ABSTRACT: Physicians should be familiar with May-Thurner syndrome, characterized by the compression of the left common iliac vein by the right common iliac artery and the vertebral body, resulting in pain and swelling of the left lower extremity and DVT. A 64-year-old woman presented with unexplained edema in the left lower extremity. Computed tomography with contrast enhancement revealed that the left common iliac vein was compressed and narrowed by the right common iliac artery and the vertebral body, leading to the diagnosis of May-Thurner syndrome.

Persistent Mullerian duct syndrome: a rare cause of unilateral cryptorchidism.

Project description:Persistent Mullerian duct syndrome (PMDS) is a rare syndrome and sometimes the cause of a common problem in paediatric and surgical practice, namely undescended testes. PMDS is a recessive disease in which there is a defect in anti-Mullerian hormone secretion or receptor activity resulting in persistence of Mullerian structures such as a uterus or fallopian tubes with otherwise normal virilisation. Here the authors present a case of a 1½-year-old boy who was referred to their hospital because of unilateral cryptorchidism. During laparoscopic surgery, two gonads were present joined together by a uterus-like structure. Additional investigations showed a normal male karyotype and biopsies of the gonads revealed infantile testis parenchyma making the diagnosis PMDS likely.

A Case Report of May-Thurner Syndrome Identified on Abdominal Ultrasound.

Project description:May-Thurner syndrome (MTS) is most commonly caused by the compression of the left iliac vein by the right iliac artery against the lumbar spine, which leads to the development of a partial or occlusive deep venous thrombosis (DVT).1 Diagnosis begins with a duplex ultrasound of the lower extremities to evaluate for a femoropopliteal thrombus, and in high-risk patients where a more proximal DVT is suspected and the DVT ultrasound is negative, a computed tomography venogram (CTV) or magnetic resonance venogram (MRV) is performed.1,3 In this case report, a patient presented to the emergency department (ED) with two days of left lower extremity pain and swelling. Initial lower extremity DVT ultrasound was negative, so a CTV was ordered and revealed a thrombus in the left common iliac vein with overlying compression by the right common iliac artery, suggesting the diagnosis of May-Thurner syndrome (Figure 1). Afterwards, a point-of-care ultrasound (POCUS) was performed at bedside to evaluate the caval and iliac arteries and the findings were consistent with the CTV (Figure 2, 3, 4). If the POCUS was performed prior to the CTV, the patient may have been spared the radiation exposure from CT, as well as the risks of intravenous (IV) contrast required for a venogram. Therefore, in high risk patients in whom a negative DVT ultrasound will prompt advanced imaging with CTV or MRV, I propose the addition of a lower abdominal ultrasound using a curvilinear probe to assess the caval and iliac arteries prior to obtaining a CTV or MRV.TopicsMay-Thurner Syndrome, leg swelling, POCUS, ultrasound, deep venous thrombosis.

May-Thurner syndrome in patients with postural orthostatic tachycardia syndrome and Ehlers-Danlos syndrome: a case series.

Project description:BackgroundPostural orthostatic tachycardia syndrome (POTS), Ehlers-Danlos syndrome (EDS), and May-Thurner syndrome (MTS) are three syndromes that are often misdiagnosed or underdiagnosed. The true prevalence of these syndromes may be higher than currently reported. The following case series is the first to report a three-way association between POTS, EDS, and MTS.Case summaryWe describe three patients with concomitant POTS, EDS, and MTS. Although abdominopelvic vasculature evaluation can be difficult via conventional imaging techniques, we present the use of novel dynamic contrast-enhanced magnetic resonance angiography with Differential Subsampling with Cartesian Ordering (DISCO) and four-dimensional flow magnetic resonance imaging to aid vasculature evaluation and the diagnosis of MTS. Two patients underwent left common iliac vein stenting to treat MTS, experiencing significant improvement in their POTS symptoms and quality of life.DiscussionEhlers-Danlos syndrome, POTS, and MTS may interact synergistically to exacerbate symptoms. Patients with EDS should be evaluated for possible POTS and pelvic venous complications. Left common iliac vein stenting for MTS can mitigate POTS symptoms by decreasing lower extremity venous pooling and should be considered in this patient population. Further research is needed to understand the exact mechanism and intricacies of this syndrome triad.

Leg edema quantification for heart failure patients via 3D imaging.

Project description:Heart failure is a common cardiac disease in elderly patients. After discharge, approximately 50% of all patients are readmitted to a hospital within six months. Recent studies show that home monitoring of heart failure patients can reduce the number of readmissions. Still, a large number of false positive alarms as well as underdiagnoses in other cases require more accurate alarm generation algorithms. New low-cost sensors for leg edema detection could be the missing link to help home monitoring to its breakthrough. We evaluated a 3D camera-based measurement setup in order to geometrically detect and quantify leg edemas. 3D images of legs were taken and geometric parameters were extracted semi-automatically from the images. Intra-subject variability for five healthy subjects was evaluated. Thereafter, correlation of 3D parameters with body weight and leg circumference was assessed during a clinical study at the Medical University of Graz. Strong correlation was found in between both reference values and instep height, while correlation in between curvature of the lower leg and references was very low. We conclude that 3D imaging might be a useful and cost-effective extension of home monitoring for heart failure patients, though further (prospective) studies are needed.

Pediatric May-Thurner Syndrome-Systematic review and individual patient data meta-analysis.

Project description:BackgroundThe outcomes of deep vein thrombosis (DVT) in children with May-Thurner Syndrome (MTS) remain unclear.ObjectivesThis systematic review and patient-level meta-analysis aims to describe the outcomes of children with MTS presenting with DVT.MethodsA systematic review of the published literature was performed. Data related to patients <18 years diagnosed with MTS and DVT was extracted. Risk of bias was assessed using the Murad criteria. Outcomes included vessel patency post-treatment, DVT recurrence, and post-thrombotic syndrome (PTS). Predictive and explanatory models were developed for these outcomes.ResultsIn total, 109 cases were identified (age range 4-17 years; 77 females) in 28 studies; 75% of patients had ≥1 additional risk factor for DVT. PTS was seen in 61% of patients, DVT recurrence in 38%, and complete vessel patency post-treatment in 65%. The models developed to predict and explain PTS performed poorly overall. Recurrent thrombosis (adjusted for age and patency) predicted PTS (odds ratio [OR] 3.36, 95% confidence interval [CI] 1.28-8.82). DVT management strategies (adjusted for age and DVT characteristics) predicted vessel patency (OR 2.10, 95% CI 1.43-3.08). Lack of complete vessel patency (adjusted for age and thrombophilia) predicted recurrent DVT (OR 2.70, 95% CI 1.09-6.67). Sensitivity analyses showed the same direction of effects for all outcomes.ConclusionsPTS and DVT recurrence occur frequently in pediatric MTS. PTS prediction is complex and it was not possible to identify early predictors to guide clinical practice. Use of imaging-guided therapy and thrombus burden predicted venous patency, and lack of patency predicted DVT recurrence.

Severe mitral regurgitation causing unilateral pulmonary edema: A case report.

Project description:This is a case of a 70-year-old male patient with a history of degenerative mitral valve disease who presented to the emergency department with progressively worsening dyspnea. Unilateral Asterixis in Arm and Leg Caused by Internal Capsula Stroke.

Project description:We report an unusual clinical manifestation of ischemic stroke with acute right-sided asterixis affecting the arm as well as the leg due to a lesion in the left posterior limb of the internal capsula. After treatment with intravenous thrombolysis the patient made a good recovery. Notably, in this case unilateral asterixis affected the arm as well as the leg, resulting in postural and gait instability. In addition, damage in the basal ganglia-thalamo-cortical network, as in our patient, has to be distinguished from other supratentorial causes of acute asterixis like thalamic or frontal lobe lesions linked to the cerebello-brainstem-thalamo-frontal lobe circuits.
