Project description:Genomic profiling can provide prognostic and predictive information to guide clinical care. Biomarkers that reliably predict patient response to chemotherapy and immune checkpoint inhibition in gastric cancer are lacking. In this retrospective analysis, we use our machine learning algorithm NTriPath [Park, Sunho et al. “An integrative somatic mutation analysis to identify pathways linked with survival outcomes across 19 cancer types.” Bioinformatics (2016): 1643-51. doi:10.1093/bioinformatics/btv692] to identify a gastric-cancer specific 32-gene signature. Using unsupervised clustering on expression levels of these 32 genes in tumors from 567 patients, we identify four molecular subtypes that are prognostic for survival. We then built a support vector machine with linear kernel to generate a risk score that is prognostic for five-year overall survival and validate the risk score using three independent datasets. We also find that the molecular subtypes predict response to adjuvant 5-fluorouracil and platinum therapy after gastrectomy and to immune checkpoint inhibitors in patients with metastatic or recurrent disease. In sum, we show that the 32-gene signature is a promising prognostic and predictive biomarker to guide the clinical care of gastric cancer patients and should be validated in a prospective manner.

Project description:Objective: Develop a genomic assay to assess prognosis and guide therapy for gastric cancer patients.Design: This was a retrospective study of multiple, independent cohorts of gastric cancer patients. We performed a pan-cancer analysis of somatic mutations found in 6,681 patients with 19 different cancers using our machine learning algorithm NTriPath to identify a gastric cancer-specific gene signature. We applied this signature to a 567-patient cohort to establish genomic-based molecular subtypes and then used a support vector machine to build a molecular subtype-based risk-scoring model. We evaluated the prognostic capability of the model and investigated whether molecular subtypes predicted response to adjuvant chemotherapy and immune checkpoint blockade. Results: We identified a 32-gene signature that categorized gastric cancer patients into four subtypes that were independently associated with overall survival and predicted response to therapy. The 5-year overall survival of Group 1 patients who received adjuvant 5-fluorouracil and platinum was significantly worse than patients who did not receive therapy (hazard ratio (HR), 6.80 (95% CI, 1.46-31.6), P=0.015). Group 3 patients who received therapy did significantly better than those who did not (HR, 0.28 (95% CI, 0.08-0.96), p=0.043). The subtypes also predicted response to immune checkpoint inhibitors in patients with metastatic or recurrent disease. Groups 1 and 3 patients had significantly higher response rates (44-100%) than patients in Groups 2 and 4 (0 to 13%;P < 0.01).Conclusions: The 32-gene signature is a promising prognostic and predictive biomarker to guide the use of adjuvant chemotherapy and immune checkpoint inhibitors in gastric cancer patients

This study identified predictive gene signatures in primary pT1 bladder cancer with BCG immunotherapy. Fourty-Eight patients with primary pT1 bladder cancer treated with BCG immunotherapy were used. Microarray gene expression analysis of the 48 primary bladder cancers was carried out. Predictive gene signatures were individually selected based on the recurrence and progression status. Among 43,148 unique genes, 424 and 287 candidate predictive genes that could predict recurrence and progression, respectively, were selected. Time to recurrence or progression was shorter for patients with poor-predictive gene signatures than good-predictive gene signatures (log-rank test, p <0.001, respectively). Validation of predictive gene signatures with RT-PCR was nearly identical to those of microarray (log-rank test, p <0.05, respectively). In multivariate regression analysis, predictive gene signatures were the only independent predictors of recurrence (HR 3.38, p = 0.048) or progression (HR 10.49, p = 0.048) in validation cohorts. The quality and integrity of the RNA were confirmed by agarose gel electrophoresis and ethidium bromide staining, followed by visual examination under ultraviolet light. Five-hundred nanograms of total RNA were used for labeling hybridization according to the manufacturerâ??s protocol (Illumina HumanWG-6 BeadChip, version 2, Illumina, Inc., San Diego, CA, USA). Arrays were scanned with an Illumina Bead Array Reader confocal scanner (BeadStation 500GXDW; Illumina, Inc.) according to the manufacturer's instructions. Project description:Relapse and metastatic progression is a frequent event in colorectal cancer patients detected at early stages. The risk of recurrence requires the development of new biomarkers to correctly predict biological behavior of early stage II and stage III patients and their response to adjuvant chemotherapy. Here, we combined the proteomic quantification of secreted proteins involved in metastasis with a transcriptional analysis to develop a risk score algorithm based on the expression of six genes (SEC6). The SEC6 signature was predictive of survival and recurrence for stage II and III patients in four different datasets including a total of 1534 patients and was also associated with deficient mismatch repair, CpG-island methylator positive status and BRAF mutation. SEC6 was also predictive of beneficial or detrimental effects from 5-Fluorouracil-containing regimes and the improved response to more aggressive chemotherapies based on FOLFOX and FOLFIRI. In summary, the SEC6 risk-score algorithm may constitute a new tool for decision-making in colorectal cancer management.

Project description:Importance: Severe alcoholic hepatitis (AH) is a highly lethal disease with a 3-month mortality up to 50%. Corticosteroids are the current standard of care although nearly 40% of the patients do not respond and accurate pre-treatment predictors of survival are lacking. Objective: To develop a prognostic score based on molecular and clinical variables before initiation of corticosteroids. Design, Setting, and Participants: Gene expression profiling of fixed liver biopsy obtained for diagnosis of severe AH assessed in 3 independent cohorts (1 prospective cohort for prognostic gene signature and integrative score derivation, and 2 retrospective cohorts for validation). Samples were obtained from 4 European and 1 US medical centers between July 2006 and February 2015. Exposures: Biopsy proven severe AH patients treated with corticosteroids. Main Outcome Measures: Identification of a gene signature combined with a validated clinical index to develop an integrative prognostic score of survival without death or liver transplantation in a derivation cohort (n=71). Prognostic performance of the score was validated in an independent multi-center validation cohort 1 (n=48). Finally, the score was implemented in an FDA-approved clinical diagnostic platform (NanoString) and tested in another independent validation cohort 2 (n=20). Results: A prognostic score, integrating a 123-gene signature and the model for end-stage liver disease (gs-MELD score), was defined in the derivation cohort, in which poor- and good-prognosis patients were discriminated with a cut-off value of 2.66 at 3 months (event-free survival rates of 32% vs. 76%, respectively, p<.001) and 6 months (26% vs. 65%, respectively, p<.001). In the validation cohort 1, the score similarly discriminated poor- and good-prognosis patients at 3 months (43%, [95% CI, 26%-70%] vs. 96% [95% CI, 89%-100%], respectively, p<.001, c-index of 0.83 [95% CI, 0.66-0.99]) and 6 months (34% [95% CI, 18%-61%] vs 84% [95% CI, 72%-100%], respectively, p<.001, c-index of 0.80 [95% CI, 0.66-0.94]), and outperformed other existing clinical indices. The NanoString-based gs-MELD score remained predictive of 3- (p=.03) and 6-month (p=.009) even-free survival in the validation cohort 2. Conclusion and Relevance: The gs-MELD score, incorporating clinically applicable gene signature test and the MELD score, enables pre-treatment survival prediction in severe AH patients.

Project description:Medullary breast cancers (MBC) display a basal profile, but a favorable prognosis. We hypothesized that a previously published 368-gene expression signature associated with MBC might serve to define a prognostic classifier in basal cancers. We collected public gene expression and histoclinical data of 2145 invasive early breast adenocarcinomas. We developed a Support Vector Machine (SVM) classifier based on this 368-gene list in a learning set, and tested its predictive performances in an independent validation set. Then, we assessed its prognostic value and that of six prognostic signatures for disease-free survival (DFS) in the remaining 2034 samples. The SVM model accurately classified all MBC samples in the learning and validation sets. A total of 466 cases were basal across other sets. The SVM classifier separated them into two subgroups, subgroup 1 (resembling MBC) and subgroup 2 (not resembling MBC). Subgroup 1 exhibited 71% 5-year DFS, whereas subgroup 2 exhibited 50% (p=9.93E-05). The classifier outperformed the classical prognostic variables in multivariate analysis, conferring lesser risk for relapse in subgroup 1 (HR=0.52, p=3.9E-04). This prognostic value was specific to the basal subtype, in which none of the other prognostic signatures was informative.
