately classified symptomatic versus asymptomatic individuals. We experimentally inoculated healthy volunteers with intranasal influenza, respiratory syncytial virus or rhinovirus. Symptoms were documented and peripheral blood samples drawn into PAXgene tubes for RNA isolation.

Samples were collected at baseline (pre-challenge; Group: CTRL) and 7 days after challenge (Group: sEF) in individuals who did not reach diagnostic criteria. In those reaching the diagnostic endpoint (typhoid fever; Group: EF) the sample on the day of diagnosis when antibiotic treatment commenced was taken. These samples were derived from the placebo arm of a recent a Single Centre, Randomised, Doubleblind, Placebo Controlled Study to Evaluate M01ZH09 in a Healthy Adult Challenge Model, Using Ty21a Vaccine as a Positive Control. Project description:Rationale: Respiratory syncytial virus (RSV) and Streptococcus pneumoniae are major respiratory pathogens. Co-infection with RSV and S. pneumoniae is associated with severe and often fatal pneumonia but the molecular basis for this remains unclear. Objectives: To determine if interaction between RSV and pneumococci enhances pneumococcal virulence. Methods: We used confocal microscopy and western blot to identify the receptors involved in direct binding of RSV and pneumococci, the effects of which were studied in both in vivo and in vitro models of infection. Human ciliated respiratory epithelial cell cultures were infected with RSV for 72h and then challenged with pneumococci. Pneumococci were collected after 2h exposure and changes in gene expression determined using qRT-PCR. Results: Following incubation with RSV or purified G protein, pneumococci demonstrated a significant increase in the inflammatory response and bacterial adherence to human ciliated epithelial cultures and markedly increased virulence in a pneumonia model in mice. This was associated with extensive changes in the pneumococcal transcriptome and significant upregulation in the expression of key pneumococcal virulence genes, including the gene for the pneumococcal toxin, pneumolysin. We show that mechanistically this is due to RSV G glycoprotein binding penicillin binding protein 1a. Conclusion: The direct interaction between a respiratory virus protein and the pneumococcus resulting in increased bacterial virulence and worsening disease outcome is a new paradigm in respiratory infection. Project description:This studies describes the transcriptional response in whole blood derived from healthy adult volunteers experimentally infected with S. Paratyphi A. Samples were collected at pre-challenge baseline (Group: CTRL), at day 7 after challenge in those participants who stayed well over 14 days following challenge (Group: suspected Enteric Fever - sEF). Participants who developed signs of enteric fever were sampled at the time of inititiation of antibiotics (Group: EF).In this group diagnosis was confirmed by blood culture positive for S. Paratyphi (SPT). Antibiotic therapy commenced at time of diagnosis or at day 14 after challenge in those who did not develop symptoms.
