SPECTA RP-1759-AYA Sarcoma cohort

ABSTRACT: EORTC-SPECTA RP1759 study focuses on cancers in adolescent and young adult (AYA), age 12 to 29 years. This study explicitly recruited AYA patients and therefore collected empirical data to bridge the molecular gap between pediatric and adult oncology. The initial pilot study, activated in February 2019 across Europe, aimed to recruit 100 AYA patients (aged 12-29 years) with newly diagnosed or relapsed high-grade gliomas and high-grade bone and soft tissue sarcomas. The primary objective of the pilot is to determine feasibility and recruitment rates. Here we submit the data from patients with newly diagnosed, recurrent intermediate or high-grade sarcoma that were collected and analyzed during the study. More specifically, we submit raw FASTQ files obtained from whole exome sequencing (WES) of matching tumor and blood samples, tumor RNA sequencing and DNA methylation profiling.

Methylation profiling of BRAF-altered pediatric low-grade gliomas

Project description:Gliomas are a major cause of cancer-related death in adolescents and young adults (AYA, ages 15-39 years). Different molecular alterations drive gliomas in children and adults leading to distinct biology and clinical consequences. At the convergence of these age groups - the AYA population – little is known about the implications of pediatric vs adult-type alterations. To address this, we analyzed a population-based cohort of 1456 clinically and molecularly characterized gliomas aged 0-39 years. Potentially targetable pediatric-type alterations were found in 31% of AYA gliomas and conferred superior outcomes compared to adult-type alterations (p<0.01). AYA low-grade gliomas with specific RAS/MAPK alterations exhibited senescent phenotype, tended to arise in different locations and were associated with superior outcome compared with children, suggesting different cellular origins. IDH mutations, BRAF p.V600E and FGFR alterations were associated with risk of malignant transformation for hemispheric tumors with worse outcome with increased age. These observations have significant implications for understanding gliomagenesis. These insights may provide rationale for earlier intervention for certain tumors to disrupt the typical behavior, leading to improved outcome.

The BurnAlong Pilot Study for Adolescent and Young Adult Cancer Survivors

Project description:The purpose of this prospective, interventional, single-arm pilot study is to evaluate whether virtually delivered group-based physical activity is feasible for adolescent and young adult (AYA) cancer survivors. AYAs who were diagnosed with cancer and have completed cancer treatment will be recruited for this study. This study will enroll 20 participants in total and will last approximately 3 months.

Gene expression profiling in pediatric and adolescent and young adult rhabdomyosarcomas

Project description:Adolescents and young adults (AYA) with rhabdomyosarcoma (RMS) form a subgroup of patients whose optimal clinical management and best possible access to care remain a challenge and survival lacks behind that of children while diagnosed with histologically similar tumors. Understanding the tumor biology that differentiate children from AYA-RMS could provide critical information and drive new initiatives to improve their final outcome. Gene expression profiling was evaluated in a RMS series of 48 tumor and 13 non-neoplastic tissues. GEP analysis detected 793 age-correlated genes in tumors. NOTCH2, FGFR1 were significantly down-modulated in AYA-RMS. GEP showed an increase in CD4 memory resting cells and a decrease in γδ T-cells in AYA-RMS. IHC analysis demonstrated an increase of infiltrated CD4, CD8 and neutrophils in AYA-RMS tumors.

The plasma miRnome and venous thromboembolism in high-grade glioma: miRNA sequencing of a nested case-control cohort

Project description:Background: Patients with high-grade gliomas are at high risk of venous thromboembolism (VTE). MicroRNAs (miRNAs) are small non-coding RNAs with multiple roles in tumor biology, hemostasis and platelet function. We aimed to explore the association between plasma miRNAs and risk of VTE in high-grade glioma. Results: We conducted a nested-case control study within 152 patients with WHO grade IV glioma that had been included in the Vienna Cancer and Thrombosis Study (CATS), a prospective cohort study focused on risk factors for VTE in newly diagnosed or recurrent cancer. At study inclusion a single blood draw was taken, and patients were thereafter followed for a maximum of two years. During that time, 24 patients (16%) developed VTE. Of the other 128 patients, we randomly selected 24 age- and sex-matched controls. After sample quality control, the final group size was 21 VTE-patients and 23 without VTE. Small RNA next-generation sequencing of plasma was performed. We observed that hsa-miR-451a was globally the most abundant miRNA. Notably, 51% of all miRNAs showed a correlation with platelet count. The analysis of miRNAs differentially regulated in VTE patients – with and without platelet adjustment – identified potential VTE biomarker candidates such as has-miR- 221-3p. Conclusion: We here provide one of the largest and deepest peripheral blood miRNA datasets of high-grade glioma patients so far. Further, we confirm previous observations of a considerable impact of platelet count on the blood miRNome. And, finally, we present first VTE biomarker candidates that can serve as the starting point for future confirmatory research.

Ribosome biogenesis and its regulation by the exportin, CSE1L, are selective 4 dependencies in childhood and young adult acute myeloid leukemia

Project description:Nuclear transport receptors, termed importins and exportins, regulate protein transport between the nucleus and cytoplasm through the nuclear pore. Although genetic alterations in the nuclear pore complex are more common in childhood, adolescent and young adult (AYA) AML, nucleocytoplasmic transport has not been studied extensively in childhood/AYA AML. We compared the expression of genes associated with nucleocytoplasmic transport in young and older AML patients and identified enrichment of nucleocytoplasmic transport pathways in childhood/AYA AML. Of the nucleocytoplasmic genes, CSE1L was the top expressed exportin in young AML patients. CSE1L depletion reduced growth, viability and clonogenicity in young (<40 years) AML cell lines and patient samples but spared normal hematopoietic cells. Mechanistically, CSE1L contributed to leukemogenesis by participating in ribosome biogenesis and nuclear export of the pre-ribosomal large subunit (pre-60S). Ribosome biogenesis pathways were the most abundantly expressed gene sets in young AML patients and young primary AML samples demonstrated enhanced rates of ribosome biogenesis and increased sensitivity to ribosome biogenesis inhibitors, compared to older AML samples. Our findings reinforce growing evidence that childhood/AYA AML is a fundamentally different disease to AML in older adults, and is driven by different biologic processes. Through inhibition of CSE1L, or the downstream process it regulates, ribosome biogenesis, we have identified a novel way to target leukemogenesis in childhood/AYA AML.
