Project description:Assessment of efficacy in important subgroups - such as those defined by sex, age, race and region - in confirmatory trials is typically performed using separate analysis of the specific subgroup. This ignores relevant information from the complementary subgroup. Bayesian dynamic borrowing uses an informative prior based on analysis of the complementary subgroup and a weak prior distribution centred on a mean of zero to construct a robust mixture prior. This combination of priors allows for dynamic borrowing of prior information; the analysis learns how much of the complementary subgroup prior information to borrow based on the consistency between the subgroup of interest and the complementary subgroup. A tipping point analysis can be carried out to identify how much prior weight needs to be placed on the complementary subgroup component of the robust mixture prior to establish efficacy in the subgroup of interest. An attractive feature of the tipping point analysis is that it enables the evidence from the source subgroup, the evidence from the target subgroup, and the combined evidence to be displayed alongside each other. This method is illustrated with an example trial in severe asthma where efficacy in the adolescent subgroup was assessed using a mixture prior combining an informative prior from the adult data in the same trial with a non-informative prior.

Project description:BackgroundThe integration of historical data into ongoing clinical trials through Bayesian Dynamic Borrowing offers significant advantages, including reduced sample size, trial duration, and associated costs. However, challenges such as ensuring exchangeability between historical and current data and mitigating Type I error inflation remain critical. This study proposes a Bayesian group-sequential design incorporating a Self-Adaptive Mixture (SAM) prior framework to address these challenges in medical device trials.MethodsThe SAM prior combines informative priors derived from historical data with weakly informative priors, dynamically adjusting the weight of historical information based on congruence with current trial data. The design includes interim analyses, with Bayesian decision rules leveraging futility and efficacy boundaries derived using the frequentist spending functions. Effective Sample Size calculations informed adjustments to sample size and allocation ratios between experimental and control arms at each interim. The methodology was evaluated using a motivating example from a cardiovascular device trial with a noninferiority hypothesis.ResultsFour historical studies with substantial heterogeneity were incorporated. The SAM prior showed improved adaptation to prior-data conflicts compared to static methods, maintaining Type I error and Power at their nominal levels. In the motivating trial, the MAP prior was approximated as a mixture of beta distributions, facilitating congruence testing and posterior inference. Simulation studies confirmed the proposed design's efficiency under both congruent and incongruent scenarios.ConclusionsThe proposed Bayesian Group-Sequential Design with SAM prior offers a robust, adaptive framework for medical device trials, balancing statistical rigor with clinical interpretability. This approach enhances decision-making and supports timely, cost-effective evaluations, particularly in dynamic contexts like medical device development.

Project description:The purpose of this paper is to demonstrate and evaluate the use of Bayesian dynamic borrowing (Viele et al, in Pharm Stat 13:41-54, 2014) as a means of systematically utilizing historical information with specific applications to large-scale educational assessments. Dynamic borrowing via Bayesian hierarchical models is a special case of a general framework of historical borrowing where the degree of borrowing depends on the heterogeneity among historical data and current data. A joint prior distribution over the historical and current data sets is specified with the degree of heterogeneity across the data sets controlled by the variance of the joint distribution. We apply Bayesian dynamic borrowing to both single-level and multilevel models and compare this approach to other historical borrowing methods such as complete pooling, Bayesian synthesis, and power priors. Two case studies using data from the Program for International Student Assessment reveal the utility of Bayesian dynamic borrowing in terms of predictive accuracy. This is followed by two simulation studies that reveal the utility of Bayesian dynamic borrowing over simple pooling and power priors in cases where the historical data is heterogeneous compared to the current data based on bias, mean squared error, and predictive accuracy. In cases of homogeneous historical data, Bayesian dynamic borrowing performs similarly to data pooling, Bayesian synthesis, and power priors. In contrast, for heterogeneous historical data, Bayesian dynamic borrowing performed at least as well, if not better, than other methods of borrowing with respect to mean squared error, percent bias, and leave-one-out cross-validation.

Project description:We report use of a pharmacometrics enhanced Bayesian borrowing (PEBB) approach to leverage historical clinical trial data on a drug product to build models, project the outcome of future clinical trials, and borrow information from these projections to improve the efficiency of future target trials. This design takes a two-stage approach. First, a design phase is performed before target trial data are available to determine the operating characteristics and an appropriate tuning parameter that will be used in the subsequent analysis phase of a chosen target trial. Second, once the target trial data are available, the analysis phase is performed with the determined tuning parameter. This step is where borrowing is applied from these projections to inform the results for the target trial. To illustrate how a PEBB could improve the efficiency of clinical trials, we apply our design to trials with empagliflozin for treating patients with type 2 diabetes. We performed a retrospective evaluation applying the method to a phase III target trial and a hypothetical smaller trial. The type I error could be kept below 10% while increasing the trial power and effective sample size. Our findings suggest that a PEBB has the potential to increase the power of clinical trials, while controlling for type I error, by leveraging the information from previous trials through population pharmacokinetic/pharmacodynamic modeling and simulation.

Project description:BackgroundThe Flexible Adaptive Algorithmic Surveillance Testing (FAAST) program represents an innovative approach for improving the detection of new cases of infectious disease; it is deployed here to screen and diagnose SARS-CoV-2. With the advent of treatment for COVID-19, finding individuals infected with SARS-CoV-2 is an urgent clinical and public health priority. While these kinds of Bayesian search algorithms are used widely in other settings (eg, to find downed aircraft, in submarine recovery, and to aid in oil exploration), this is the first time that Bayesian adaptive approaches have been used for active disease surveillance in the field.ObjectiveThis study's objective was to evaluate a Bayesian search algorithm to target hotspots of SARS-CoV-2 transmission in the community with the goal of detecting the most cases over time across multiple locations in Columbus, Ohio, from August to October 2021.MethodsThe algorithm used to direct pop-up SARS-CoV-2 testing for this project is based on Thompson sampling, in which the aim is to maximize the average number of new cases of SARS-CoV-2 diagnosed among a set of testing locations based on sampling from prior probability distributions for each testing site. An academic-governmental partnership between Yale University, The Ohio State University, Wake Forest University, the Ohio Department of Health, the Ohio National Guard, and the Columbus Metropolitan Libraries conducted a study of bandit algorithms to maximize the detection of new cases of SARS-CoV-2 in this Ohio city in 2021. The initiative established pop-up COVID-19 testing sites at 13 Columbus locations, including library branches, recreational and community centers, movie theaters, homeless shelters, family services centers, and community event sites. Our team conducted between 0 and 56 tests at the 16 testing events, with an overall average of 25.3 tests conducted per event and a moving average that increased over time. Small incentives-including gift cards and take-home rapid antigen tests-were offered to those who approached the pop-up sites to encourage their participation.ResultsOver time, as expected, the Bayesian search algorithm directed testing efforts to locations with higher yields of new diagnoses. Surprisingly, the use of the algorithm also maximized the identification of cases among minority residents of underserved communities, particularly African Americans, with the pool of participants overrepresenting these people relative to the demographic profile of the local zip code in which testing sites were located.ConclusionsThis study demonstrated that a pop-up testing strategy using a bandit algorithm can be feasibly deployed in an urban setting during a pandemic. It is the first real-world use of these kinds of algorithms for disease surveillance and represents a key step in evaluating the effectiveness of their use in maximizing the detection of undiagnosed cases of SARS-CoV-2 and other infections, such as HIV.

Project description:Interest in incorporating historical data in the clinical trial has increased with the rising cost of conducting clinical trials. The intervention arm for the current trial often requires prospective data to assess a novel treatment, and thus borrowing historical control data commensurate in distribution to current control data is motivated in order to increase the allocation ratio to the current intervention arm. Existing historical control borrowing adaptive designs adjust allocation ratios based on the commensurability assessed through study-level summary statistics of the response agnostic of the distributions of the trial subject characteristics in the current and historical trials. This can lead to distributional imbalance of the current trial subject characteristics across the treatment arms as well as between current control data and borrowed historical control data. Such covariate imbalance may threaten the internal validity of the current trial by introducing confounding factors that affect study endpoints. In this article, we propose a Bayesian design which borrows and updates the treatment allocation ratios both covariate-adaptively and commensurate to covariate dependently assessed similarity between the current and historical control data. We employ covariate-dependent discrepancy parameters which are allowed to grow with the sample size and propose a regularized local regression procedure for the estimation of the parameters. The proposed design also permits the current and the historical controls to be similar to varying degree, depending on the subject level characteristics. We evaluate the proposed design extensively under the settings derived from two placebo-controlled randomized trials on vertebral fracture risk in post-menopausal women.

Project description:PurposeFacilitating genomic research may require the use of samples and data collected via consent processes that did not include specific descriptions of secondary uses. We explore whether a waiver of consent with notification and the option to withdraw (WNOW) is a viable alternative to written informed consent for secondary uses of samples and data.MethodsWe developed a retrospective case study of a rare-disease protocol involving 1,978 participants that implemented WNOW for genomic data-sharing activities. We analyzed institutional review board and investigator records and conducted in-depth semistructured interviews with key staff members.ResultsWNOW was largely successful at achieving its goals in this case, although the recontact effort, relative to proceeding with a waiver, decreased participation in genomic data sharing by 13.8% (n = 253), primarily because 224 letters were returned as undeliverable. A small number of participants responded (n = 89), and some of them expressed confusion and frustration. In the pediatric arm of the study, the research may have been practicable without a waiver, given the relationship between the pediatric clinicians and families.ConclusionThe practicability of conducting research on existing specimens without a waiver of informed consent, and whether WNOW is a viable alternative, depend on contextual factors, including a reliable way to communicate with participants.Genet Med advance online publication 26 January 2017.

Project description:BackgroundDNA microarrays provide an efficient method for measuring activity of genes in parallel and even covering all the known transcripts of an organism on a single array. This has to be balanced against that analyzing data emerging from microarrays involves several consecutive steps, and each of them is a potential source of errors. Errors tend to accumulate when moving from the lower level towards the higher level analyses because of the sequential nature. Eliminating such errors does not seem feasible without completely changing the technologies, but one should nevertheless try to meet the goal of being able to realistically assess degree of the uncertainties that are involved when drawing the final conclusions from such analyses.ResultsWe present a Bayesian hierarchical model for finding differentially expressed genes between two experimental conditions, proposing an integrated statistical approach where correcting signal saturation, systematic array effects, dye effects, and finding differentially expressed genes, are all modeled jointly. The integration allows all these components, and also the associated errors, to be considered simultaneously. The inference is based on full posterior distribution of gene expression indices and on quantities derived from them rather than on point estimates. The model was applied and tested on two different datasets.ConclusionsThe method presents a way of integrating various steps of microarray analysis into a single joint analysis, and thereby enables extracting information on differential expression in a manner, which properly accounts for various sources of potential error in the process.

Project description:Basket trials are an innovative precision medicine clinical trial design evaluating a single targeted therapy across multiple diseases that share a common characteristic. To date, most basket trials have been conducted in early-phase oncology settings, for which several Bayesian methods permitting information sharing across subtrials have been proposed. With the increasing interest of implementing randomised basket trials, information borrowing could be exploited in two ways; considering the commensurability of either the treatment effects or the outcomes specific to each of the treatment groups between the subtrials. In this article, we extend a previous analysis model based on distributional discrepancy for borrowing over the subtrial treatment effects ('treatment effect borrowing', TEB) to borrowing over the subtrial groupwise responses ('treatment response borrowing', TRB). Simulation results demonstrate that both modelling strategies provide substantial gains over an approach with no borrowing. TRB outperforms TEB especially when subtrial sample sizes are small on all operational characteristics, while the latter has considerable gains in performance over TRB when subtrial sample sizes are large, or the treatment effects and groupwise mean responses are noticeably heterogeneous across subtrials. Further, we notice that TRB, and TEB can potentially lead to different conclusions in the analysis of real data.

Project description:Basket trials are increasingly used for the simultaneous evaluation of a new treatment in various patient subgroups under one overarching protocol. We propose a Bayesian approach to sample size determination in basket trials that permit borrowing of information between commensurate subsets. Specifically, we consider a randomized basket trial design where patients are randomly assigned to the new treatment or control within each trial subset ("subtrial" for short). Closed-form sample size formulae are derived to ensure that each subtrial has a specified chance of correctly deciding whether the new treatment is superior to or not better than the control by some clinically relevant difference. Given prespecified levels of pairwise (in)commensurability, the subtrial sample sizes are solved simultaneously. The proposed Bayesian approach resembles the frequentist formulation of the problem in yielding comparable sample sizes for circumstances of no borrowing. When borrowing is enabled between commensurate subtrials, a considerably smaller trial sample size is required compared to the widely implemented approach of no borrowing. We illustrate the use of our sample size formulae with two examples based on real basket trials. A comprehensive simulation study further shows that the proposed methodology can maintain the true positive and false positive rates at desired levels.
