ABSTRACT: In adolescents and young adults, instability is a common shoulder pathology with a myriad of coexisting soft tissue and bony lesions. When evaluating a patient for the cause of instability, care must be given to assess for the infrequent lesions, including glenoid avulsion of the glenohumeral ligaments. This case example illustrates key points in the diagnosis, surgical, and postsurgical management of this less common cause of anterior shoulder instability.

Project description:Objectives: To theoretically confirm that the glenohumeral ligament (GHL), specifically the inferior glenohumeral ligament (IGHL), plays an important role in posterior shoulder stability in different postures, and to provide reference for clinical diagnosis and treatment of posterior shoulder instability (PSI). Materials and Methods: In this retrospective study, bone-ligament-bone models were established in 15 fresh adult shoulder joint specimens and selective cutting was performed for analysis. The humeral head was loaded posteriorly at a central pressure of 22N using the INSTRON8874 biomechanical testing system and the load-displacement curve was plotted. The posterior displacement of the humeral head was measured after continuous cutting of the following structures: (1) complete; (2) superior glenohumeral ligament (SGHL); (3) SGHL + middle glenohumeral ligament (MGHL); (4) SGHL + MGHL + IGHL; (5) MGHL; (6) MGHL + IGHL; (7) anterior-bundle IGHL (IGHL-AB); (8) posterior-bundle IGHL (IGHL-PB); (9) IGHL. The results obtained were analyzed using the SPSS10.0 statistical software. Results: Favorable posterior stability of the complete bone-ligament-bone model was observed, with an average displacement of 11.32±3.89 mm. The displacement of SGHL and SGHL + MGHL groups was not significantly increased compared with that in the complete group (P>0.05). After cutting of SGHL + MGHL + IGHL, the posterior displacement of all angles increased (P<0.05), resulting in PSI that was manifested in dislocation or subluxation. There was no obvious increase in posterior displacement after cutting the IGHL-AB (P>0.05). Significantly increased posterior displacement was observed at 45° abduction after cutting the IGHL-PB compared with the complete group, but not at the 90° abduction. The posterior displacement increased obviously at both 45° and 90° abduction when the IGHL was completely cut off (P<0.05). Conclusions: Repairing the IGHL plays a certain role in rebuilding the posterior stability of the shoulder joint. Detecting the function of the IGHL in the abduction and external rotation positions of the shoulder joint has certain significance for diagnosing PSI.

Project description:BackgroundOptimal implant placement in reverse total shoulder arthroplasty (rTSA) remains controversial. Specifically, the optimal glenoid inclination is unknown. Therefore, a cadaveric shoulder simulator with 3-dimentional human motion specific to rTSA was used to study joint contact and muscle forces as a function of glenoid component inclination.MethodsEight human cadaver shoulders were tested before and after rTSA implantation. Scapular plane abduction kinematics from control subjects and those with rTSA drove a cadaveric shoulder simulator with 3-dimentional scapulothoracic and glenohumeral motion. Glenoid inclination varied from -20° to +20°. Outputs included compression, superior-inferior (S/I) shear, and anterior-posterior shear forces from a 6° of freedom load cell in the joint, and deltoid and rotator cuff muscle forces. Data were evaluated with statistical parametric mapping and t-tests.ResultsInferior glenoid inclination (-) reduced S/I shear by up to 125% relative to superior inclination, with similar compression to the neutral condition (0°). Superior inclinations (+) increased the S/I shear force by approximately the same magnitude, yet decreased compression by 25% in the most superior inclination (+20°). There were few differences in deltoid or rotator cuff forces due to inclination. Only the middle deltoid decreased by approximately 7% for the most inferior inclination (-20°). Compared with native shoulders, the neutral (0°) rTSA inclination showed reduced forces of 30%-75% in the anterior deltoid and a trend toward decreased forces in the middle deltoid. Force demands on the rotator cuff varied as a function of elevation, with a trend toward increased forces in rTSA at peak glenohumeral elevation.ConclusionsInferior inclination reduces superior shear forces, without influencing compression. Superior inclination increased S/I shear, while decreasing compression, which may be a source of component loosening and joint instability after rTSA. Inferior inclination of the rTSA glenoid may reduce the likelihood of glenoid loosening by reducing the magnitude of cyclic shear and compressive loading during arm elevation activities, although this may be altered by specific-subject body habitus and motion. These factors are especially important in revision rTSA or glenoid bone grafting where there is already a 3-fold increase in glenoid baseplate loosening vs. primary rTSA.
