This study is the first to report the clinical lifetime of Varian Kapton sealed ion chambers as a retrospective review. The data have been analyzed using ion chamber gain values, daily quality assurance results, monthly quality assurance results, and delivered treatment field data were analyzed to comprehensively review trends. Project description:PURPOSE:While critical for safe and accurate radiotherapy, monthly quality assurance of medical linear accelerators is time-consuming and takes physics resources away from other valuable tasks. The previous methods at our institution required 5 hours to perform the mechanical and dosimetric monthly linear accelerator quality assurance tests. An improved workflow was developed to perform these tests with higher accuracy, with fewer error pathways, in significantly less time. METHODS:A commercial ion chamber array (IC profiler, Sun Nuclear, Melbourne, Florida) is combined with automation scripts to consolidate monthly linear accelerator QA. The array was used to measure output, flatness, symmetry, jaw positions, gated dose constancy, energy constancy, collimator walkout, crosshair centering, and dosimetric leaf gap constancy. Treatment plans were combined with automation scripts that interface with Sun Nuclear's graphical user interface. This workflow was implemented on a standard Varian clinac, with no special adaptations, and can be easily applied to other C-arm linear accelerators. RESULTS:These methods enable, in 30 minutes, measurement and analysis of 20 of the 26 dosimetric and mechanical monthly tests recommended by TG-142. This method also reduces uncertainties in the measured beam profile constancy, beam energy constancy, field size, and jaw position tests, compared to our previous methods. One drawback is the increased uncertainty associated with output constancy. Output differences between IC profiler and farmer chamber in plastic water measurements over a 6-month period, across 4 machines, were found to have a 0.3% standard deviation for photons and a 0.5% standard deviation for electrons, which is sufficient for verifying output accuracy according to TG-142 guidelines. To minimize error pathways, automation scripts which apply the required settings, as well as check the exported data file integrity were employed. CONCLUSIONS:The equipment, procedure, and scripts used here reduce the time burden of routine quality assurance tests and in most instances improve precision over our previous methods.

Project description:BackgroundThis study set out to evaluate the effect of dose rate on normal tissues (the lung, in particular) and the variation in the treatment efficiency as determined by the monitor unit (MU) and energy applied in Linac-based volumetric arc therapy (VMAT) total marrow irradiation (TMI).MethodsLinac-based VMAT plans were generated for the TMI for six patients. The planning target volume (PTV) was divided into six sub-volumes, each of which had their own isocenter. To examine the effect of the dose rate and energy, a range of MU rates (40, 60, 80, 100, 300, and 600 MU/min) were selected for 6, 10, and 15 MV. All the plans were verified by portal dosimetry.ResultsThe dosimetric parameters for the target and normal tissue were consistent in terms of the energy and MU rate. The beam-on time was changed from 59.6 to 6 min for 40 and 600 MU/min. When 40 MU/min was set for the lung, the dose rate delivered to the lung was less than 6 cGy/min (that is, 90%), while the beam-on time was approximately 10 min. The percentage volume of the lung receiving 20 cGy/min was 1.47, 3.94, and 6.22% at 6, 10, and 15 MV, respectively. However, for 600 MU/min, the total lung volume received over 6 cGy/min regardless of the energy, and over 20 cGy/min for 10 and 15 MV (i.e., 54.4% for 6 MV).ConclusionsIn TMI treatment, reducing the dose rate administered to the lung can decrease the incidence of pulmonary toxicity. To reduce the probability of normal tissue complications, the selection of the lowest MU rate is recommended for fields including the lung. To minimize the total treatment time, the maximum MU rate can be applied to other fields.

Project description:We evaluated the influence of impurities in the vacuum chamber used for the fabrication of organic light-emitting diodes on the lifetime of the fabricated devices and found a correlation between lifetime and the device fabrication time. The contact angle of the ITO substrates stored the chamber under vacuum were used to evaluate chamber cleanliness. Liquid chromatography-mass spectrometry was performed on Si wafers stored in the vacuum chamber before device fabrication to examine the impurities in the chamber. Surprisingly, despite the chamber and evaporation sources being at room temperature, a variety of materials were detected, including previously deposited materials and plasticizers from the vacuum chamber components. We show that the impurities, and not differences in water content, in the chamber were the source of lifetime variations even when the duration of exposure to impurities only varied before and after deposition of the emitter layer. These results suggest that the impurities floating in the vacuum chamber significantly impact lifetime values and reproducibility.

Project description:BackgroundCritically ill pediatric patients frequently require hemoglobin monitoring. Accurate noninvasive Hb (SpHb) would allow practitioners to decrease anemia from repeated blood draws, traumatic blood draws, and a decreased number of laboratory Hb (LabHb) medical tests. The Food and Drug Administration has approved the Masimo Pronto SpHb and associated Rainbow probes; however, its use in the pediatric intensive care unit (PICU) is controversial. In this study, we define the degree of agreement between LabHb and SpHb using the Masimo Pronto SpHb Monitor and identify clinical and demographic conditions associated with decreased accuracy.Materials and methodsWe performed a prospective, observational study in a large PICU at an academic medical center. Fifty-three pediatric patients (30-d and 18-y-old), weighing >3 kg, admitted to the PICU from January-April 2013 were examined. SpHb levels measured at the time of LabHb blood draw were compared and analyzed.ResultsOnly 83 SpHb readings were obtained in 118 attempts (70.3%) and 35 readings provided a result of "unable to obtain." The mean LabHb and SpHb were 11.1 g/dL and 11.2 g/dL, respectively. Bland-Altman analysis showed a mean difference of 0.07 g/dL with a standard deviation of ±2.59 g/dL. Pearson correlation is 0.55, with a 95% confidence interval between 0.38 and 0.68. Logistic regression showed that extreme LabHb values, increasing skin pigmentation, and increasing body mass index were predictors of poor agreement between SpHb and LabHb (P < 0.05). Separately, increasing body mass index, hypoxia, and hypothermia were predictors for undetectable readings (P < 0.05).ConclusionsThe Masimo Pronto SpHb Monitor provides adequate agreement for the trending of hemoglobin levels in critically ill pediatric patients. However, the degree of agreement is insufficient to be used as the sole indicator for transfusion decisions and should be used in context of other clinical parameters to determine the need for LabHb in critically ill pediatric patients.

Project description:Precise lifetime predictions for lithium-ion cells are crucial for efficient battery development and thus enable profitable electric vehicles and a sustainable transformation towards zero-emission mobility. However, limitations remain due to the complex degradation of lithium-ion cells, strongly influenced by cell design as well as operating and storage conditions. To overcome them, a machine learning framework is developed based on symbolic regression via genetic programming. This evolutionary algorithm is capable of inferring physically interpretable models from cell aging data without requiring domain knowledge. This novel approach is compared against established approaches in case studies, which represent common tasks of lifetime prediction based on cycle and calendar aging data of 104 automotive lithium-ion pouch-cells. On average, predictive accuracy for extrapolations over storage time and energy throughput is increased by 38% and 13%, respectively. For predictions over other stress factors, error reductions of up to 77% are achieved. Furthermore, the evolutionary generated aging models meet requirements regarding applicability, generalizability, and interpretability. This highlights the potential of evolutionary algorithms to enhance cell aging predictions as well as insights.

Project description:Ion pairing is commonly considered as a culprit for the reduced ionic conductivity in polymer electrolyte systems. However, this simple thermodynamic picture should not be taken literally, as ion pairing is a dynamical phenomenon. Here we construct model poly(ethylene oxide)-bis(trifluoromethane)sulfonimide lithium salt systems with different degrees of ion pairing by tuning the solvent polarity and examine the relation between the cation-anion distinct conductivity σ+-d and the lifetime of ion pairs τ+- using molecular dynamics simulations. It is found that there exist two distinct regimes where σ+-d scales with 1/τ+- and τ+-, respectively, and the latter is a signature of longer-lived ion pairs that contribute negatively to the total ionic conductivity. This suggests that ion pairs are kinetically different depending on the solvent polarity, which renders the ion-pair lifetime highly important when discussing its effect on ion transport in polymer electrolyte systems.

Project description:This study explores the use of static chamber boxes to detect whether there are fugitive emissions of greenhouse gases (GHGs) from a willow chip storage heap. The results from the boxes were compared with those from 3-m stainless steel probes inserted into the core of the heap horizontally and vertically at intervals. The results from probes showed that there were increases of carbon dioxide (CO2) concentrations in the heap over the first 10 days after heap establishment, which were correlated with a temperature rise to 60 °C. As the CO2 declined, there was a small peak in methane (CH4) concentration in probes orientated vertically in the heap. Static chambers positioned at the apex of the heap detected some CO2 fluxes as seen in the probes; however, the quantities were small and random in nature. A small (maximum 5 ppm) flux in CH4 occurred at the same time as the probe concentrations peaked. Overall, the static chamber method was not effective in monitoring fluxes from the heap as there was evidence that gases could enter and leave around the edges of the chambers during the course of the experiment. In general, the use of standard (25 cm high) static chambers for monitoring fluxes from wood chip heaps is not recommended.

Project description:The DAVID is a transparent, multi-wire transmission-style detector that attaches to a linear accelerator (linac) collimator for use as an in vivo detector. Currently, the normal method for using the DAVID is to measure a signal at the time of phantom-based pretreatment verification and use that signal as a baseline to compare with in vivo measurements for subsequent treatment fractions. The device has previously been shown to be both stable and accurate.(1,2) This work presents the development of a predictive algorithm for the expected signal, eradicating the need to spend time on the linac prior to treatment, and thereby making the process more efficient. The DAVID response at each wire is a consequence of both primary radiation, from the leaf pair associated with the wire, and scatter radiation as a result of radiation incident on other parts of the detector scattering in the Perspex plate. The primary radiation was shown to be linearly proportional to both leaf separation and delivered monitor units (MU). The scatter signal dropped off exponentially with regard to distance. Both of these effects were modeled; the resulting algorithm was used to predict the response from ten five-field IMRT head and neck plans. The system predicted all DAVID signals to within 5%, and was able to detect artificially generated changes in linac output. Having shown that the algorithm works, a new working paradigm is suggested, and the errors that can be detected are outlined.

Project description:An essential component of quality assurance in radiation therapy is verifying the accuracy of monitor unit calculations. For tangential breast fields, monitor unit differences between primary calculations and second checks are usually larger than considered acceptable at other anatomical sites. A simple model to reconcile the differences between sophisticated and simple algorithms is presented, based on estimating the the volume irradiated by the field, replacing the breast contour with a rectangular block having an equal volume but a new field width which provides almost equivalent scatter to the prescription point. This analysis can also assist the treatment planning physicist in selecting a tolerance window for verifying monitor unit calculations for tangential breast fields.

Project description:PurposeTo analyze in-room video recordings of operating room (OR) to intensive care unit (ICU) handoffs to determine tempo and quality of team interactions on nights and weekends compared with weekdays, and to demonstrate how existing telemedicine technology can be used to evaluate handoffs.MethodThis prospective observational study of OR-to-ICU bedside handoffs was conducted in the surgical ICU of the Hospital of the University of Pennsylvania in July 2014-January 2015. Handoff video recordings were obtained for quality improvement purposes using existing telemedicine cameras. Evaluators used adapted validated in-person assessment measures to analyze basic characteristics and quality measures (timing, report types, report duration, presence of physical exam, teamwork skills, engagement, report delivery skills, listening skills, interruptions, unprofessional comments or actions).ResultsSixteen weekday and 16 night and weekend handoffs were compared. There were no significant differences in basic characteristics. Most quality measures were similar on weekdays compared with nights and weekends. Surgeons demonstrated better report delivery skills and engagement on nights and weekends (P = .002 and P = .04, respectively), whereas OR anesthesiologists' scores were similar during both time frames.ConclusionsThis study presents a novel approach of assessing handoff quality in OR-to-ICU handoffs using an existing telemedicine infrastructure. Using this approach, quality measures of night and weekend handoffs were found to be no worse-and sometimes better-than those during weekdays. Video analysis may emerge as an ideal unobtrusive quality improvement methodology to monitor handoffs and improve education and compliance with institutional handoff policies.
