Plasma proteome profiling to assess human health and disease

ABSTRACT: Proteins in the circulatory system mirror an individual´s physiology. They are easy to obtain and consequently, plasma and serum are the predominant matrices for diagnostic analyses in daily clinical practice. Protein levels are generally determined using single protein immuno-assays. Mass spectrometry (MS)-based proteomics of this body fluid is challenging due to the high dynamic range of protein abundances. Here we introduce a rapid and robust single-run proteomic workflow that enables the quantitative analysis of hundreds of plasma proteomes from single finger pricks with 20 min gradients. The apolipoprotein family, inflammatory markers such as C-reactive protein, gender-related proteins and more than 40 FDA approved biomarkers are reproducibly quantified (CV<20% with label-free quantification). Furthermore, we functionally interpret a 1000 protein, quantitative plasma proteome obtained by simple peptide pre-fractionation. ‘Plasma proteome profiling’ delivers an informative portrait of a person’s health state and we envision its large-scale use in biomedicine.

Plasma proteome profiling discovers novel proteins associated with non-alcoholic fatty liver disease

Project description:Non-alcoholic fatty liver disease (NAFLD) affects 25% of the population and can progress to cirrhosis, where treatment options are limited. As the liver secrets most of the blood plasma proteins its diseases should affect the plasma proteome. Plasma proteome profiling on 48 patients with cirrhosis or NAFLD with normal glucose tolerance or diabetes, revealed 8 significantly changing (ALDOB, APOM, LGALS3BP, PIGR, VTN, IGHD, FCGBP and AFM), two of which are already linked to liver disease. Polymeric immunoglobulin receptor (PIGR) was significantly elevated in both cohorts with a 2.7-fold expression change in NAFLD and 4-fold change in cirrhosis and was further validated in mouse models. Furthermore, a global correlation map of clinical and proteomic data strongly associated DPP4, ANPEP, TGFBI, PIGR, and APOE to NAFLD and cirrhosis. DPP4 is a known drug target in diabetes. ANPEP and TGFBI are of interest because of their potential role in extracellular matrix remodeling in fibrosis.

Plasma proteome profiling to detect and avoid sample-related biases in biomarker studies

Project description:Blood-based protein tests inform medical decision-making, but despite major investments, few new biomarkers reach the clinic. Plasma and serum are rich sources of information about an individual’s health state and mass spectrometry (MS)-based proteomics now allows highly specific and quantitative read-out of the plasma proteome. Here we employ plasma proteome profiling to define marker panels assessing the quality of plasma and serum samples and the likelihood that suggested biomarkers are instead artifacts related to sample handling and processing. We acquired reference proteomes of erythrocytes, platelets, plasma and whole blood of 20 individuals (>6000 proteins), and compared serum and plasma proteomes. Based on spike-in experiments we defined a panels of contamination-associated proteins, many of which have been reported as biomarker candidates. We provide sample preparation guidelines and an online resource (www.plasmaproteomeprofiling.com) to assess overall sample-related bias in clinical studies and to prevent costly miss-assignment of biomarker candidates.

Furthermore, ANPEP and TGFBI have potential roles in extracellular matrix remodeling in fibrosis. Thus, Plasma Proteome Profiling can identify potential biomarkers and drug targets in liver disease.

Fetal transcripts in maternal blood

Project description:The discovery of fetal mRNA transcripts in maternal circulation holds great promise for noninvasive prenatal diagnosis. To identify potential fetal biomarkers, we studied whole blood and plasma transcripts common to term pregnant women and their newborns but reduced or absent in the postpartum mothers. In whole blood, 157 potentially-fetal transcripts were identified. RT-PCR confirmed the presence of specific transcripts, SNP analysis confirmed the presence of fetal transcripts in maternal circulation. Comparison of whole blood and plasma samples from the same women suggested that placental genes are more easily detected in plasma. We conclude that fetal and placental mRNA circulates in the blood of pregnant women. [I] We profiled whole antepartum (A), postpartum (P), and umbilical cord (U) blood samples from each of 9 mothers and their 10 newborns (1 set of twins, denoted as a and b after the sample names). [II] We also profiled plasma samples (A, P, and U) from three of those mothers to allow for a direct comparison between blood and plasma.

Mechanisms preserving insulin action during high dietary fat intake in healthy men

Project description:Prolonged intervention studies investigating molecular metabolism are necessary for a deeper understanding of dietary effects on health. Here we provide mechanistic information about metabolic adaptation to fat-rich diets. Healthy men ingested saturated (SFA) or poly unsaturated (PUFA) fat-rich diets for six weeks during weight maintenance. Hyperinsulinemic clamps combined with leg balance technique revealed unchanged peripheral insulin sensitivity, independent of fatty acid type. Both diets increased fat oxidation potential in muscle. Hepatic insulin clearance increased, while glucose production, de novo lipogenesis and plasma triacylglycerol decreased. High fat intake changed the plasma proteome in immune-supporting direction and the gut microbiome displayed changes at taxonomical and functional level with PUFA. In mice, eucaloric feeding of human PUFA and SFA diets lowered hepatic triacylglycerol content compared to low-fat fed control mice, and induced adaptations in the liver supportive of decreased gluconeogenesis and lipogenesis. Intake of fat-rich diets thus induces extensive metabolic adaptations enabling disposition of dietary fat without metabolic complications.

Glucose-regulated and drug perturbed beta-cell phosphoproteome reveals molecular mechanisms controlling insulin secretion

Project description:Insulin-secreting beta cells play an essential role in maintaining physiological blood glucose levels, and their dysfunction leads to the development of diabetes. To elucidate the signaling events regulating insulin secretion, we applied a recently developed proteomics and phosphoproteomics workflow. We quantified the time-resolved phosphoproteome of beta cells following their exposure to glucose and in combination with small molecule compounds that promote insulin secretion. The quantitative phosphoproteome of 30,000 sites clustered into three main groups in concordance with the modulation of three key signaling pathways. In addition to their differential modulation of key cellular kinases, all compounds affected pathways regulating the cell cycle. A high-resolution time course revealed key novel regulatory sites in early and late insulin secretion, and unexpected connections to epigenetic regulation of gene expression. Our comprehensive and multi-parametric phosphoproteomics study reveals that control of insulin secretion is embedded in an unexpectedly broad and complex range of cellular functions.

Loss of H3K9 tri-methylation alters mitotic chromosome compaction and transcription factor retention during mitosis

Project description:Recent studies have shown that repressive chromatin machinery, including DNA methyltransferases (DNMTs) and Polycomb Repressor Complexes (PRCs), bind to chromosomes throughout mitosis and their depletion results in increased chromosome size. Enzymes that catalyse H3K9 methylation, such as Suv39h1, Suv39h2, G9a, GLP are also retained by mitotic chromosomes. Surprisingly however, mutants lacking H3K9me3 have unusually small and compact mitotic chromosomes that are associated with increased H3S10ph and H3K27me3 levels. Chromosome size and centromere compaction in these mutants was rescued by providing exogenous Suv39h1, or inhibiting Ezh2 activity. Quantitative proteomic comparisons of native mitotic chromosomes isolated from wildtype versus Suv39h1,2 double null ESCs revealed that H3K9me3 was essential for the efficient retention of bookmarking factors such as Esrrb. These results highlight an unexpected role for repressive heterochromatin domains in preserving transcription factor binding through mitosis, and underscore the importance of H3K9me3 for sustaining chromosome architecture and epigenetic memory during cell division.

Expression profiling in extracellular miRNAs (plasma miRNAs) of CML patients after imatinib discontinuation (STOP-IM)

Project description:An approximately 40% of chronic myeloid leukemia (CML) patients who discontinued imatinib (IM) therapy maintained undetectable minimal residual disease (UMRD) for more than one year (STOP-IM). We set out to examine plasma miRNAs expression in CML patients who could discontinue IM, to seek the possible distinguishable biomarker in STOP-IM CML patients. We compared CML patients who sustained UMRD for more than one year after discontinuation of imatinib (STOP-IM group) with healthy volunteers (controls). In 7 patients who had discontinued IM with sustained UMRD for more than 6 months (STOP-IM group), samples were collected when IM was stopped. Seven healthy volunteers served as control. Plasma samples were harvested form 2ml of whole blood. Isolation of total RNA was performed using the mirVana PARIS kit (Ambion, Austin, TX, USA). The expression profile of miRNAs was determined using the Human Taqman miRNA Arrays A (Applied Biosystems). Synthetic ath-miR-159 (Hokkaido System Science, Hokkaido, Japan) were used as a control. QRT-PCR was carried out on an Applied Biosystems 7900HT thermal cycler using the manufacturerâ??s recommended program. With the use of SDS2.2 software and Data Assist (Thermo Fisher Sciences), the expression of plasma miRNAs was calculated based on cycle threshold (Ct) values normalized by those of ath-miR-159, which was spiked in each plasma sample. Data analysis was done using GeneSiferâ?? software (Perkin Elmer, Waltham, MA, USA). The Benjamini-Hochberg algorithm was used for estimation of false discovery rates.

Plasma proteome profiling reveals global and specific changes of inflammatory and lipid homeostasis markers after bariatric surgery

Project description:Obesity-related diseases now affect half of the global population, and bariatric surgery is one of the few interventions with long lasting weight loss and cardio-metabolic effects. Here, we investigated the effect of Roux-en-Y gastric bypass surgery on the plasma proteome, hypothesizing that specific proteins or protein patterns may serve as key mediators and markers of the metabolic response. We performed MS-based proteomics on two longitudinal studies encompassing 47 morbidly obese patients, generating quantitative information on more than 1700 proteins. A global correlation matrix incorporating about 200,000 relationships revealed functional connections between proteins and assigned them to physiological processes. The main classes of significantly altered proteins were markers of systemic inflammation and those involved in lipid metabolism, and our data highlight robust correlative and anti-correlative behavior to each other and to clinical parameters. A group of inflammation-related proteins showed distinct inverse relationships to proteins consistently associated with insulin sensitivity.
