The Impact of Choice on Colorectal Cancer Screening

ABSTRACT: In May of 2021, the United States Preventive Service Task Force (USPSTF) updated their colorectal cancer (CRC) screening guidelines by recommending screening at an earlier age for average-risk adults starting at the age of 45 years old (Grade B recommendation). This is in addition to their Grade A recommendations of continuing to screen average-risk adults ages 50-75 years old. UCLA Health previously implemented a fecal immunochemical test (FIT) outreach program wherein FIT kits are mailed to average-risk patients overdue for CRC screening twice annually to promote screening uptake. As the investigators health system aims to screen the newly eligible population of average-risk patients between the ages of 45-49, the investigators proposed randomized controlled trial is aimed to determine the most effective patient outreach approach to maximize screening uptake within this age-group.

Transcriptome profiling of Drosophila motor neurons with age

Project description:The purpose of this array was to investigate age-dependent changes the transcriptome of a highly homogenous cell population (Drosophila motor neurons) in order to help understand the contribution of changes in gene expression to age-dependent motor deficiencies. Drosophila motor neurons expressing GFP (via the UAS-Gal4 binary expression system) were isolated using flow cytometry from flies that were 7, 35 or 45 days old. The microarray design was highly redundant for calcium-binding and ion channel genes as we had apriori reasons to believe changes in these genes would be important. The ages were chosen because previous work had identified a major shift in neurotransmitter release between the ages of 35 and 45 days in Drosophila motor neurons. Although no significant changes were identified in that window, several genes showed elevated expression in old (35 day and 45 day) motor neurons compared to young (7 day) including matrix metalloproteinase 1 (dMMP1).

Transcriptome response of mouse mammary gland from age 12 to 30 months of age in Esr1 and CYP19A1 genetically engineered mouse models of breast cancer risk in the presence and absence of transgene induction

Project description:Breast cancer is an age-related cancer in women with two peaks, one at 50 and one at 70 years of age. Here we used two conditional genetically engineered mouse models of breast cancer risk to study mammary gland transcriptional changes that occur as female mice age from 12 to 30 months of age, paralleling aging from 58 to 85 years of age in women. The two models express either mammary epithelial cell-targeted Estrogen Receptor (ER) alpha (Esr1) or Aromatase (CYP19A1A) over-expression beginning at age 12 or 18 months of age. Both of these risk factors increase estrogen pathway signaling, either directly though the receptor or by increasing local estrogen production. The goals of the study are to determine how quickly significant transcriptional changes occur in the mammary gland following transgene induction, to determine how the transcriptome becomes modified as the mice age past reproductive senescence through old and very old life stages, and to identify similarities and differences in the transcriptomes between the two risk conditions at different ages and conditions. The first specific objective of this transcriptome-based study is to identify significantly differentially expressed genes within each model between different ages and times of transgene induction. This includes transcriptional changes induced by 1 week, 6, 12 and 18 months of transgene expression with transgene expression initiated at age 12 months as well as transcriptional changes induced by 1 week and 6 months of transgene expression initiated at age 18 months. The second specific objective is to compare the two models at different ages (12, 18, 24 and 30 months of age) and different transgene induction times (6, 12 and 18 months) to identify differentially expressed genes between the models. Comparative control mice from each model include age 18- and 24-month-old mice without transgene induction.

Pre-Transition Rules: Age increased by 1 and BP by Age/10 each simulation cycle. Post-Transition Rules: Treatment = BP>140 , becomes 1 when BP crosses 140 threshold; BP =BP-Treatment*10 , meaning a drop of 10 once treatment is applied; CostThisYear = Age + \Treatment*10 , cost depends on age and if treatment was taken; Cost= Cost + CostThisYear , it accumulates cost over time. Initial conditions: Healthy = (50 Male, 50 Female with Age =1,2,...,50 for each individual), BP =120, Sick = (0,0) and Dead = (0,0). Output: Number of men and women in each disease state for years 1-10 and their ages and costs in each state. A stratified report by male and female and young – up to age 30 and old above age 30 is produced.

Age-Dependent Changes in the Plasma Proteome of Healthy Adults

Project description:Human blood plasma is a complex which communicates with most parts of the body and reflects changes in the state of the organism, therefore identifying age-related biomarkers can help to predict and monitor age-related physiological decline and diseases, as well as to find new treatments for diseases. In this study, TMT-LC-MS/MS was utilized to screen for the differentially expressed plasma proteins in 118 healthy adults with different ages. Participants were divided into three groups: 21-30 (Young), 41-50 (Middle) and ≥60 (Old), the number of differentially expressed proteins when Young vs Middle, Middle vs Old and Young vs Old were 82, 22 and 99, respectively. These proteins were found to be involved in numerous physiological processes such as “negative regulation of smooth muscle cell proliferation” and “blood coagulation”. Meanwhile when Young vs Middle or Old, “Complement and coagulation cascades” was confirmed the top enriched pathway by KEGG pathway enrichment analysis. Functional phenotyping of the proteome demonstrated that the plasma proteomic profiles of Young adults were strikingly dissimilar to the Middle or Old adults. The results of this study mapped the variation in expression of plasma proteins, and provided information about possible biomarkers/treatments for different kinds of age-related function disorders.

Test Up Now Education Program

Project description:This study will test the effectiveness of an outreach strategy to increase colorectal cancer (CRC) screening in African Americans. The investigators will recruit 250 African Americans ages 45-64 years who are not up-to-date with CRC screening or have never been screened, with the goal to evaluate screening knowledge, behavior, and intervention effects on CRC screening outcomes. Participants will be randomly assigned to the TUNE-UP intervention or a control group. The TUNE-UP intervention arm will utilize a community health advisor (CHA) to encourage return of stool blood testing kits through cell phone outreach. The control group will receive educational materials about CRC screening plus a resource list but no CHA counseling support or cellphone / text contact. The primary study outcome is receipt of CRC screening (colonoscopy or FIT) following the intervention. The secondary outcomes will include CRC screening knowledge, self-efficacy (confidence to receive CRC screening), intention to screen, and follow-up in the case of an abnormal test result. The research objective is to test the CHA intervention effectiveness for promoting stool blood testing as a preferred screening test in an under-screened African American population.
