Project description:The effective development of innovative surgical applications and immunosuppressive agents have improved remarkable advancements in solid organ transplantation. Despite these improvements led to prevent acute rejection and to promote short-term graft survival, the toxicity of long-term immunosuppression regiments has been associated to organ failure or chronic graft rejection. The graft acceptance is determined by the balance between the regulatory and the alloreactive arm of the immune system. Hence, enhance regulatory cells leading to immune tolerance would be the solution to improve long-term allograft survival which, by reducing the overall immunosuppression, will provide transplanted patients with a better quality of life. Regulatory T cells (Tregs), and regulatory myeloid cells (MRCs), including regulatory macrophages and tolerogenic dendritic cells, are promising cell populations for restoring tolerance. Thus, in the last decade efforts have been dedicated to apply regulatory cell-based therapy to improve the successful rate of organ transplantation and to promote allogeneic tolerance. More recently, this approach has been translated into clinical application. The aim of this review is to summarize and discuss results on regulatory cell-based strategies, focusing on Tregs and MRCs, in terms of safety, feasibility, and efficacy in clinical studies of organ transplantation.
Project description:The Mehri Language is an endangered language spoken in eastern Yemen, a sub-group of the Semitic language family, and a Southern Arabic language. The syntax of Mehri wh-questions has not been explored within minimalism; hence, there is a morpho-syntactic need to provide a modern analysis of wh-questions in order to show how the interrogative structures can be derived. This study aims to examine the syntax of the wh-question movement in Mehri's unaccusative/ergative and unergative structures and answer the following questions within Chomsky's (2000 and 2008) Phase-based Theory: (i) Does the Mehri language allow fronting of wh-phrases to [Spec-CP]? And (ii) how can wh-movement in Mehri unaccusative and (un)ergative structures be accounted for? This work presents a novel analysis of wh-question movement in unaccusative/ergative and unergative structures in Mehri; it demonstrates that the source head C triggers the movement of wh-adjunct and wh-subject phrases. In wh-adjunct extraction, two strategies are employed: overt wh-fronting and wh-in-situ; when the head Foc inherits an edge feature from C, wh-adjunct overtly undergoes movement from its original position within v*P to the left peripheries of [Spec-FocP] and subsequently to [Spec-CP]. When the lexical wh-adjunct remains within v*P, its question features covertly move to [Spec-CP], because the head Foc does not inherit an edge feature from C. In wh-subject extraction, the wh-subject overly undergoes movement to [Spec-CP] because C obligatorily inherits the edge feature to the head Top, which triggers movement of the illogical subject in unaccusative/ergative structures and the logical external specifier in unergative structures to [Spec-CP]. Moreover, Mehri obeys the Phase Impenetrability Condition of Chomsky, where wh-subject and wh-adjunct phrases must pass through certain phases until [Spec-CP].
Project description:BackgroundApproximately 1 in 4 adults will develop hallux valgus (HV). Up to 80% of adult Internet users reference online sources for health-related information. Overall, with the high prevalence of HV combined with the numerous treatment options, we believe patients are likely turning to Internet search engines for questions relevant to HV. Using Google's people also ask (PAA) or frequently asked questions (FAQs) feature, we sought to classify these questions, categorize the sources, as well as assess their levels of quality and transparency.MethodsOn October 9, 2022, we searched Google using these 4 phrases: "hallux valgus treatment," "hallux valgus surgery," "bunion treatment," and "bunion surgery." The FAQs were classified in accordance with the Rothwell Classification schema and each source was categorized. Lastly, transparency and quality of the sources' information were evaluated with the Journal of the American Medical Association's (JAMA) Benchmark tool and Brief DISCERN, respectively.ResultsOnce duplicates and FAQs unrelated to HV were removed, our search returned 299 unique FAQs. The most common question in our sample was related to the evaluation of treatment options (79/299, 26.4%). The most common source type was medical practices (158/299, 52.8%). Nearly two-thirds of the answer sources (184/299; 61.5%) were lacking in transparency. One-way analysis of variance revealed a significant difference in mean Brief DISCERN scores among the 5 source types, F(4) = 54.49 (P < .001), with medical practices averaging the worst score (12.1/30).ConclusionPatients seeking online information concerning treatment options for HV search for questions pertaining to the evaluation of treatment options. The source type encountered most by patients is medical practices; these were found to have both poor transparency and poor quality. Publishing basic information such as the date of publication, authors or reviewers, and references would greatly improve the transparency and quality of online information regarding HV treatment.Level of evidenceLevel V, mechanism-based reasoning.
Project description:BackgroundOrgan donation has been claimed as a prosocial behavior to prolong the recipient's life and deliver great love. However, the supply-demand ratio of organs in China is highly unbalanced. Being entangled with multiple factors derived from individual and supra-individual levels, organ donation in China is important but sensitive. Previous scholars usually depended on obtrusive approaches to explore the facilitators and hindrances of organ donation, which is hard to discover genuine perceptions toward organ donation. Besides, relatively limited scholarly attention has been paid to what hampers organ donation in China.ObjectiveWe intended to excavate the diversified social representations and perceived barriers to organ donation in China over the past decade.MethodTwo kinds of text analysis methods-semantic network analysis and conventional content analysis, were applied to 120,172 posts from ordinary users on the Sina Weibo platform to address the research questions.ResultsRegarding social representations, the "hope, understanding, and acceptance" of organ donation was the most pronounced one (34% of the whole semantic network), followed by "family story" (26%), "the procedure of organ donation in NGOs" (15%), "the practical value of organ donation" (14%), and "organ donation in the medical context" (11%). Regarding perceived barriers, a four-layer framework was constructed, including (1) the individual level, mainly about the fear of death and postmortem autopsy; (2) the familial level, which refers to the opposition from family members; (3) the societal level, which alludes to distrust toward medical institutions and the general society; (4) the cultural level, which covers religious-cultural concerns about fatalism.ConclusionIn concordance with prior works on social representations regarding organ donation, the current study also uncovered the coexistence of antithetical representations about organ donation-the longing for survival and the fear of death. This representation pair serves as the foundation of Chinese people's ambivalence. Besides, family-related narratives were dispersed over various representations, demonstrating the critical position of family support in organ donation. Moreover, the four-layer framework concerning donation barriers affords a reference for future empirical studies. The practical implications of this work are further discussed.
Project description:Harvesting the energy from the interaction between hygroscopic materials and atmospheric water can generate green and clean energy. However, the ion diffusion process of moisture-induced dissociation leads to the disappearance of the ion concentration gradient gradually, and there is still a lack of moisture-based power generation devices with truly continuous operation, especially the duration of the current output still needs to be extended. Here, we propose a design for reconstructing the ion concentration gradient by coupling photocatalytic hydrogen evolution reaction with hydrovoltaic effect, to report a moisture-enabled electric generator (MEG) with continuous current output. We show that the introduction of the photocatalytic layer not only absorbs light energy to greatly increase the power generation of the MEG (500% power density enhancement), but more importantly, the photocatalytic hydrogen evolution process consumes the pre-stacked ions to restore the ion concentration gradient, allowing the MEG to continuously output current for more than 600 hours, which is 1 to 2 orders of magnitude higher than the great majority of existed MEGs in terms of the current output duration.

Le principali cause di febbre al ritorno da un viaggio in un paese tropicale sono la malaria, le infezioni gastrointestinali, le infezioni respiratorie e urinarie e le patologie cutanee. È fondamentale e urgente escludere la malaria, a prescindere dai sintomi associati. Le altre possibili eziologie devono essere considerate in funzione del ritardo di comparsa della febbre rispetto al tempo di incubazione, dei segni associati e dei risultati degli esami di laboratorio iniziali. Infine, non si deve trascurare un’infezione comunitaria o cosmopolita a potenziale evolutivo grave.
Project description:Las enfermedades más frecuentes al regresar de un viaje al extranjero son las infecciones gastrointestinales, las enfermedades febriles y los problemas dermatológicos. La evaluación de un niño febril al regreso de un viaje es clínica: verificación de los antecedentes y las vacunaciones, búsqueda de otros casos en contacto, signos funcionales y físicos, y características del viaje. Unas pruebas complementarias de rutina completan a menudo la evaluación para una orientación sindrómica. Si bien las causas de fiebre son, sobre todo, cosmopolitas, la gravedad de la enfermedad exótica obliga a buscarlas prioritariamente. El paludismo debe sospecharse ante una fiebre procedente de África subsahariana. El diagnóstico se basa en la asociación de un frotis sanguíneo y una prueba sensible (gota gruesa, reacción en cadena de la polimerasa [PCR]) o una prueba de diagnóstico rápido (para Plasmodiumfalciparum). Los criterios clínico-biológicos de gravedad son esenciales para orientar y tratar al paciente. En caso de acceso no complicado por P. falciparum, el tratamiento se basa en un tratamiento combinado a base de artemisinina (ACT), arteméter-lumefantrina o artenimol-piperaquina. Los accesos graves se tratan con artesunato intravenoso, seguido de ACT oral. Dengue, chikungunya e infección por virus Zika tienen características clínicas comunes (fiebre-erupción-artralgias, tratamiento sintomático). En caso de dengue, conviene controlar la aparición de signos de alerta, que podrían hacer temer una evolución negativa. El chikungunya es grave en caso de transmisión en el parto, con un riesgo de encefalitis neonatal. En el niño, el Zika es asintomático o poco sintomático. Pero, en caso de infección durante el embarazo, el riesgo es el de una embriofetopatía. El diagnóstico de estas arbovirosis se basa en la PCR en la fase aguda y en la serología secundariamente. La sintomatología de la fiebre tifoidea es poco específica, lo cual justifica la práctica de hemocultivos sistemáticos ante una fiebre tras un viaje. Su tratamiento se basa en las cefalosporinas de tercera generación o el ciprofloxacino, pero las resistencias aumentan. En suma, la diversidad de las etiologías de una fiebre al regreso de un viaje y la potencial gravedad de las infecciones importadas imponen una reflexión sobre el proceso de tratamiento de estos pacientes, en especial en lo referente al riesgo de fiebre hemorrágica.
Project description:In crowding, perception of a target deteriorates in the presence of nearby flankers. Surprisingly, perception can be rescued from crowding if additional flankers are added (uncrowding). Uncrowding is a major challenge for all classic models of crowding and vision in general, because the global configuration of the entire stimulus is crucial. However, it is unclear which characteristics of the configuration impact (un)crowding. Here, we systematically dissected flanker configurations and showed that (un)crowding cannot be easily explained by the effects of the sub-parts or low-level features of the stimulus configuration. Our modeling results suggest that (un)crowding requires global processing. These results are well in line with previous studies showing the importance of global aspects in crowding.
Project description:Parent-child conversations are important for children's cognitive development, children's ability to cope with stressful events, and can shape children's beliefs about the causes of illness. In the context of a global pandemic, families have faced a multitude of challenges, including changes to their routines, that they need to convey to their children. Thus, parent-child conversations about the coronavirus pandemic might convey information about the causes of illness, but also about how and why it is necessary for children to modify their behaviors to comply with new social norms and medical guidance. The main goal of this study was to examine the questions children ask about the COVID-19 pandemic and how parents answer them. This survey included responses from a national sample of 349 predominantly white parents of children between the ages of 3 and 12 recruited through Amazon's Mechanical Turk in United States. Parents reported that although children asked about COVID-19 and its causes (17.3%), children asked primarily about lifestyle changes that occurred as a result of the pandemic (24.0%) and safety (18.4%). Parents reported answering these questions by emphasizing that the purpose of different preventative measures was to protect the child (11.8%) or the family (42.7%) and providing reassurance (13.3%). Many parents discussed how it was their social responsibility to slow the spread of the virus (38.4%). Parents of younger children tended to shield them from information about COVID-19 (p = .038), while parents with more knowledge were more likely to provide explanations (p < .001). Our analysis shows that families not only discuss information about the virus but also information about changes to their lifestyle, preventative measures, and social norms.
