Baseline Plasma Proteomic Profiling Associated with Response to Immune Checkpoint Inhibitor-Based Chemoimmunotherapy in Non-Small Cell Lung Cancer

ABSTRACT: This study aimed to identify candidate plasma protein biomarkers associated with response to immune checkpoint inhibitor (ICI)-based chemoimmunotherapy in patients with advanced non-small cell lung cancer (NSCLC). Although ICI-based chemoimmunotherapy has become an important treatment strategy for advanced NSCLC, clinical responses vary considerably among patients, highlighting the need for reliable biomarkers to support treatment response assessment. Pretreatment plasma samples from 35 patients with NSCLC receiving ICI-based chemoimmunotherapy were analyzed using the Olink Target 96 Immuno-Oncology panel (v.3113), which measures 92 immune-related proteins based on the proximity extension assay (PEA) technology. Protein abundance was reported as normalized protein expression (NPX) values on a log2 scale. The cohort included 21 responders and 14 non-responders. Differential protein expression between responders and non-responders was assessed using Welch’s t-test, followed by Benjamini–Hochberg correction for multiple comparisons. Five proteins—TRAIL, FASLG, GZMA, LAP-TGF-β1, and NOS3—showed nominal differences between the two groups and were selected as candidates for exploratory investigation. However, none of the 92 proteins remained statistically significant after false discovery rate correction. These data provide an exploratory plasma proteomic profile of patients with advanced NSCLC receiving ICI-based chemoimmunotherapy and may facilitate further investigation of circulating proteins associated with treatment response.
