PURPOSE: Diagnostic trial to study the feasibility of lymph node mapping and sentinel lymph node biopsy in patients who have stage I, stage II, or stage III colorectal cancer.

Spatial and Single Cell Mapping of Human Lymph Node Disease

Project description:To determine the cellular and molecular basis of Castleman Disease (CD), we analyze the spatial proteome of spatial proteome and transcriptome of 22 cases of Unicentric CD, idiopathic Multicentric CD, HHV8-associated MCD, and reactive lymph nodes. CD shows increased stromal cells that form unique microenvironments. Interaction of activated follicular dendritic cell (FDC) cytoplasmic meshworks with mantle-zone B cells is associated with B-cell activation and differentiation. CXCL13+ FDCs, PDGFRA+ T-zone reticular cells (TRC), and ACTA2-positive perivascular reticular cells (PRC) were the predominant source of increased VEGF expression and IL-6 signaling. MCD is characterized by increased TRC while UCD shows increased B-reticular cells (BRC). VEGF expression by FDCs is associated with peri-follicular neovascularization. FDC, TRC and PRC of CD activates JAK-STAT, TGFβ, and MAPK pathways via specific ligand-receptor interactions. Here, we show that stromal-cell activation and associated B-cell activation and differentiation, neovascularization and stromal remodeling underlie CD.

PURPOSE: This phase II trial is studying how well lymph node mapping during surgery together with sentinel lymph node analysis and blood testing work in detecting and predicting early micrometastases in patients with colorectal cancer.

Identification and mapping of human lymph node stromal cell subsets by combining single-cell RNA sequencing with spatial transcriptomics.

Project description:Lymph node stromal cells (LNSCs) have a crucial immunomodulatory function, but their heterogeneity in human is incompletely understood. Here, we report the single cell RNA sequencing (scRNA-seq) of 12’000 LNSCs isolated from a human lymph node (LN). This study comprehensively defines the gene signatures of 10 fibroblast subtypes: CCL21+SC, CCL19+SC, CD34+CXCL14+SC, pericytes, DES+SC, LAMP5+SC, NR4A1+BCAM+ SC, HLA-DR+SC, SEPT4+SC and GLDN+SC. To explore the heterogeneous stromal compartment within the complex LN tissue architecture, we integrated the scRNA-seq profiles of the identified LNSC subsets with a publicly available human spatial transcriptomic LN dataset and predicted their location within the complex LN tissue architecture. Each LNSC subtype was spatially restricted to specific LN regions, indicating different LNSC-lymphocyte interactions which was further investigated using NicheNet. The positioning of distinct LNSC subtypes in different LN regions sets the stage for future research on the relationship between LNSC-specific niches and immunomodulatory function during health and disease.

Project description:Castleman disease (CD) is a rare hematologic disorder involving inflamed lymph nodes and distinct histological features. Unicentric Castleman disease (UCD) and idiopathic multicentric Castleman disease (iMCD) are of particular interest as their etiologies are poorly understood and treatment options are limited. As iMCD patients, and occasionally UCD patients, also present with a cytokine storm, we sought to investigate if the cytokine release from iMCD and UCD subtypes have pathogenic origins as observed in human herpesvirus 8 MCD (HHV-8-MCD). To identify potential active viruses that may be contributing to immune system dysregulation in Castleman Disease tissues we conducted bulk RNA sequencing on FFPE preserved lymph node tissue from patients with various phenotypes including Idiopathic multicentric Castleman disease (n = 9), systemic lupus erythematosus (n =3), diffuse large B cell lymphoma (n =5), and reactive lymph nodes with inconclusive pathology (n = 7).
