Project description:BackgroundThe present study firstly aimed to identify context patterns in sport and exercise among adolescents at lower and upper secondary education. The organisational, social and competitive contexts of leisure-time sport and exercise were included as pattern indicators. The second aim was to examine the stability of these patterns across educational transition. The last aim was to investigate whether a subjective evaluation of the transition influences whether people stay in the same pattern across time.MethodsOne-year longitudinal data of 392 adolescents were analysed.ResultsBoth before and after the educational transition, four context patterns were identified: the traditional competitive club athletes with friends, the self-organised individualists, the non-club-organised sportspersons and the mostly inactives. More than half of the individuals stayed in the same pattern across time. When individuals changed pattern, their change was most often from the self-organised individualists and the non-club-organised to the mostly inactives. A subjective evaluation of the transition influenced the stability of only the traditional competitive club athletes with friends. The chance of these people staying in the same pattern decreased with increased transitional stress.ConclusionsKnowledge about the stability and change of context patterns can be used to make recommendations for policy strategies and to develop more individually-tailored promotion programs.
Project description:ObjectiveTo describe PCORnet, a clinical research network developed for patient-centered outcomes research on a national scale.Study design and settingDescriptive study of the current state and future directions for PCORnet. We conducted cross-sectional analyses of the health systems and patient populations of the 9 Clinical Research Networks and 2 Health Plan Research Networks that are part of PCORnet.ResultsWithin the Clinical Research Networks, electronic health data are currently collected from 337 hospitals, 169,695 physicians, 3,564 primary care practices, 338 emergency departments, and 1,024 community clinics. Patients can be recruited for prospective studies from any of these clinical sites. The Clinical Research Networks have accumulated data from 80 million patients with at least one visit from 2009 to 2018. The PCORnet Health Plan Research Network population of individuals with a valid enrollment segment from 2009 to 2019 exceeds 60 million individuals, who on average have 2.63 years of follow-up.ConclusionPCORnet's infrastructure comprises clinical data from a diverse cohort of patients and has the capacity to rapidly access these patient populations for pragmatic clinical trials, epidemiological research, and patient-centered research on rare diseases.
Project description:PURPOSE OF REVIEW:Chronic lymphocytic leukaemia is now recognised as a heterogenous disease with a variety of clinical outcomes. Here we summarise the way it is currently stratified according to genetic risk and patient characteristics and the treatment approaches used for these different subgroups. RECENT FINDINGS:Certain patients appear to sustain MRD negativity after combination chemoimmunotherapy, leading to the suggestion that their CLL may be cured. However, 17p-deleted, p53-mutated or IGHV-UM subgroups are generally resistant to FCR, and much better responses are seen with ibrutinib and venetoclax, frequently inducing MRD negativity that hopefully will be translated into durable remissions. Small molecule inhibitors have already revolutionised CLL treatment. Going forward, we anticipate their use in the majority of patients, early after diagnosis and with curative intent.
Project description:BackgroundThe relationship between slow loris (Nycticebus spp.) venom (BGE protein) and the major cat allergen (Fel d 1) from domestic cat (Felis catus) is known for about two decades. Along this time, evidence was accumulated regarding convergences between them, including their almost identical mode of action.MethodsLarge-scale database mining for Fel d 1 and BGE proteins in Felidae and Nycticebus spp., alignment, phylogeny proposition and molecular modelling, associated with directed literature review were assessed.ResultsFel d 1 sequences for 28 non-domestic felids were identified, along with two additional loris BGE protein sequences. Dimer interfaces are less conserved among sequences, and the chain 1 shows more sequence similarity than chain 2. Post-translational modification similarities are highly probable.ConclusionsFel d 1 functions beyond allergy are discussed, considering the great conservation of felid orthologs of this protein. Reasons for toxicity being found only in domestic cats are proposed in the context of domestication. The combination of the literature review, genome-derived sequence data, and comparisons with the venomous primate slow loris may point to domestic cats as potentially poisonous mammals.
Project description:BackgroundModern Next Generation- and Third Generation- Sequencing methods such as Illumina and PacBio Circular Consensus Sequencing platforms provide accurate sequencing data. Parallel developments in Deep Learning have enabled the application of Deep Neural Networks to variant calling, surpassing the accuracy of classical approaches in many settings. DeepVariant, arguably the most popular among such methods, transforms the problem of variant calling into one of image recognition where a Deep Neural Network analyzes sequencing data that is formatted as images, achieving high accuracy. In this paper, we explore an alternative approach to designing Deep Neural Networks for variant calling, where we use meticulously designed Deep Neural Network architectures and customized variant inference functions that account for the underlying nature of sequencing data instead of converting the problem to one of image recognition.ResultsResults from 27 whole-genome variant calling experiments spanning Illumina, PacBio and hybrid Illumina-PacBio settings suggest that our method allows vastly smaller Deep Neural Networks to outperform the Inception-v3 architecture used in DeepVariant for indel and substitution-type variant calls. For example, our method reduces the number of indel call errors by up to 18%, 55% and 65% for Illumina, PacBio and hybrid Illumina-PacBio variant calling respectively, compared to a similarly trained DeepVariant pipeline. In these cases, our models are between 7 and 14 times smaller.ConclusionsWe believe that the improved accuracy and problem-specific customization of our models will enable more accurate pipelines and further method development in the field. Project description:On hearing a novel voice, listeners readily form personality impressions of that speaker. Accurate or not, these impressions are known to affect subsequent interactions; yet the underlying psychological and acoustical bases remain poorly understood. Furthermore, hitherto studies have focussed on extended speech as opposed to analysing the instantaneous impressions we obtain from first experience. In this paper, through a mass online rating experiment, 320 participants rated 64 sub-second vocal utterances of the word 'hello' on one of 10 personality traits. We show that: (1) personality judgements of brief utterances from unfamiliar speakers are consistent across listeners; (2) a two-dimensional 'social voice space' with axes mapping Valence (Trust, Likeability) and Dominance, each driven by differing combinations of vocal acoustics, adequately summarises ratings in both male and female voices; and (3) a positive combination of Valence and Dominance results in increased perceived male vocal Attractiveness, whereas perceived female vocal Attractiveness is largely controlled by increasing Valence. Results are discussed in relation to the rapid evaluation of personality and, in turn, the intent of others, as being driven by survival mechanisms via approach or avoidance behaviours. These findings provide empirical bases for predicting personality impressions from acoustical analyses of short utterances and for generating desired personality impressions in artificial voices.
Project description:BackgroundCare providers working with adolescents with anorexia nervosa (AN) encounter difficulties inherent in the illness (denial, ambivalence) and those related to the fact that it is most often the parents who bring adolescents to care units. Our aim was to study attitudes towards care among adolescents with AN treated in a specialised day hospital using an analysis of letters written before and after treatment.MethodsAdolescents (12-20 years old) treated for AN in a specialised day hospital, providing multidisciplinary care while enabling a return to schooling inside the facility were included. We analysed 50 admission letters and 23 goodbye letters using general inductive analysis. A mirror analysis was conducted.ResultsIn the admission letters, symptoms, calls for help, and reports on the adolescents' care trajectories were central themes. Among the categories noted in both the admission and the goodbye letters, some were similar, some mirrored others and a few differed.ConclusionsThis study highlights how ambivalence and motivations towards care, recovery and illness all interact. It also shows the evolution of the adolescents' positions via their narratives on their experience of care, the constraints involved and its benefits for them. The results are discussed in a care perspective.
Project description:Immunoglobulin (Ig)G medicinal products manufactured in 2020 were tested for infectivity neutralization and hemagglutination inhibition against World Health Organization-selected influenza strains included in worldwide vaccines 2020-2022. The IgG batches (from US plasma) showed potent activity. Intravenous immunoglobulin could potentially add to therapies for serious influenza cases in immunocompromised patients. Further study is warranted.
