The Correlation between Metal Mixed Exposure and Lung Function in Different Ages of the Population.

ABSTRACT: Herein, we explored the overall association between metal mixtures and lung functions in populations of varying ages and the relationship among the associated components. The 2007-2012 National Health and Nutrition Examination Survey data of 4382 American participants was analyzed, and generalized linear, elastic net, quantile g-computation, and Bayesian kernel machine regression models were used to evaluate the relationship between exposure to the metal mixture and lung function at various ages. The results of barium exposure at distinct stages revealed that children and adolescents exhibited greater lung function changes than those in adults and the elderly. Additionally, compared with children and adolescents, cadmium- and arsenic-containing metabolites contributed to nonconductive lung function changes in adults and the elderly exposed to metal mixtures. The results showed that the effects of exposure to metal mixtures on lung function in children and adolescents were predominantly caused by lead and barium. Altogether, children and adolescents were found to be more susceptible to metal-exposure-mediated lung function changes than adults and the elderly.

Project description:BACKGROUND:Oxidative stress related genes modify the effects of ambient air pollution or tobacco smoking on lung function decline. The impact of interactions might be substantial, but previous studies mostly focused on main effects of single genes. OBJECTIVES:We studied the interaction of both exposures with a broad set of oxidative-stress related candidate genes and pathways on lung function decline and contrasted interactions between exposures. METHODS:For 12679 single nucleotide polymorphisms (SNPs), change in forced expiratory volume in one second (FEV(1)), FEV(1) over forced vital capacity (FEV(1)/FVC), and mean forced expiratory flow between 25 and 75% of the FVC (FEF(25-75)) was regressed on interval exposure to particulate matter <10 µm in diameter (PM10) or packyears smoked (a), additive SNP effects (b), and interaction terms between (a) and (b) in 669 adults with GWAS data. Interaction p-values for 152 genes and 14 pathways were calculated by the adaptive rank truncation product (ARTP) method, and compared between exposures. Interaction effect sizes were contrasted for the strongest SNPs of nominally significant genes (p(interaction)<0.05). Replication was attempted for SNPs with MAF>10% in 3320 SAPALDIA participants without GWAS. RESULTS:On the SNP-level, rs2035268 in gene SNCA accelerated FEV(1)/FVC decline by 3.8% (p(interaction)?=?2.5×10(-6)), and rs12190800 in PARK2 attenuated FEV1 decline by 95.1 ml p(interaction)?=?9.7×10(-8)) over 11 years, while interacting with PM10. Genes and pathways nominally interacting with PM10 and packyears exposure differed substantially. Gene CRISP2 presented a significant interaction with PM10 (p(interaction)?=?3.0×10(-4)) on FEV(1)/FVC decline. Pathway interactions were weak. Replications for the strongest SNPs in PARK2 and CRISP2 were not successful. CONCLUSIONS:Consistent with a stratified response to increasing oxidative stress, different genes and pathways potentially mediate PM10 and tobacco smoke effects on lung function decline. Ignoring environmental exposures would miss these patterns, but achieving sufficient sample size and comparability across study samples is challenging.

Project description:This study investigated the relationship between lead (Pb) speciation determined using Extended X-ray Absorption Fine Structure (EXAFS) spectroscopy in <10 μm particulate matter (PM10) from mining/smelting impacted Australian soils (PP, BHK5, BHK6, BHK10 and BHK11) and inhalation exposure using two simulated lung fluids [Hatch's solution, pH 7.4 and artificial lysosomal fluid (ALF), pH 4.5]. Additionally, elemental composition of Pb rich regions in PP PM10 and the post-bioaccessibility assay residuals were assessed using a combination of Scanning Electron Microscopy (SEM) and Energy Dispersive X-ray Spectroscopy (EDX) to provide insights into how extraction using simulated lung fluids may influence Pb speciation in vitro. Correlation between Pb speciation (weighted %) and bioaccessibility (%) was assessed using Pearson r (α = 0.1 and 0.05). Lead concentration in PM10 samples ranged from 782 mg/kg (BHK6) to 7796 mg/kg (PP). Results of EXAFS analysis revealed that PP PM10 was dominated by Pb adsorbed onto clay/oxide, while the four BHK PM10 samples showed variability in the weighted % of Pb adsorbed onto clay/oxide and organic matter bound Pb, Pb phosphate, anglesite and galena. When bioaccessibility was assessed using different in vitro inhalation assays, results varied between samples and between assays, Pb bioaccessibility in Hatch's solution ranged from 24.4 to 48.4%, while in ALF, values were significantly higher (72.9-96.3%; p < 0.05). When using Hatch's solution, bioaccessibility outcomes positively correlated to anglesite (r:0.6246, p:0.0361) and negatively correlated to Pb phosphate (r: -0.9610, p:0.0041), organic bound Pb (r: -0.7079, p: 0.0578), Pb phosphate + galena + plumbojarosite (r: -0.9350, p: 0.0099). No correlation was observed between Pb bioaccessibility (%) using Hatch's solution and weighted % of Pb adsorbed onto clay/oxide and between bioaccessibility (%) using ALF and any Pb species. SEM and EDX analysis revealed that a layer of O-Pb-Ca-P-Si-Al-Fe formed during the in vitro extraction using Hatch's solution.

Project description:BackgroundAssociations of low lung function with features of poor cardio-metabolic health have been reported. It is, however, unclear whether these co-morbidities reflect causal associations, shared genetic heritability or are confounded by environmental factors.MethodsWe performed three analyses: (1) cardio-metabolic health to lung function association tests in Northern Finland Birth cohort 1966, (2) cross-trait linkage disequilibrium score regression (LDSC) to compare genetic backgrounds and (3) Mendelian randomisation (MR) analysis to assess the causal effect of cardio-metabolic traits and disease on lung function, and vice versa (bidirectional MR). Genetic associations were obtained from the UK Biobank data or published large-scale genome-wide association studies (N > 82,000).ResultsWe observed a negative genetic correlation between lung function and cardio-metabolic traits and diseases. In Mendelian Randomisation analysis (MR), we found associations between type 2 diabetes (T2D) instruments and forced vital capacity (FVC) as well as FEV1/FVC. Body mass index (BMI) instruments were associated to all lung function traits and C-reactive protein (CRP) instruments to FVC. These genetic associations provide evidence for a causal effect of cardio-metabolic traits on lung function. Multivariable MR suggested independence of these causal effects from other tested cardio-metabolic traits and diseases. Analysis of lung function specific SNPs revealed a potential causal effect of FEV1/FVC on blood pressure.ConclusionsThe present study overcomes many limitations of observational studies by using Mendelian Randomisation. We provide evidence for an independent causal effect of T2D, CRP and BMI on lung function with some of the T2D effect on lung function being attributed to inflammatory mechanisms. Furthermore, this analysis suggests a potential causal effect of FEV1/FVC on blood pressure. Our detailed analysis of the interplay between cardio-metabolic traits and impaired lung function provides the opportunity to improve the quality of existing intervention strategies.

Project description:BackgroundChronic respiratory diseases pose a significant threat to global health, underscoring the urgent need for effective preventative and therapeutic interventions. The neutrophil-to-albumin ratio (NPAR), an emerging biomarker for inflammation and nutritional status, has shown promising associations with respiratory health, necessitating an investigation into its potential for predicting lung function decline.ObjectiveThis study aimed to delineate the relationship between the NPAR and pulmonary function within a sample of the American adult population and assess the viability of the NPAR as a prognostic indicator for compromised lung function.MethodsWith data available from the National Health and Nutrition Examination Survey (NHANES) for the years 2007 to 2012, 10,055 American adults who met the exclusion criteria were included in the current study. Multivariate linear regression, smoothed curve fitting, and subgroup analyses were applied to evaluate the associations observed between the NPAR and lung function indicators.ResultsEven after accounting for all potential confounding factors, a significant inverse relationship persisted between the NPAR and key lung function indicators, including forced expiratory volume in one second (FEV1), forced vital capacity (FVC), and peak expiratory flow rate (PEF). This association remained robust even after potential confounding factors were considered. Subgroup analysis revealed that the negative correlation was more pronounced in certain demographic groups, such as young individuals, males, and current smokers. The study also revealed an "N-shaped" relationship between the NPAR and fractional exhaled nitric oxide (FENO), suggesting that the NPAR may play a role in promoting airway inflammation.ConclusionsA significant correlation between the NPAR and the decline in lung function among American adults was revealed in this research, emphasizing the potential clinical relevance of the NPAR as a respiratory health biomarker, as well as the importance of considering systemic inflammation in the management and prevention of respiratory disorders.
