An RNA signature for tuberculosis risk in pregnant women: a prospective cohort study from Pune, India

Project description:Background: Pregnant and postpartum women are at high risk of developing active tuberculosis (TB), but transcriptional TB studies have excluded pregnant women. We identified differentially expressed genes (DEGs) in pregnant women who did and did not progress to active TB. Methods: We followed a cohort of pregnant Indian women with TB infection for one year postpartum, collecting blood at study entry, 6 weeks postpartum and active TB diagnosis. A prospective signature of risk was identified by comparing whole blood RNA sequencing data from women who developed active TB postpartum (cases) with those who remained healthy (controls). Results: We identified 9 cases and matched them to 18 controls by HIV status and gestational age. A gene set of risk was identified: Expression of KCNIP4 > 2.2 log CPM and S1PR4 < 7.3 log CPM indicated a high probability of developing active TB postpartum. SF3B4 (>4.3 log CPM) and PGAM1 (>6.6 log CPM) correctly classified postpartum cases and controls. Both pairs displayed high accuracy (AUC >0.9) and were unique from 36 published TB signatures.Conclusions: We identified two genes that prospectively differentiated pregnant women who developed active TB postpartum from those who did not. If validated, this signature could be useful in targeted TB prevention programs.

Retrospective and prospective validation of a 33-gene signature to predict recurrence of lung cancer after surgery.

Project description:We developed a 33-gene signature that is strongly correlated to the time to recurrence in non-small cell lung cancer (NSCLC). The signature was validated retrospectively in 5 cohorts of 972 NSCLC patients and in one prospective study of 111 NSCLC Stage IA patients. In all cohorts, and all stages of the disease, the signature identified a rare, aggressive tumor type that had a high proportion of recurrence after surgery and a median survival of 35 months (95% C.I.: 19-58). This tumor type forms a separate cluster in an analysis of the expression of the 33 genes in patient tumors. The signature is associated with cellular processes required by rapidly growing and spreading tumors: cell migration and invasion, vascularization, and response to hypoxia. The signature also identifies patients with good prognosis (median survival 114 months, (95% C.I.: 85-160), and intermediate prognosis (median survival 61 months (95% C. I.: 50-73). The signature is quite robust and works on tumor samples archived in RNAlater, Tissue-Tek, or formalin-fixed and paraffin embedded. 156 samples -------------------------------- *** Submitter has not provided information such as time to recurrence. Project description:PURPOSE: The IRSN-23 model uses DNA microarray analysis of tumor tissue to personalize patients into highly sensitive chemotherapy (Gp-R) or less sensitive (Gp-NR) based on the expression of immune-related genes. This study assessed the reproducibility of the IRSN-23 in prospective independent validation sets and investigated its clinical significance and impact on breast cancer subtype classification. METHODS: Tumour tissues were collected from 146 breast cancer patients undergoing preoperative chemotherapy (paclitaxel-FEC) ± trastuzumab in Osaka University Hospital (OUH). Patients were classified into Gp-R or Gp-NR using IRSN-23, and the model was examined for its ability to predict the pathological complete response (pCR). RESULTS: In the OUH prospective dataset, the pCR rate was significantly higher in the Gp-R group (29·3% (11/41)) than in the Gp-NR group (1·4% (1/71)) not using trastuzumab (P = 1·70E-5). CONCLUSION: This study provides prospective validation of the IRSN-23 in predicting chemotherapy efficacy, demonstrating a high degree of reproducibility.
