Project description:iPSC-based modeling of preeclampsia identifies defects in extravillous trophoblast differentiation

Project description:Preeclampsia (PE) is a hypertensive disorder, which affects up to 10% of pregnancies worldwide. The primary etiology is considered to be abnormal development and function of placental cells called trophoblasts. We previously developed a two-step protocol for differentiation of human pluripotent stem cells, first into cytotrophoblast (CTB) progenitor-like cells, and then into both syncytiotrophoblast (STB)- and extravillous trophoblast (EVT)-like cells, and showed that this protocol can be used to model both normal and abnormal trophoblast differentiation. We have now applied this protocol to differentiate induced pluripotent stem cells (iPSC) derived from placentas of pregnancies with or without PE. While we did not note any differences in CTB induction, PE-iPSC-derived CTB showed a defect in syncytialization, and had a >180-fold reduction in expression of PSG4, a mature STB marker (p<0.02). While EVT formation did not appear to be altered, the invasive capacity of PE-iPSC-derived EVT was non-responsive to a decrease in oxygen tension, while that of non-PE-iPSC-derived EVT was enhanced 3.8-fold. RNA sequencing analysis showed gene expression changes consistent with defects in STB formation and response to hypoxia in PE-iPSC derived trophoblast. However, differences in DNA methylation were minimal, with only documented differences consistent with part of the response to hypoxia. Overall, PE-associated iPSC recapitulated multiple defects associated with placental dysfunction, with their lack of response to decreased oxygen tension emphasizing the importance of the interface with the maternal microenvironment in normal placentation.

Project description:Abnormal placentation is a fundamental cause of pregnancy complications including preeclampsia, miscarriage, and intrauterine growth restriction. Proliferation, differentiation, migration, and invasion are key properties of trophoblast cells that regulate trophoblast functions. Imbalance of these cellular processes is linked to placental pathologies. Using HTR-8/SVneo immortalized human trophoblast cells, we demonstrate that steroid receptor coactivator-2 (SRC-2), a transcriptional regulator of nuclear receptors, is essential for extravillous trophoblast cell proliferation, migration, and invasion. Genome-wide transcriptomics identified an SRC-2–dependent transcriptome in HTR-8/SVneo cells that includes a diverse spectrum of proteins involved in placental tissue development and function. To emphasize the utility of this transcriptomics data set, we demonstrate that WNT family member 9A (WNT9A) is not only a molecular target of SRC-2 but also crucial for maintaining many of the cellular properties of human extravillous trophoblast cells.

Project description:The human placenta (the extra-embryonic organ in which the fetus is made) is essential for conception and protection of the fetus during pregnancy. Extravillous trophoblasts (EVT) from placental villi infiltrate the decidua and convert maternal arteries into high-conductance vessels. Defects in trophoblast invasion and arterial conversion in early pregnancy are at the root of common pregnancy disorders such as preeclampsia. Here, we generated ecdysone and chorionic monocytic profiles across different gestational weeks spanning the 22-45 year age range, which allowed us to resolve the differentiation trajectory of trophoblast cells at implantation and its relationship with maternal age. We used this age atlas to identify age nodes of placental implantation change, revealing patterns of EVT implantation in advanced versus normal maternal age. Our data provide a comprehensive analysis of post-bedding trophoblast cell differentiation and may inform the design of experimental models of the placenta in early human pregnancy.

Project description:Preeclampsia (PE) is a hypertensive pregnancy disorder with increased risk of maternal and fetal morbidity and mortality. Abnormal extravillous trophoblast (EVT) development and function is considered to be the underlying cause of PE, but has not previously been modeled in vitro. We previously derived induced pluripotent stem cells (iPSC) from placentas of PE patients and characterized abnormalities in formation of syncytiotrophoblast and responses to changes in oxygen tension. In this study, we converted these primed iPSC to naïve iPSC, then derived trophoblast stem cells (TSC) and EVT to evaluate molecular mechanisms underlying PE. We found that primed (but not naïve) iPSC-derived PE-EVT have reduced surface HLA-G, blunted invasive capacity, and altered EVT-specific gene expression. These abnormalities correlated with promotor hypermethylation of genes associated with the epithelial-mesenchymal transition pathway, specifically in primed-iPSC derived PE-EVT. Our findings indicate that abnormal epigenetic regulation might play a role in the PE pathogenesis.

Project description:Preeclampsia, a life threatening hypertensive disorder of pregnancy, is a leading cause of maternal and perinatal morbidity and mortality. Its early onset form (EO PE), requiring delivery before 34 weeks of gestation, is particularly severe and closely linked to defective trophoblast differentiation. Here, we identify BRCA1-associated protein 1 (BAP1) and its cofactors ASXL2 and ASXL3 are upregulated in EO PE placentas. Enforced BAP1 expression in human trophoblast stem cells reinforced epithelial identity, enhanced adhesion, and impaired both extravillous trophoblast differentiation and syncytiotrophoblast formation. Integrated transcriptomic and proteomic analyses revealed suppression of lineage-specific pathways alongside maintenance of progenitor-like and pro-inflammatory signatures. In trophoblast organoids, excess of BAP1 disrupted syncytial maturation and induced interferon-driven pathways overlapping with EO PE transcriptomes. Together, these findings establish BAP1 as a key regulator of human trophoblast differentiation and implicate its dysregulation in the pathogenesis of EO PE, providing mechanistic insight into the cellular basis of placental dysfunction.

Project description:Impaired trophoblast differentiation and invasion are a hallmark of severe pregnancy complications such as preeclampsia and fetal growth restriction. Women with immune-mediated inflammatory diseases, in particular systemic lupus erythematosus (SLE), have an increased risk for adverse pregnancy outcomes. These diseases are characterized by high activity of pro-inflammatory cytokines, such as interferon alpha (IFN-α) and tumor necrosis factor alpha (TNF-α). Here, we investigated the direct effects of these cytokines on extravillous trophoblast (EVT) differentiation and invasion, using trophoblast stem cells (TSCs), trophoblast organoids, and first-trimester placental and decidual tissue. While neither cytokine impaired EVT differentiation, both significantly reduced EVT invasion into extracellular matrix and/or maternal decidua. Transcriptomic analysis revealed cytokine-induced alterations in gene expression associated with epithelial-mesenchymal transition (EMT), a key process in EVT invasion. These findings suggest that elevated IFN-α and TNF-α levels may directly impair placental development by inhibiting EVT invasion, providing a mechanistic link between inflammatory conditions such SLE and adverse pregnancy outcomes.
