Presurgical ablative radiation downstages high-risk features and alters immune response in pancreatic cancer

Project description:Neoadjuvant (NA) stereotactic ablative radiotherapy (SAbR) may improve outcomes in pancreatic cancer. We compared outcomes and molecular characteristics of patients who had surgery after neoadjuvant chemotherapy, with and without SAbR. We compared clinical data from surgically resected patients treated with either NA chemotherapy or NA chemotherapy plus SAbR. Recurrence-free and overall survival (RFS/OS) were analyzed using stepwise-AIC-optimized multivariable Cox modeling and log-rank tests. When available, we used transcriptomic analyses (GSEA, CIBERSORTx, and SCISSOR) to link clinical outcomes to gene expression patterns. We reviewed 133 patients treated with NA chemotherapy and 48 with NA chemotherapy + SAbR, including RNA sequencing data from 29 and 14 patients, respectively. Before surgery, the SAbR group had more advanced disease with arterial involvement, yet still achieved similar OS and RFS. Patients treated with SAbR had better post-treatment pathology linked to improved OS and RFS. RNA sequencing showed that patients with nodal involvement had enriched MYC targets and worse distant RFS. Those with arterial involvement had worse locoregional RFS with chemotherapy alone, but not in the SAbR group. GSEA highlighted immune-related changes in myeloid and T cell activation, suggesting treatment-related functional changes. SCISSOR analysis revealed cytotoxic CD8 and NK/NKT cell signatures correlating with better local control, while Treg signatures were linked to worse LRFS. Patients treated with NA chemotherapy + SAbR showed better pathologic outcomes, improved local control, and comparable survival, along with a distinct immune response. Further evaluation of SAbR in the neoadjuvant setting, combined with enhanced chemotherapy and novel treatment combinations, may lead to even greater clinical benefits.

Gene expression of inflammatory monocytes/macrophages after ablative radiotherapy in pancreatic adenocarcinoma

Project description:Ablative RT results in increased expression of CCL2 within the tumor microenvironment of pancreatic ductal adenocarcinoma (PDAC) and also increased recruitment of CD45+CD11b+Ly6Chi inflammatory monocytes/macrophages. This increase in CCL2 expression and recruitment of inflammatory monocytes/macrophages is a mechanism of resistance to the anti-tumor effects of ablative radiotherapy (RT). We used microarrays to study changes in gene expression patterns of inflammatory monocytes/macrophages sorted from the tumor microenvironment after ablative RT in a subcutenous model of pancreatic adenocarcinoma. From this, we identified 8 genes with an absolute fold change of expression equal to or greater than 2 with a false discovery rate equal to or less than 25 %. A pancreatic cancer tumor cell line derived from spontaneously arising tumors in KrasLSL-G12D/+, Trp53LSL-R172H/+, Pdx1-Cre (KPC) mice was subcutaneously implanted into 8 week old female C57BL/6 and allowed to grow for 14 days. After 14 days, 4 mice received 20 Gy of radiation, and 4 mice received a sham treatment. One day post treatment, tumors were harvested, and inflammatory monocytes/macrophages were isolated using flow sorting based on a surface expression phenotype of CD45+ CD11b+ Ly6Chi. From this cell population, total RNA was extracted for creation of cDNA and hybridization on Affymetrix microarrays. From the microarrays, a set of genes associated with radiation treatment of PDAC was identified.

Tumorgenic properties of high-dose ionizing radiation of primary cancer-associated fibroblasts

Project description:In the context of radiotherapy, the effects elicited by ionizing radiation on the supportive stroma of tumors remains understudied. Tumor infiltrating fibroblasts are abundantly found in the stromal tissue of most tumor types and are key determinants of tumor progression and metastasis. Given the clinical attention that ablative radiotherapy is gaining nowadays, and the paramount role played by CAFs in cancer sustainability, in this study the effect of high-dose ionizing radiation on primary CAF?S have been investigated. We have used high-throughput transcriptome analysis to ascertain in which degree tumor stromal fibroblast become pro-tumorigenic or anti-tumorigenic after ablative irradiation. Hence, genome-wide transcriptome analysis has been carried out on CAFs isolated from NSCLC tumors after receiving a single dose of 18 Gray. The expressions of relevant genes have been validated by real-time PCR, and transformed cellular pathways have been checked by functional assays. Overall, our data reveals profound changes in biological pathways including cellular stress, DNA damage, cell cycle, aging, apoptosis, oxidative stress, and matrix remodeling. Ablative radiation doses may exert both pro-and anti-tumorigenic effects on CAFs. The ultimate effect of such disturbances merits further investigation in more complex in vivo settings.

Deciphering the molecular effects of non-ablative Er:YAG laser treatment in an in vitro model of the non-keratinized mucous membrane

Project description:This study aimed to investigate the molecular effects of non-ablative Er:YAG laser treatment using an in vitro model of the non-keratinized mucous membrane and to compare its molecular effects with other ablative and non-ablative laser systems. In dermatology, the use of non-ablative and ablative fractional lasers has become the gold standard treatment for a number of indications. Each of the individual laser types is advantageous for different types of indications due to its respective properties, but new technologies open up new fields of application for individual laser systems. Performing a comprehensive gene expression profiling we compared the gene regulatory effects of non-ablative Er:YAG laser with other non-ablative and ablative laser systems. In vitro 3D models have proven to be a reliable and reproducible tool to study the molecular biological effects of different laser settings.

Chronic Alcohol Consumption Alters micro-RNA Radiation Responses in the Mouse Liver

Project description:Accumulating evidence suggests that lifestyle-related factors may influence radiation responses and the resulting cancer risks through epigenetic mechanisms, such as miRNA regulations. Chronic alcohol consumption is a major risk factor for various pathologies, including alcoholic liver disease. We have recently shown that consumption of Japanese sake promotes glutathione metabolism and anti-oxidative activities in the liver of irradiated C57BL/6 mice. Here we show that chronic alcohol consumption resulted in elevated ciculating levels of the inflammatory cytokine TNF-Î± and that it triggered specific miRNA regulations (such as the upregulation of the radio-resistant miR-210) that are susceptible to influence the resulting radiation effects in the mouse liver. Japanese sake was administrated to C3H mice irradiated with 3 Gy X-rays. miRNA expression was measured in the livers of 3 mice for each experimental group.
