Project description:Diagnostic tests for tuberculosis (TB) infection poorly predict future incident disease risk. We investigated a cohort of asymptomatic HIV-uninfected household contacts of TB in South Africa in which baseline lung lesions indicative of Subclinical TB identified by [18F]-fluoro-2-deoxy-D-glucose (FDG)-positron emission/computed tomography (PET/CT) had high predictive value for future TB over 5 years. RNA-sequencing analysis of whole blood samples at baseline PET/CT, 5-15m PET/CT2, and the TB diagnosis of incident TB revealed persistent enrichment of neutrophil and monocyte transcriptional modules in those with Subclinical TB and worsening and discrete lung abnormalities, while expression of a core set of type I IFN genes related to FDG-avid lymph nodes. Leveraging multiple machine learning algorithms, we derived gene signatures of various PET/CT lesion Subclinical phenotypesTB, future lesion worsening and TB diagnosis. prevalent and incident TB. Validating signatures in two independent cohorts against TB progression within 6,12,18 and fup to 24-29-months, confirmed gave AUC of 0.75-0.95 and 0.77-0.87 with performance above optimal sensitivity and wminimum specificity for the WHO target product profile for TB progression.

Project description:Interventions: 89Zr-cetuximab PET scan: After a therapeutic dose of cetuximab, 10mg of 89Zr-cetuximab is injected. Six days after injection a PET-scan will be made. If this scan is considerd positive, meaning uptake in extra-hepatic metastases is present, the normal dose of cetuximab will be continued.
If the first PET-scan is negative, we will increase the dose of cetuximab (3 by 3 cohort design) and repeat the PET-scan. The patient will continue the higher dose biweekly.
Furthermore we will preform a 18F-FDG PET scan and 15O-H2O PET scan, on baseline and after 2 weeks of treatment, to evaluate early changes in respectively metabolic changes and changes in tumor perfusion.
At baseline and after 3-4 week of treatment a skin biopsy and tumor biopsy will be done. We will preform oncogenic mutation analysis, we will determine EGFR expression and saturation.
Primary outcome(s): 89Zr-cetuximab uptake in non-hepatic metastases (corrected VOIs) and the correlation with clinical benefit rate.

Project description:Interventions: A standard whole body PET/CT scan will be performed first. This involves a patient lying on a scanning table for around 20 mins while breathing normally. The patient will recieve CT and a PET scan from the base of the brain to thighs.
Then at the conclusion of the whole body PET/CT scan the patient will be administered with IV contrast and asked to hold their breath while a high quality CT scan of the liver is acquired taking about 30 seconds. Once the contrast CT scan has concluded the patient will be instructed to breath normally again and a 10 minute gated PET scan of the liver will be acquired.
Primary outcome(s): To determine the number of positive liver metastases on contrast enhanced respiratory gated 4D PET/CT scanning compared to conventional ungated whole body PET/CT without contrast-enhanced CT.[At the time of PET/CT scan]

* The evaluation of the efficiency of 18F deoxyglucose-Positron Emission Tomography (FDG-PET) in staging patients eligible for hepatic resection of colorectal liver metastases in a randomized clinical multicentre setting.
Research questions of the proposed study:
* What are the effects and costs for patients with liver metastases of colorectal cancer indicated for potentially curative hepatic resection, using the conventional diagnostic strategy with computed tomography (CT) scan in comparison to the experimental diagnostic strategy incorporating FDG-PET scan (CT + FDG-PET scan), based on a health care perspective and a time horizon of 9 months.
More specifically:
* Does the experimental diagnostic strategy which includes FDG-PET scan in the diagnostic work-up of patients eligible for potentially curative hepatic resection of colorectal liver metastases lead to a better disease-free survival at 9 months after hepatic resection in comparison to the conventional diagnostic strategy using CT scan without FDG-PET scan.
* What are the costs of diagnostic and therapeutic care for the two diagnostic strategies for patients eligible for potentially curative hepatic resection of colorectal liver metastases.
* What is the effect of including the FDG-PET scan in the diagnostic work-up of patients eligible for potentially curative hepatic resection of colorectal liver metastases after hepatic resection, expressed as disease-free survival at 9 months adjusted for quality of health (Q-TWIST), in comparison to the use of CT scan only.

Project description:Measuring tumor response to treatment based on computed tomography (CT) and/or magnetic resonance imaging (MRI) has been a widely debated issue (response criteria in solid tumors [RECIST] and World Health Organization criteria). Furthermore, early identification of nonresponding patients is of great importance because the rates of response of common malignant solid tumors to chemotherapy are in the range of only 20-30%. Therefore, quantitative imaging of tumor metabolism with 18F-FDG PET/CT may provide important advantages and thus reduce side effects and costs of ineffective therapy. However, the evidence to date for the use of 18F-FDG-PET/CT with this indication is limited.
The purpose of the present trial is to determine the impact of 18F-FDG PET/CT in the management of advanced colorectal cancer. The aim is also to confirm whether a metabolic response can be used as a surrogate end point in monitoring treatment response in this cancer type.
The study consists of 40 patients with advanced colorectal cancer patients. All patients will be studied with 18F-FDG PET/CT combined with diagnostic contrast enhanced abdominal CT before the start of chemotherapy and re-evaluated 4-5 weeks after the initiation of therapy. Effect of this metabolic and anatomic change in therapy are evaluated and correlated to survival, morbidity, and treatment -related costs. Histopathologic confirmation of response is evaluated whenever possible. The data will be collected between 2008 and 2012.

Project description:Polyethylene terephthalate (PET) is one of the most commonly used plastics, utilized in synthetic fibers, water containers, and food packaging. From the 1990s onwards, the demand for PET, and therefore its production, increased exponentially. This increased usage of PET has resulted in a staggering accumulation of undegraded plastic waste. Nearly 80% of the 6300 million tons of plastic waste that had been generated as of 2015 were accumulated in landfills or the natural environment. Moreover, the production of PET relies heavily on non‐renewable fossil fuels, exacerbating environmental concerns over its widespread use. In alignment with principles of environmental sustainability, the biotechnological upcycling of PET has recently emerged as a compelling solution. Since the discovery of PETase, a hydrolase capable of depolymerizing this polyester, enzymatic plastic breakdown is increasingly considered as a promising solution for managing PET waste. This enzyme and its improved derivatives, such as Fast‐PETase, enable the breakdown of PET into bis(2‐41 hydroxyethyl) terephthalate (BHET) and mono(2‐hydroxyethyl) terephthalate (MHET). Subsequently, the enzyme MHETase is responsible for further degradation of MHET into ethylene glycol and terephthalic acid. The metabolic capability of microorganisms to utilize these monomers of PET for growth has been explored in various biotechnological applications, especially in the context of bioremediation and bioconversion processes aimed at transforming plastic waste into useful products using genetically engineered bacteria.
