ABSTRACT: In this article, we argue that the size and cultural proximity of immigrant populations in people's residential surroundings shape national and European identities. This means that the type of migrant population activates cultural threat perceptions and opportunities for contact to varying degrees. Geocoded survey data from the Netherlands suggests that large non-Western immigrant shares are associated with more exclusive national identities, while mixed contexts with Western and non-Western populations show more inclusive identities. These results suggest that highly diverse areas with mixed immigrant populations hold a potential for more tolerance. In contrast, exclusive national identities become strongly pronounced under the presence of sizeable culturally distant immigrant groups.

Project description:PurposeTo investigate the representation of various gender, racial, and ethnic groups in ophthalmology clinical trials conducted in the United States (US) between 1997 and 2022.DesignRetrospective cross-sectional study.ParticipantsWe included all participants in completed phase II/III, III, and IV ophthalmology clinical trials reported on the ClincialTrials.gov database.MethodsThe proportional enrollment of each racial/ethnic and gender group in the clinical trials was calculated and compared with the US population. We also investigated the impact of various clinical trial features on the rate of reporting demographic information and enrollment of minorities.Main outcome measuresProportional enrollment of each gender and race/ethnicity group compared with the US Census.ResultsOf the total clinical trials included in the study, less than half (43.6%) provided information on the racial or ethnic backgrounds of their participants. The majority of the enrollees in trials were female (median: 57.5%, interquartile range [IQR]: 47.2%-65.8%). Among the trials that reported race and/or ethnicity data, White populations were overrepresented (median: 76.6%, IQR: 69.0%-84.0%, P = 0.001), and minorities, including Asian, Hispanic, and "other" groups, were underrepresented compared with the 2010 US Census (P < 0.001). Enrollment of Black individuals was found to be comparable to the US population estimates (median: 12.4%, IQR: 6.2%-20.8%, P = 0.44). The trial phase, the number of study participants, the primary clinical condition, and the year the trial started all affected demographic reporting and minority enrollments.ConclusionsOur findings highlight the need for increased efforts to promote diversity and inclusivity in ophthalmology clinical trials. Project description:Classic and clinical variant galactosemia (CG/CVG) are allelic, autosomal recessive disorders that result from deficiency of galactose-1-P uridylyltransferase (GALT). CG/CVG has been reported globally among patients of diverse ancestries, but most large studies of outcomes have included, almost exclusively, patients categorized as White or Caucasian. As a first step to explore whether the cohorts studied are representative of the CG/CVG population at large, we sought to define the racial and ethnic makeup of CG/CVG newborns in a diverse population with essentially universal newborn screening (NBS) for galactosemia: the United States (US). First, we estimated the predicted racial and ethnic distribution of CG/CVG by combining the reported demographics of US newborns from 2016 to 2018 with predicted homozygosity or compound heterozygosity of pathogenic, or likely pathogenic, GALT alleles from the relevant ancestral groups. Incorporating some simplifying assumptions, we predicted that of US newborns diagnosed with CG/CVG, 65% should be White (non-Hispanic), 23% should be Black (non-Hispanic), 10% should be Hispanic, and 2% should be Asian (non-Hispanic). Next, we calculated the observed racial and ethnic distribution of US newborns diagnosed with CG/CVG using available de-identified data from state NBS programs from 2016 to 2018. Of the 235 newborns in this cohort, 41 were categorized as other or unknown. Of the remaining 194, 66% were White (non-Hispanic or ethnicity unknown), 16% were Black (non-Hispanic or ethnicity unknown),15% were Hispanic, and 2% were Asian (non-Hispanic or ethnicity unknown). This observed distribution was statistically indistinguishable from the predicted distribution. To the limits of our study, these data confirm the racial and ethnic diversity of newborns with CG/CVG in the US, demonstrate an approach for estimating CG/CVG racial and ethnic diversity in other populations, and raise the troubling possibility that current understanding of long-term outcomes in CG/CVG may be skewed by ascertainment bias of the cohorts studied.

Project description:How are evaluative reactions pertaining post-national citizenship identities interrelated and what are the potential mechanisms how post-national identities evolve? Previous efforts to operationalize and measure post-national citizenship identities leave it open how people's stances on different issues are related and suffer from a variety of theoretical and methodological shortcomings regarding the nature of political attitudes and ideologies. A recently proposed approach conceptualizes ideologies as networks of causally connected evaluative reactions to individual issues. Individual evaluative reactions form the nodes in a network model, and these nodes can influence each other via linked edges, thereby giving rise to a dynamic thoughts system of networked political and identity-related views. To examine this system at large, we apply network analysis to data from the European Values Study. Specifically, we investigate 33 evaluative reactions regarding national and supra-national identity, diversity, global empathy, global environmentalism, immigration, and supra-national politics. The results reveal a strongly connected network of citizenship identity-related attitudes. A community analysis reveals larger clusters of strongly related evaluative reactions, which are connected via bridges and hub nodes. Centrality analysis identifies evaluative reactions that are strategically positioned in the network, and network simulations indicate that persuasion attempts targeted at such nodes have greater potential to influence the larger citizenship identity than changes of more peripheral attitude nodes. We lastly show that socio-demographic characteristics are not only associated with the overall level of post-national citizenship, but also with the network structure, suggesting that these structural differences can affect the network function as people develop national or post-national citizenship identities, or respond to external events. These results provide new insights into the structure of post-national identities and the mechanism how post-national identities might evolve. We end with a discussion of future opportunities to study networked attitudes in the context of civic and citizenship education.
