Project description:Lichen planopilaris (LPP) and pseudopelade of Brocq (PPB) are two scarring alopecia diagnoses that often exhibit similar clinical features. Some suggest LPP and PPB are not distinct diseases, but rather different clinical presentations in a spectrum derived from the same underlying pathogenetic mechanism. We explored the degree of similarity between LPP and PPB gene expression patterns and the potential for common and unique gene pathway and gene activity in LPP and PPB using microarrays. Microarray analysis, using a 21K cDNA set was performed on pairs of biopsies obtained from affected and unaffected scalp of untreated patients (4 LPP and 4 PPB biopsy pairs). Diagnosis was confirmed by histopathology. Significantly differentially expressed genes were identified by analysis of microarray results in various data sets and screened for signaling pathway involvement. Selected genes were validated by quantitative PCR and immunohistology. The global gene expression profiles in LPP and PPB versus comparative intra-control scalp tissue were distinguishable by Significance Analysis of Microarrays (SAM). Specific genes, such as MMP11, were identified with significantly differential expression in association with LPP versus PPB. There was very limited commonality in the gene expression profiles between LPP and PPB. Our findings may have important implications for understanding the pathogenesis of LPP and PPB at the molecular level. Results suggest LPP and PPB involve different mechanisms of disease development and can be regarded as biologically distinct cicatricial alopecia diagnoses. Genes that we have identified may potentially be useful as markers of the respective diagnoses.

Whilst destruction of epithelial hair follicle stem cells (eHFSCs) that reside in an immunologically protected niche of the HF epithelium, the bulge, is a likely key event in LPP pathogenesis, this remains to be demonstrated. Laser capture microdissection of bulge cells from biopsies of lesional and non-lesional scalp skin from adult LPP patients were analyzed by microarray analysis.

Project description:Background: Central Centrifugal Cicatricial Alopecia (CCCA) is a primary cicatricial alopecia that most commonly affects women of African descent. Like CCCA, fibroproliferative disorders (FPDs) such as keloids, atherosclerosis and fibroids, are characterized by low-grade inflammation and irritation resulting in end stage fibrosis. Objective: We sought to determine if fibroproliferative genes were upregulated in patients with CCCA Methods: Five patients with biopsy proven CCCA were recruited for this study. Two scalp biopsies were obtained from each patient; one from CCCA affected vertex scalp and one from the unaffected occipital scalp. Microarray analysis was performed to determine the differential gene expression patterns Results: Several pathways responsible for excess collagen deposition and aberrant wound healing were up-regulated in CCCA affected scalp when compared to unaffected scalp with patterns similar to other FPDs. Genes downregulated in CCCA affected tissue included those with roles in lipid metabolism and fatty acid biosynthesis. Limitations: Small sample size and the use of whole skin tissue for analysis. Conclusion: We have identified the up-regulation of critical genes implicated in fibroproliferative disorders in the gene expression profile of patients with CCCA. These findings may help identify future therapeutic targets for this otherwise difficult to treat condition.

Project description:To investigate the molecular signatures of the different sub-types of Primary cicatricial alopecia (PCA). We performed high-throughput RNA-sequencing on scalp biopsies of PCA patient. We developed gene expression-based signatures, trained into machine-learning based predictive models and pathways associated with dysregulated gene expression. We performed morphological and cytokine analysis to define the immune cell populations found in PCA sub types. Using bulk RNA sequencing, we identified a common PCA gene signature that was shared between LPP, FFA, and CCCA which revealed a significant over-representation of mast cell signature genes, as well as down regulation of cholesterogenic pathways and up regulation of fibrosis and immune cell signaling genes. Thw Immunohistological analyses revealed an increased presence of mast cells in PCAs lesions.

Project description:Frontal fibrosing alopecia (FFA), a clinical variant of follicular lichen planus, is a predominantly postmenopausal, immune-mediated, inflammatory, primary lymphocytic cicatricial alopecia. FFA mainly involves the scalp, facial hair and eyebrows but also affects other body regions. MicroRNAs (miRNAs) have emerged as potential candidates of pathobiologic, diagnostic and therapeutic interest in chronic inflammatory, fibrotic and autoimmune diseases. To explore miRNAs in FFA for their disease relevance we isolated plasma from venous blood from 10 biopsy-proven treatment-naïve FFA cases and 10 matched controls and undertook miRNA expression analysis using the Human Fibrosis miRNA PCR Array (Qiagen). A separate cohort of 7 active FFA cases and 7 matched healthy controls were ascertained for tissue microRNA analysis and all 14 scalp-biopsy-extracted microRNA samples were subjected to microarray analysis on Affymetrix GeneChip miRNA 4.0 arrays. We generated a list of communities for the two skin tissue networks (cases and controls) and identified cluster centres (exemplars) as representative miRNAs of each community. Twenty exemplars in the control network were significantly enriched in the plasma (control) dataset compared to 27 for FFA. Amongst these, there were 17 miRNAs common in both networks, 9 of which were representative in the FFA disease phenotype. Random Forest analysis suggested that 4 circulating miRNAs (hsa-let-7d-5p, hsa-miR-18a-5p, has-miR-20a-5p and hsa-miR-19a-3p) can discriminate between FFA and controls. Our study of skin and plasma miRNA co-expression highlights circulating miRNAs of potential predictive value as biomarkers that should now be validated in larger cohorts.

Project description:Central centrifugal cicatricial alopecia (CCCA) has recently been associated with increased expression of genes implicated in fibroproliferative disorders (FPDs) in the affected scalp and higher prevalence of uterine leiomyomas (UL). We therefore sought to examine the effect of UL status on the gene expression profile of the lesional scalp in CCCA patients. Scalp biopsy was obtained from 16 patients with a confirmed diagnosis of CCCA between 2017 and 2020. Microarray analysis was used to identify differential gene expression between CCCA patients with and without a history of UL. Of the 20,000 genes analyzed, 23 of 25 genes with highest expression in CCCA patients with UL held no statistical significance. No genes previously implicated in FPDs were found among the upregulated transcripts. Of all genes analyzed, only 8 upregulated genes and zero downregulated genes had a fold change >2 in CCCA patients with UL compared to CCCA patients without UL. Our findings highlight similar gene expression patterns in the scalp of CCCA patients with and without a history of UL.
