Project description:Non-alcoholic fatty liver disease (NAFLD), alongside the global obesity epidemic, is rapidly emerging as a dominant liver disease etiology that leads to progressive liver fibrosis, its terminal stage, cirrhosis, and hepatocellular carcinoma (HCC). We identified and validated a 133-gene signature (Prognostic Liver Signature for NAFLD [PLS-NAFLD]) to predict long-term HCC risk in patients with NAFLD. By analyzing PLS-NAFLD, IDO1 was identified as a potenial chemopreventive target for HCC from NAFLD. To test this hypothesis, we utilized our clinical-prognostic-signature-inducible cell culture model. We first confirmed that free fatty acid treatment (800 μM oleic acid and 400 μM palmitic acid) can induce PLS-NAFLD, then IDO1 inhibitor, epacadostat, can reverse the high-risk pattern in a dose-dependent manner.

Project description:Non-alcoholic fatty liver disease (NAFLD), also referred to metabolic associated fatty liver disease (MAFLD), displays characteristics indicative of metabolic disorders. Despite extensive research efforts, the underlying pathogenesis of NAFLD remain largely unknown and currently lack recommended treatment options. It is widely acknowledged that obesity and type 2 diabetes are significant risk factors for NAFLD, with the accumulation of free fatty acids (FFA) in the liver considered an initial trigger for its development. In this study, we exposed the human liver cell line Huh7 to a high concentration of a mixture containing both saturated (palmitic acid) and unsaturated (oleic acid) fatty acids for 24 hours. TMT and LC-MS based proteomics yielded a set of 172 significantly regulated proteins.

Project description:A key mechanism of malignant cell evasion from anti-tumor immunity centers on tryptophan metabolism by indole-2,3-dioxygenase (IDO1). By degrading tryptophan, IDO1 impairs anti-tumor T cell responses, which should be restored by IDO1 inhibitors (IDO1i). All IDO1i have, however, failed their clinical trials. Therefore, we sought to further investigate the cell intrisic biological function of IDO1 in the context of ovarian cancer by combining spatial proteomics and bulk RNA sequencing. We discovered that IFN-γ killed ovarian cancer cells via a linear pathway that required IFN-γ signaling, IDO1, tryptophan depletion followed by a duality of effects of the integrated stress response that initially protect against tryptophan depletion and later initiate cell death, potentially accounting for the heterogenous expression on IDO1 in situ. By contrast, IDO1i rescued these effects and promoted cancer cell survival. In summary, our results uncover a new facet of IDO1 biology in cancer and suggest that further investigation is required to justify IDO1i use in clinics.

Project description:Background/Aims: There is a major unmet need to assess prognostic impact of anti-fibrotics in clinical trials due to the slow rate of liver fibrosis progression. We aimed to develop a surrogate biomarker to predict future fibrosis progression. Methods: A Fibrosis Progression Signature (FPS) was defined to predict fibrosis progression within 5 years in HCV and NAFLD patients with no to minimal fibrosis at baseline (n=421), and validated in an independent NAFLD cohort (n=78). The FPS was used to assess response to 13 candidate anti-fibrotics in organotypic ex vivo cultures of clinical fibrotic liver tissues (n=78), and cenicriviroc in NASH patients enrolled in a clinical trial (n=19, NCT02217475). A serum-protein-based surrogate FPS (FPSec) was developed and technically evaluated in a liver disease patient cohort (n=79). Results: A 20-gene FPS was defined and validated in an independent NAFLD cohort (aOR=10.93, AUROC=0.86). Among computationally inferred fibrosis-driving FPS genes, BCL2 was confirmed as a potential pharmacological target using clinical liver tissues. Systematic ex vivo evaluation of 13 candidate anti-fibrotics identified rational combination therapies based on epigallocatechin gallate, some of which were validated for enhanced anti-fibrotic effect in ex vivo culture of clinical liver tissues. In NASH patients treated with cenicriviroc, FPS modulation was associated with 1-year fibrosis improvement accompanied by suppression of the E2F pathway. Induction of PPARalpha pathway was absent in patients without fibrosis improvement, suggesting benefit of combining PPARalpha agonism to improve anti-fibrotic efficacy of cenicriviroc. A 7-protein FPSec panel showed concordant prognostic prediction with FPS. Conclusion: FPS predicts long-term fibrosis progression in an etiology-agnostic manner, which can inform anti-fibrotic drug development.
