Targeted Plasma Proteomics Reveals a Central Role of Upregulated TNFRSF Proteins in HIV-associated Stroke: A Cross-sectional Discovery and Longitudinal Validation Study

ABSTRACT: This study investigated circulating proteomic signatures associated with stroke in people with HIV (PWH) using a nested case–control design. Plasma samples were collected from 135 HIV-positive individuals with stroke (HIV+/Stroke+), 135 HIV-positive stroke-free controls (HIV+/Stroke−), and 135 HIV-negative stroke-free controls (HIV−/Stroke−) from the CHART cohort (Taizhou, China). A longitudinal sub-cohort of 37 HIV+/Stroke+ individuals with paired pre- and post-stroke samples was additionally included for temporal validation. All plasma samples were profiled using the Olink Target 96 Cardiovascular II and Cardiovascular III panels simultaneously, quantifying 184 non-overlapping protein biomarkers. The study aimed to identify stroke-associated proteomic signatures, characterise underlying biological pathways and protein interaction networks, and validate key proteins longitudinally.
