Integrated Multi-Omics Analysis of Dampness-Heat Gout Reveals Diagnostic Biomarkers and Therapeutic Targets

ABSTRACT: Damp-heat gout (DHG) is a clinically recognized subtype of gout characterized by a specific syndrome in traditional Chinese medicine (TCM). However, its pathomechanism remains unclear, and there is a lack of quantifiable indicators for accurate clinical diagnosis and treatment. This study aimed to elucidate the pathological mechanisms underlying DHG and to establish a symptom-centered model for its diagnosis and treatment. A total of 136 participants, comprising healthy controls and diagnosed DHG patients, were recruited for the analysis. Utilizing serum metabolomics and proteomics methodologies, we identified 21 common pathways associated with DHG, highlighting four crucial pathways: bile secretion, cholesterol metabolism, purine metabolism, and arachidonic acid metabolism, which were found to correlate significantly with the primary symptoms of DHG. Additionally, six key biomarkers—hypoxanthine, prostaglandin E2, uric acid, deoxycholic acid, taurochenodeoxycholic acid, and bilirubin—were identified. These biomarkers demonstrated robust diagnostic potential, with AUC values of 0.987 in the discovery cohort and above 0.99 in the validation cohort. Furthermore, six potential protein therapeutic targets were identified, including ATP1A1, APRT, ANGPTL4, GLUT1, PTGES3, and LIPA. In conclusion, our study establishes a symptom-centered diagnostic and therapeutic model for DHG based on identified biomarkers. It also clarifies key metabolic pathways involved, paving the way for improved diagnosis and targeted treatment strategies in clinical management.

Project description:Here we performed a transcriptomic study on complete symptom development process of CMV-infected Nicotiana tabacum using Solexa/Illumina's high-throughput digital gene expression (DGE) system. 12 DGE libraries (from six virus-infected samples and six corresponding mock-inoculated samples) were constructed, and the gene expression variations between the virus-infected sample and the mock-inoculated sample in each symptom stage were compared. Thousands of differentially expressed genes were obtained by the comparison, and KEGG pathway analysis of these genes suggested that many biological processes (such as phtosynthesis, pigment metabolism and plant-pathogen interaction) were related to the symptom development. The authenticity of the DGE data was further confirmed by analyzing real-time RT-PCR using several random-selected genes. We sequenced a cDNA library constructed from mixture of total RNA from six virus-infected samples and six mock-inoculated samples to get gene information for tobacco leaves in different symptom stages, including vein clearing, mosaic, severe chlorosis, partial recovery, total recovery and re-mosaic, and 95,916 Unigenes were obtained Tobacco leaves were inoculated by CMV-infected leaf homogenate and healthy leaf homogenate, respectivley. Six time points with different symptom stage were selected, and one virus-infect sample and one mock-inoculated sample were collected at each time. In order to average out variation of different plants, five leaves from five different plants were mixed to prepare every RNA sample. Twelve individual tag libraries of samples (six infected samples and six mock-inoculated samples) were constructed in parallel. For the gene expression analysis, the twelve samples were grouped into six groups, and each group contained a virus-infected sample and a mock-inoculated sample collected at the same time. In each group, the DGE data of virus-infected sample were compared to that of mock-inoculated sample to obtain the gene expression variations. Illumina sequencing of transcripts from virus-infected and mock-inoculated samples to get gene information for tobacco leaves in different symptom stages. In order to get more gene information, the systemically infected leaves and mock-inoculated leaves were harvested at six time points and five leaves from five different plants were collected at each time point. The RNA-Seq analysis provided gene information for mock-inoculated and virus-infected tobacco leaves in different symptom stages.

Project description:IHF is a major chronic disease that seriously threatens human health. Qi deficiency and blood stasis syndrome (QDBS), Yang deficiency with blood stasis syndrome (YDBS) and Yang deficiency and blood stasis with fluid retention syndrome (YDBSFR) are the basic syndromes of IHF in Chinese medicine. This study aims to explore the biological basis of the three IHF syndromes through integrated multi-omics research. We analyzed and integrated transcriptomic, proteomic, and targeted metabolomic data from IHF patients and healthy persons to obtain the key biomarkers and enriched pathways of QDBS, YDBS and YDBSFR(Registration No.: ChiCTR2200058314). These biomarkers were combined with clinical indicators to construct the "Disease-Syndromes-Clinical phenotypes-Biomarkers-Pathways" network, and the obtained differential genes and proteins were externally validated. The potential biomarkers for QDBS included SDHD, IL10, ACTG1, VWF, MDH2, COX5A, Valeric acid, Succinic Acid and L-Histidine, which were predominantly enriched in TCA cycle, oxidative phosphorylation, platelet activation, and neutrophil extracellular trap formation pathways, demonstrating associations with energy metabolism, coagulation system, and immune-inflammatory responses.YDBS potential biomarkers included TSHR, PRKG1, ATP1A2, GNAI2, APOA2, PLTP, 3-Hydroxybutyrate, Hexadecanoic acid and Palmitelaidic acid, and the combined pathways were mainly enriched in thyroid hormone synthesis, regulation of lipolysis in adipocytes, cholesterol metabolism and PPAR signaling pathways, correlating with hormonal regulation and lipid metabolism. The potential biomarkers of YDBSFR were CNGB1, KCNMA1, PIK3R2, HSPA8, C3, FH, Oxamic acid, N-Acetyl-L-alanine, 4-Hydroxyhippuric acid, and the combined pathways were mainly enriched in aldosterone-regulated sodium reabsorption, cGMP-PKG signaling pathway, neutrophil extracellular trap formation and TCA cycle signaling pathways, which are related to hormone regulation, signal transduction, immune-inflammatory response and energy metabolism. Platelet activation was involved in the whole process of IHF. External validation demonstrated the above core targets. This study investigated the biological basis of QDBS, YDBS and YDBSFR in IHF from a modern biomedical perspective, providing references for the objective research of TCM syndrome differentiation.
