Project description:We combined a mechanism-based and a cell-based drug screening coupled to a computational analysis, which allowed to identify potential drugs and metabolic pathways relevant for the pathophysiology of nephropathic cystinosis. This result was achieved by comparing gene-expression signature of lead compounds that share common mechanisms of action or that involve similar pathways with the disease gene-expression signature revealed by RNA-seq.

Drug repurposing via ultra-high throughput 3D spheroids screening in rare cancer treatment

Project description:Rare cancers comprise over 200 distinct types accounting for 20–25% of U.S. cancer diagnoses yet receive limited therapeutic investment due to small patient populations and resource constraints. Drug repurposing offers a viable strategy, and large-animal models such as minipigs provide physiologically realistic platforms for preclinical evaluation. However, scalable systems for systematically screening large drug libraries in rare cancer models remain unavailable. We developed an integrated pipeline combining a minipig spinal cord glioma (SCG) model with miniaturized ultra-high-throughput screening (uHTS). Primary SCG cells were cultured in 3D platforms for rapid screening of bioactive compounds. Lead candidates underwent validation through 3D spheroid assays, in vivo xenografting, RNA sequencing, immunohistochemistry, and histological analysis. Human SCG tissue samples and population-level datasets assessed translational relevance. uHTS screening identified Clofoctol as a lead compound with potent glioma growth inhibition in both primary SCG cells and xenograft models. Mechanistic studies revealed pleiotrophin (PTN) as a key target, with confirmatory expression patterns in human SCG samples, establishing translational validity. Integration of large-animal cancer models with drug-repurposing pipelines and uHTS platforms is feasible and effective for rare cancer drug discovery. This approach successfully identified Clofoctol, an FDA-approved compound with therapeutic potential for spinal cord gliomas and establishes a scalable model applicable to other orphan cancers.

Drug Repurposing Using Molecular Network Analysis Identifies Janus Kinase as Targetable Driver in Necrobiosis Lipoidica

Project description:Drug repurposing is an attractive strategy for therapy development, particularly in rare diseases where traditional drug development approaches may be challenging due to high cost and small numbers of patients. Here we used a novel drug identification and repurposing pipeline to identify candidate targetable drivers of disease and corresponding therapies through application of causal reasoning using a combination of open-access resources and transcriptomics data. We optimized our approach on psoriasis as a disease model, demonstrating the ability to identify known and novel molecular drivers of psoriasis and link them to current and emerging therapies. Application of our approach to a cohort of tissue samples of necrobiosis lipoidica, an unrelated, rare, and to date molecularly poorly characterized cutaneous inflammatory disorder, identified a unique set of upstream regulators, particularly highlighting the role of IFNG and the JAK-STAT pathway as a likely driver of disease pathogenesis and linked it to JAK inhibitors as potential therapy. Analysis of an independent cohort of NL samples validated these findings with the overall agreement of drug matched upstream regulators above 96%. These data highlight utility of our approach in rare diseases and offer a novel opportunity for drug discovery in other rare diseases in dermatology and beyond.

Drug repurposing strategy reveals Cobimetinib as a promising disease-modifying drug for Osteoarthritis

Project description:Osteoarthritis (OA) is a common disabling disease for which no effective pharmacological therapy exists. The progression of osteoarthritis is characterized by the loss of homeostasis in the cartilage. Since in the early stages of the disease, a phenotypic switch is seen in which articular chondrocytes become hypertrophic and promote degradation of the cartilage extracellular matrix, targeting this phenomenon might be the key to developing a disease-modifying osteoarthritis drug (DMOAD). In this study, the signature reversing principle combined with a drug repurposing strategy was used to accelerate the identiﬁcation and validation of potential DMOADs. Starting from single-cell transcriptomic analysis of patient-derived cartilage, we identified 6 drugs predicted to reduce chondrocyte hypertrophy. Subsequent in vitro evaluation in human chondrocytes and cartilage explants demonstrated that Cobimetinib, a MEK1/2 inhibitor, reduced chondrocyte hypertrophy-related and catabolic gene expression, such as SPP1, COL10A1, MMP13 and ADAMTS5, while promoting collagen type 2 and aggrecan gene expression. Finally, single-cell RNA sequencing performed on osteoarthritic cartilage explants exposed to Cobimetinib ex vivo confirmed the anti-hypertrophic effect of Cobimetinib on hypertrophy-related gene expression and additionally showed a deletion of chondrocytes with a fibrotic phenotype. This study is a proof of concept that omics data together with a drug repurposing strategy accelerate drug discovery in diseases like osteoarthritis and could quickly lead to many new therapeutic roles for safe and effective drugs.

Unraveling Novel Drug candidate for ALS through an Integrative Network Analysis of approved Drug-Gene Expression Signatures in Human Pluripotent Stem Cell-Based Model

Project description:Given the high attrition rates, substantial costs and slow pace of new drug discovery and development, repurposing of 'old' drugs to treat both common and rare diseases is increasingly becoming an attractive proposition. In this study, we evaluated the potential of using human pluripotent stem cells to identify repurposable drug candidates. For this purpose, we treated derivatives from human embryonic stem cells with 50 marketed drugs and we annotated the induced molecular changes by RNA deep sequencing. Focusing on genes previously implicated in monogenic diseases, we identified drugs capable of modulating the expression of PLP1, PMP22, LMNB1 and SQSTM1 genes known to be involved in Pelizaeus-Merzbacher disease (PMP), hereditary neuropathy with liability to pressure palsies (HNPP), adult-onset autosomal dominant leukodystrophy (ALDL) and amyotrophic lateral sclerosis (ALS).
