Project description:In this pilot study, we optimized diabetic foot ulcers (DFUs) single-cell profiling from debrided DFUs of underrepresented African American (AA) patients. The healing-DFUs were significantly enriched (P=0.03) with unique heterogenous Healing-associated Fibroblasts (HE-Fibro), expressing inflammation (CHI3L1), ECM-remodeling (MMP13, ASPN) and wound healing (NRG1, COL7A1) associated genes, validating our previous findings with surgically resected healer-DFUs. HE-Fibro/Fibro cells depicted significantly lower expression of key healing-associated CHIL3L1, and TIMP1 genes in AAs compared to Whites, signifying race-associated heterogeneity. Reparative M2 macrophages were enriched in Healing-DFUs (P=0.05), while inflammatory M1 macrophages displayed distinct transcriptome differences in Nonhealing-DFUs and Healing-DFUs. Cellular communication analysis revealed a pivotal role for HE-Fibro/Fibroblasts (signal-senders) and M1 (signal-receivers). HE-Fibro’s signaling significantly upregulated wound-resolving WNT, and ECM-remodeling CXCL pathways in Healing-DFUs, while M1 signaling upregulated healing-disruptive, and inflammation-associated, SPP1 pathways, in Nonhealing-DFUs. This study justifies using debrided tissues for single-cell assays and highlights the need for in-depth investigations into dysregulated wound healing microenvironments in AAs.

Project description:The diabetic wound healing is challenging due to the sabotaged delicate balance of immune regulation via an undetermined pathophysiological mechanism, so it is crucial to decipher multicellular signatures underlying diabetic wound healing and seek therapeutic strategies. Here, we develop a strategy using novel trimethylamine N-oxide (TMAO)-derived zwitterionic hydrogel to promote diabetic wound healing, and explore the multi-cellular ecosystem around zwitterionic hydrogel, mapping out an overview of different cells in the zwitterionic microenvironment using single-cell RNA sequencing. The diverse cellular heterogeneity has been revealed, highlighting the critical role of macrophage and neutrophils in managing diabetic wound healing. It is found that zwitterionic hydrogel can upregulate chemokine-secreted macrophages and downregulate extracellular traps (NETs)-neutrophils and facilitate their interactions compared with polyanionic and polycationic hydrogels, validating the underlying effect of zwitterionic microenvironment on the activation of adaptive immune system. These findings expand our horizons of the sophisticated orchestration of immune systems in zwitterion-directed diabetic wound repair and uncover new strategies of novel immunoregulatory biomaterials.

Project description:Diabetic foot ulcer (DFU) is a serious complication of diabetes mellitus, which causes great health damage and economic burden to patients. The pathogenesis of DFU is not fully understood.We screened wound healing-related genes using bioinformatics analysis, and full-thickness skin injury mice model and cellular assays were used to explore the role of target genes in diabetic wound healing. SFRP2 was identified as a wound healing-related gene, and the expression of SFRP2 is associated with immune cell infiltration in DFU. In vivo study showed that suppression of SFRP2 delayed the wound healing process of diabetic mice, impeded angiogenesis and matrix remodeling, and increased macrophage infiltration in wound tissues. In addition, suppression of SFRP2 enhanced M1 polarization in both the early and later stage of wound healing, and decreased M2 polarization in the later stage, which impeded the transition of M1 to M2 polarization of wound healing. Moreover, suppression of SFRP2 affected the transcriptome signatures-related to inflammatory response and energy metabolism at the early stage of wound healing. Extracellular flux analysis (EFA) showed that suppression of SFRP2 decreased mitochondrial energy metabolism and increased glycolysis in injury-related macrophages. Furthermore, suppression of SFRP2 inhibited transcriptome signaturesrelated to carbohydrate metabolism, lipid metabolism and amino acid metabolism, which consists the three main components of energy metabolism of macrophages. In conclusions, SFRP2 may function as a wound healing-related gene in DFU, and suppression of SFRP2 impaired diabetic wound healing by compromising the M1-to-M2 transition of macrophages and modulating the balance between mitochondrial energy metabolism and glycolysis.

Project description:At present, there is no effective treatment for diabetic wounds, and the cost of treatment is high. MicroRNAs (miRNAs) plays an important role in the process of diabetic wound healing. By regulating the expression of target genes, it regulates growth factors, cytokines and signal pathways, thereby affecting various stages of ulcer healing such as hemostasis, anti-inflammatory, proliferation and remodeling. In this study, differential expression of miRNAs in diabetic wound was screened. MiR-206 was selected as the research object to detect the effect of miR-206 on the proliferation of fibroblasts and vascular endothelial cell by regulating HIF-1?. Finally, in vivo studies showed that miR-206 antagomir could promote the expression of HIF-1?, CD34 and VEGF, and further promote wound healing in diabetic rats.

Project description:Wound healing is a multi-step process to rapidly restore barrier function. This process is often impaired in diabetic patients resulting in chronic wounds and amputation. We previously found that paradoxical activation of the mitogen-activated protein kinase (MAPK) pathway via topical administration of the BRAF inhibitor vemurafenib accelerates wound healing by activating keratinocyte proliferation and reepithelialization pathways in healthy mice. Herein, we investigated whether this wound healing acceleration also occurs in impaired diabetic wounds and found that topical vemurafenib not only improves wound healing in a murine diabetic wound model, but unexpectedly promotes hair follicle regeneration. Neogenic hair follicles expressing Sox-9, CD34 and K15 were found in wounds of diabetic and non-diabetic mice, and their formation can be prevented by blocking downstream MEK signaling. Thus, topically applied BRAF inhibitors may accelerate wound healing, and promote the restoration of improved skin architecture in both normal and impaired wounds.

platelet-derived growth factor concentration, fibroblast density, collagen
density, and hyaluronan concentration. This enables us to examine the variation
of these components in both normal and diabetic wound healing cases, and to
model the treatment protocols of these therapies. Within the context of our
model, we find that the key component to successful healing in diabetic wounds
is hyaluronan and that the therapies work by increasing the amount of hyaluronan
available in the wound environment. The time-to-healing results correlate with
those observed in clinical trials and the model goes some way to establishing an
understanding of why diabetic wounds do not heal, and how these treatments
affect the diabetic wound environment to promote wound closure.
