Identifying Promising Themes for Adolescent Vaping Warnings: A National Experiment.

ABSTRACT:

Introduction

Adolescent vaping remains a problem in the United States, yet little is known about what health warning themes most discourage vaping among adolescents. We sought to identify the most compelling themes for vaping warnings for US adolescents.

Methods

Participants were a national probability sample of 623 US adolescents aged 13-17 years, recruited in the summer of 2020. Adolescents were randomized to one of the five warning message themes about the potential health effects of vaping: 1. chemical harms, 2. lung harms, 3. Coronavirus Disease 2019 (COVID-19) harms, 4. nicotine addiction, or 5. control (messages about vape litter). The primary outcome was perceived message effectiveness (PME; 3-item scale). Secondary outcomes were negative affect (fear), attention, anticipated social interactions, and message novelty.

Results

Adolescents rated the chemical, lung, and COVID-19 harms warning messages higher on PME than nicotine addiction and control (all p < .05), while nicotine addiction was rated higher than control (p < .05). The chemical, lung, and COVID-19 harms warning themes also elicited greater negative affect than nicotine addiction and control (all p < .05). For all other secondary outcomes, the COVID-19 harms warning message theme was rated higher than nicotine addiction and control (all p < .05).

Conclusion

Adolescents perceived warning message themes about lung, chemical and COVID-19 health effects of vaping as more effective than nicotine addiction. To discourage vaping, the FDA and others should communicate to youth about the health effects of vaping beyond nicotine addiction.

Implications

Adolescents rated warning message themes about the lung, chemical, and COVID-19 health effects of vaping as more effective than nicotine addiction, while nicotine addiction was rated as more effective than control themes about vaping litter. To discourage vaping among adolescents, health messaging should expand message themes to communicate about a broader set of health effects of vaping beyond nicotine addiction.

Project description:ObjectiveLittle evidence exists on which cigar warning statement attributes may impact cigar warning effectiveness; research is needed to identify the most effective cigar warning topics and text. This study was designed to inform the development of improved cigar warnings.DesignWe conducted a choice-based conjoint experiment. The experiment systematically manipulated cigar warning statement attributes, including cancer health effect (mention of colon cancer and/or oral cancer), non-cancer health effect (mention of heart disease and/or blood clots), causal language, warning marker word, verb use and tobacco type. Participants evaluated eight choice sets, each containing three cigar warnings with contrasting attributes, and chose the warnings that most and least encouraged them to quit smoking cigars. Using a Bayesian mixed logit model, we estimated the relative importance of each attribute and the attribute part-worth utility.SettingAn online survey of adults in the USA.ParticipantsWe enrolled 959 US adults who used little cigars, cigarillos, or large cigars in the past 30 days using an online survey from October to December 2020.Primary outcome measuresThe primary outcomes for the experiment are relative attribute importance and attribute part-worth utility.ResultsThe most important attributes to participant selection of warnings were the non-cancer and the cancer health effects (29.3%; 95%CI 28.6% to 30.0% and 29.0%; 95% CI 28.4% to 29.6%, respectively), followed by causal language (16.3%; 95% CI 15.7% to 16.8%), marker word (10.3%; 95% CI 9.9% to 10.7%), verb use (8.8%; 95% CI 8.5% to 9.2%) and tobacco type (6.3%, 95% CI 5.9% to 6.6%).ConclusionsOur findings indicate that health effects are the most important attributes when designing cigar warning statements, but other attributes, like causal terminology, also influence perceived warning effectiveness. Based on our findings, 'DANGER: Tobacco causes heart disease and blood clots' is an example of a highly effective warning statement for cigars.

Project description:IntroductionLittle systematic evidence exists about the effectiveness of cigar warnings. This study examined the perceived message effectiveness (PME) of warning statements about different health consequences caused by cigars. PME is a validated self-report scale of how effectively a health message discourages smoking.Aims and methodsWe conducted an online study from April to May 2020 with adults in the United States who used cigars in the past 30 days (n = 777). Participants were randomly assigned to view and rate PME (three items, range 1-5) for seven out of 37 text warning statements about different health consequences from cigar use. Linear mixed effects models evaluated the most effective warning characteristics (eg, type of health consequence), controlling for repeated measures and participant demographics.ResultsAnalyses showed that health consequences about the cardiovascular system (B = 0.38), mouth (B = 0.40), other digestive (B = 0.45), respiratory system (B = 0.36), and early death (B = 0.36) were associated with higher PME scores than reproductive health consequences (all p values <.001). Similar results were found for these health consequences compared with addiction (all p values p < .001). We also observed that awareness of the health consequence was associated with higher PME scores (B = 0.19, p < .001) and length of the warning message (number of characters) was associated with lower PME scores (B = -0.007, p = .03). No differences were observed between cancer and noncancer health consequences (p = .27) or health consequences that used plain language versus medical jargon (p = .94).ConclusionsOur study provides new evidence about the perceived effectiveness of different cigar health warning statements and identifies features that may strengthen statements.ImplicationsOur study with cigar smokers from across the United States provides much-needed evidence concerning the perceived effectiveness of different cigar health warning statements and features that may strengthen such statements. Mandated cigar warnings in the United States could be strengthened by including health consequences that were perceived as more effective in our study (eg, early death), using health consequences that participants were aware of, and using short warning statements.

Project description:IntroductionMinimizing electronic cigarette (e-cigarette) uptake and encouraging vaping cessation have become important components of public health agendas. Given the success of well-designed anti-smoking campaigns, attention has turned to developing health communications that target vaping. Although prior work has identified a range of potentially effective campaign messages, several gaps in the literature remain. We aimed to (1) identify message themes perceived to be effective at minimizing e-cigarette use and (2) elicit opinions on how these message themes could be optimized in terms of message execution and delivery.Aims and methodsFocus groups (FG; n = 16) were conducted with adolescents (14- to 15- and 16- to 17-year-olds), young adults (18- to 24-year-olds), and adults (25- to 39-year-olds). The groups reported on message themes they believed would assist with minimizing e-cigarette use. They then provided feedback on 14 message themes developed by the research team. Data were analyzed using reflexive thematic analysis.ResultsGroups discussed a range of message themes they believed would be effective. These included themes focused on the health harms associated with vaping, the chemical ingredients in e-cigarettes, use dependence, and social attitudes to use. Groups discussed the importance of using evidence-based personal testimonies and reported that messages would be most effectively communicated via social media, in public places, and at point of sale.ConclusionsResults build on existing research by identifying message themes perceived to be most effective, and how these could best be executed. Results have the potential to inform the development of co-designed health communication campaigns addressing youth vaping.ImplicationsUsing a stratified FG methodology, we collected rich and detailed qualitative data on Australians' perceptions of anti-vaping message themes. The themes considered most effective were those that addressed the health harms of vaping, the chemical ingredients in e-cigarettes, the social consequences of use, and the impacts of dependence. We recommend further work to develop and trial a range of specific anti-vaping messages based on the themes identified as having the most potential. This process should include iterative co-design with the target audience to optimize future health communication campaigns.
