Project description:Whole blood and purified leucocyte populations were compared to determine which cell types the whole blood interferon-inducible signature, found in both tuberculosis and sarcoidosis, was present in. This study agreed with previous publication that the neutrophil was the dominant cell in tuberculosis and also showed it was the dominant cell type in sarcoidosis. There were 5 patients in each group of healthy controls, tuberculosis and sarcoidosis. The cell types were whole blood, PBMCs, monocytes (CD14), T cells (CD8 and CD4) and neutrophils (CD15).

Project description:Sarcoidosis is a granulomatous systemic inflammatory disease predominantly affecting lungs. It shares histopathological, clinical and immune features with tuberculosis (TB). There are currently no diagnostic tests to formally identify sarcoidosis; clinicians first need to rule out the presence of other diseases, including TB. We characterized Mycobacterium tuberculosis (Mtb)-specific immune responses in blood of pulmonary sarcoidosis patients in comparison to active tuberculosis (ATB), Mtb-sensitized and non-sensitized healthy controls using flow cytometry and transcriptomics. We found that sarcoidosis was associated with i) a marked reduction in the frequency of antigen-reactive T cells in response to both Mtb peptides and Mtb lysate, ii) increased frequency of monocytes ex vivo and after in vitro stimulation, and iii) increased expression of phagocytic genes compared to Mtb-infected and healthy cohorts. Combination of Mtb-specific T cell and monocyte gene or flow cytometry signatures in Mtb-stimulated PBMC could efficiently distinguish sarcoidosis from ATB with high specificity and sensitivity, and to a lower extent from Mtb-sensitized and non-sensitized healthy controls. These biomarkers offer promising aspects for developing effective diagnostic tests for sarcoidosis, in particular for differential diagnosis from tuberculosis.

Project description:NK cells were purified from peripheral blood of healthy controls (n = 5), individuals with latent tuberculosis infection (n = 5), and active tuberculosis patients (n = 5). Total RNA was isolated from purified NK cells using RNA Isolation Kit (Dongsheng, R1061) according to the manufacturer's instructions. RNA Sequencing was conducted using the Illumina HiSeq X Ten platform (Novogene Bioinformatics Technology Co., Ltd.) with paired-end 150-bp reads.
Project description:Kveim-reagent (Kv) skin testing was a historical method of diagnosing sarcoidosis. Intradermal injection of treated sarcoidosis spleen tissue resulted in a granuloma response at injection site by 4-6 weeks. Previous work indicates proteins as the possible trigger of this reaction. We aimed to identify Kv-specific proteins and characterise the ex vivo response of Peripheral Blood Mononuclear Cells (PBMCs) from sarcoidosis, tuberculosis and healthy control patients when stimulated with both Kv and selected Kv-specific proteins.
