Effective Response to Hospital Congestion Scenarios: Simulation-Based Evaluation of Decongestion Interventions.

ABSTRACT: Hospital overcrowding is becoming a major concern in the modern era due to the increasing demand for hospital services. This study seeks to identify effective and efficient ways to resolve the serious problem of congestion in hospitals by testing a range of decongestion strategies with simulated scenarios. In order to determine more efficient solutions, interventions with smaller changes were consistently tested at the beginning through a simulation platform. In addition, the implementation patterns were investigated, which are important to hospital managers with respect to the decisions made to control hospital congestion. The results indicated that diverting a small number of ambulances seems to be more effective and efficient in congestion reduction compared to other approaches. Furthermore, instead of implementing an isolated approach continuously, combining one approach with other strategies is recommended as a method for dealing with hospital overcrowding.

Project description:Few mobile robot developers already test their software on simulated robots in virtual environments or sceneries. However, the majority still shy away from simulation-based test campaigns because it remains challenging to specify and execute suitable testing scenarios, that is, models of the environment and the robots' tasks. Through developer interviews, we identified that managing the enormous variability of testing scenarios is a major barrier to the application of simulation-based testing in robotics. Furthermore, traditional CAD or 3D-modelling tools such as SolidWorks, 3ds Max, or Blender are not suitable for specifying sceneries that vary significantly and serve different testing objectives. For some testing campaigns, it is required that the scenery replicates the dynamic (e.g., opening doors) and static features of real-world environments, whereas for others, simplified scenery is sufficient. Similarly, the task and mission specifications used for simulation-based testing range from simple point-to-point navigation tasks to more elaborate tasks that require advanced deliberation and decision-making. We propose the concept of composable and executable scenarios and associated tooling to support developers in specifying, reusing, and executing scenarios for the simulation-based testing of robotic systems. Our approach differs from traditional approaches in that it offers a means of creating scenarios that allow the addition of new semantics (e.g., dynamic elements such as doors or varying task specifications) to existing models without altering them. Thus, we can systematically construct richer scenarios that remain manageable. We evaluated our approach in a small simulation-based testing campaign, with scenarios defined around the navigation stack of a mobile robot. The scenarios gradually increased in complexity, composing new features into the scenery of previous scenarios. Our evaluation demonstrated how our approach can facilitate the reuse of models and revealed the presence of errors in the configuration of the publicly available navigation stack of our SUT, which had gone unnoticed despite its frequent use.

Project description:Many epidemiological questions concern potential interventions to alter the pathways presumed to mediate an association. For example, we consider a study that investigates the benefit of interventions in young adulthood for ameliorating the poorer mid-life psychosocial outcomes of adolescent self-harmers relative to their healthy peers. Two methodological challenges arise. First, mediation methods have hitherto mostly focused on the elusive task of discovering pathways, rather than on the evaluation of mediator interventions. Second, the complexity of such questions is invariably such that there are no well-defined mediator interventions (i.e. actual treatments, programs, etc.) for which data exist on the relevant populations, outcomes and time-spans of interest. Instead, researchers must rely on exposure (non-intervention) data, that is, on mediator measures such as depression symptoms for which the actual interventions that one might implement to alter them are not well defined. We propose a novel framework that addresses these challenges by defining mediation effects that map to a target trial of hypothetical interventions targeting multiple mediators for which we simulate the effects. Specifically, we specify a target trial addressing three policy-relevant questions, regarding the impacts of hypothetical interventions that would shift the mediators' distributions (separately under various interdependence assumptions, jointly or sequentially) to user-specified distributions that can be emulated with the observed data. We then define novel interventional effects that map to this trial, simulating shifts by setting mediators to random draws from those distributions. We show that estimation using a g-computation method is possible under an expanded set of causal assumptions relative to inference with well-defined interventions, which reflects the lower level of evidence that is expected with ill-defined interventions. Application to the self-harm example in the Victorian Adolescent Health Cohort Study illustrates the value of our proposal for informing the design and evaluation of actual interventions in the future.

Project description:BackgroundIntroducing interprofessional education (IPE) in healthcare curricula can prepare students for healthcare practices that have become increasingly complex. The use of simulation is promoted to support IPE. This study explores healthcare students' experiences of participating in common, sub-acute patient scenarios that routinely occur in clinical practice in primary care. More specifically, it looks at how sub-acute patient scenarios from primary care can help develop interprofessional collaborative competence.MethodsMedical students (N = 10), master's students in advanced geriatric nursing (N = 8) and bachelor's students in nursing (N = 9) participated in the simulations. The students were in their last or second-to-last year of education. We conducted five semi-structured focus group interviews with the participants' directly after the simulation training to elicit experiences related to the scenarios, the simulation and interprofessional collaboration. The transcripts were analysed using systematic text condensation. To supplement the focus group interviews, the students also completed the interprofessional collaborative competency attainment survey (ICCAS), which measures the students' self-assessed interprofessional competence.ResultsThree main themes emerged from the analysis of the focus group interviews: realism, uncertainty and reflection. The students emphasised the importance of authentic and recognisable scenarios. They said the vague and unspecific patient symptoms created uncertainty in the situation, making it difficult to understand the patient's diagnosis. Despite that uncertainty, they described the experience as positive. Further, the students expressed that the simulation increased their confidence in interprofessional collaboration and prepared them for future work. The results from the ICCAS questionnaire showed that the students reported a subjective positive change in their interprofessional competence after participating in the scenarios.ConclusionsThis study showed that simulation-based IPE with sub-acute primary care scenarios contributes to develop interprofessional collaborative competence in healthcare education. Sub-acute scenarios can supplement the more common approaches with acute care scenarios and aid in developing the collaborative competence required to work in healthcare teams.
