Self-Stigma in Parkinson's Disease: A 3-Year Prospective Cohort Study.

ABSTRACT:

Purpose

Self-stigma is common in patients with Parkinson's disease (PD) and may lead to social isolation and delayed search for medical help. We conducted a 3-year prospective longitudinal study to investigate the development and evolution of self-stigma in patients with early stage PD and to explore the associated and predictive factors of self-stigma in PD.

Method

A total of 224 patients with early stage PD (disease duration <3 years) were enrolled at baseline and followed up annually for 3 consecutive years. Self-stigma was assessed by the stigma subscale of the Parkinson's Disease Questionnaire (items 23-26). The generalized estimating equation model was used to investigate the associated factors of self-stigma over 3 years, and the binary logistic model was used to explore the predictors of self-stigma in patients with PD without self-stigma at baseline.

Results

The prevalence of self-stigma decreased from 58.0% at baseline to 49.2% after 3 years. The Hamilton Depression Rating Scale (HDRS) score was the only associated factor [B: 0.160 (1.106-0.214), P < 0.001] of self-stigma over 3 years and the only predictor [OR: 1.252 (1.044-1.502), P = 0.015] of the onset of self-stigma.

Conclusion

Self-stigma is very common in PD, but its prevalence tends to decrease as the disease progresses. Depression was the only associated and predictive factor of self-stigma in PD and could be an effective target of alleviating self-stigma.

Project description:Identification of blood biomarkers that prospectively predict progression of Mycobacterium tuberculosis infection to tuberculosis disease might lead to interventions that combat the tuberculosis epidemic. We aimed to assess whether global gene expression measured in whole blood of healthy people allowed identification of prospective signatures of risk of active tuberculosis disease. RESULTS:Between July 6, 2005, and April 23, 2007, we enrolled 6363 from the ACS study and 4466 from independent South African and Gambian cohorts. 46 progressors and 107 matched controls were identified in the ACS cohort. A 16 gene signature of risk was identified. The signature predicted tuberculosis progression with a sensitivity of 66Â·1% (95% CI 63Â·2â68Â·9) and a specificity of 80Â·6% (79Â·2â82Â·0) in the 12 months preceding tuberculosis diagnosis. The risk signature was validated in an untouched group of adolescents (p=0Â·018 for RNA sequencing and p=0Â·0095 for qRT-PCR) and in the independent South African and Gambian cohorts (p values <0Â·0001 by qRT-PCR) with a sensitivity of 53Â·7% (42Â·6â64Â·3) and a specificity of 82Â·8% (76Â·7â86) in 12 months preceding tuberculosis. Interpretation: The whole blood tuberculosis risk signature prospectively identified people at risk of developing active tuberculosis, opening the possibility for targeted intervention to prevent the disease. In this prospective cohort study, we followed up healthy, South African adolescents aged 12â18 years from the adolescent cohort study (ACS) who were infected with M tuberculosis for 2 years. We collected blood samples from study participants every 6 months and monitored the adolescents for progression to tuberculosis disease. A prospective signature of risk was derived from whole blood RNA sequencing data by comparing participants who developed active tuberculosis disease (progressors) with those who remained healthy (matched controls). After adaptation to multiplex qRT-PCR, the signature was used to predict tuberculosis disease in untouched adolescent samples and in samples from independent cohorts of South African and Gambian adult progressors and controls. Participants of the independent cohorts were household contacts of adults with active pulmonary tuberculosis disease.

Project description:BackgroundIndividuals with Parkinson's Disease (PD) often experience not only physical symptoms but also a higher prevalence of psychological issues, including depression, anxiety, schizophrenia, and psychotic symptoms, compared to the general population. The relationship between these physical and psychological symptoms remains poorly understood, particularly in relation to commonly used measures of physical function and a wider variety of psychological symptoms. Investigating this link is essential for optimising patient care. This cohort study is registered with OSF (https://osf.io/c7tvd).ObjectiveThe primary aim of this study is to investigate the association between physical and psychological symptoms in PD. We will focus specifically on measures of physical function such as balance and mobility, assess the similarities between physiotherapist and participant-rated measures, and monitor changes over time.MethodsThis cohort study plans to recruit 30 participants with PD, who will be monitored over a 12-month period. Data will be collected at baseline and completion, providing two data points per participant. Selected outcome measures will include physical function (objective measures), non-motor symptoms, participants perceived function, and engagement in physical activity. Correlations between outcomes will be analysed, and multivariate regression modelling may be employed for time-series analysis to evaluate how relationships change over time. Descriptive summaries of all outcomes will be presented in tables.ResultsEthical approval for this study was obtained on 15th July 2024, with participant recruitment scheduled to begin in October 2024. Data collection is expected to conclude by August 2026, with final results anticipated within six months of study completion.ConclusionsThis study will be the first to monitor this specific set of physical and psychological outcomes over time in individuals with PD. The findings are expected to provide valuable insights into the relationship between these symptoms, informing future research and potentially leading to larger-scale, multi-site studies conducted over extended assessment periods.
