Spatial transcriptomic profiling of vascular compartments of chronic antibody-mediated rejection in kidney allografts identified compartment-specific signatures: a pilot study


ABSTRACT: Chronic antibody-mediated rejection (CAMR) is a leading cause of late kidney allograft loss, and its vascular compartments are the principal site of injury. To resolve compartment-specific transcriptomic programs, we applied GeoMx Digital Spatial Profiling with the Human NGS Whole Transcriptome Atlas (18,677 targets) to formalin-fixed paraffin-embedded kidney allograft biopsies. Forty-eight regions of interest (ROIs) were profiled across three vascular compartments (glomerulus, peritubular capillary, and vessel) in patients with CAMR or no evidence of rejection (NER), stratified by donor-specific antibody (DSA) status, together with normal (non-transplant) kidney as a control. As a pilot study with one patient per group, this dataset is intended to be descriptive and hypothesis-generating rather than definitive.
