PHASE IB STUDY OF AVELUMAB WITH RADIOTHERAPY IN PATIENTS WITH LEPTOMENINGEAL DISEASE

Project description:A phase IB, single-arm, non-randomized study to establish the safety of the PDL-1 inhibitor Avelumab with radiotherapy in patients with leptomeningeal disease (LMDz) from metastatic melanoma, lung cancer, and breast cancer. We hypothesize 1) systemic Avelumab will activate peripheral T cells and, with whole-brain radiotherapy (to induce blood-brain-barrier breakdown and stimulate antigen expression), significantly increase the number and activation status of T cells that migrate into the cerebrospinal fluid (CSF), with an Inflammatory CSF cytokine profile, in patients with LMDz; 2) this treatment regime is safe and tolerated without significant toxicities and will improve overall survival compared to historical controls; and 3) point estimates (with confidence intervals) can be derived using CSF measurements, toxicities, and clinical benefit to estimate sample sizes/schedules for an expanded Recommended Phase II Study.

Avelumab induces superior Fc-Fc receptor-dependent natural killer cell stimulation and dendritic cell cross-talk compared to durvalumab

Project description:Natural killer cells and dendritic cells are instrumental in controlling cancer. However, the impact of PD-L1 blockade therapy on these immune cell types is not well understood. Furthermore, across many clinical trials, different anti-PD-L1 antibodies have produced varying levels of efficacy in patients, and the basis of these differences are obscure. Avelumab, a native Fc IgG1 antibody, and durvalumab, a mutant-Fc IgG1 antibody, are two of the most commonly used anti-PD-L1 agents for cancer immunotherapy. We compared the mechanistic effects of these agents. Our study shows the robust impact of Fc-signaling on NK cells by avelumab. In contrast, durvalumab failed to induce NK cell activation or promote NK-DC cross-talk.

Low Baseline CXCL9 Predicts Early Progressive Disease in Unresectable HCC with Atezolizumab Plus Bevacizumab Treatment.

Project description:IntroductionAtezolizumab plus bevacizumab treatment is highly effective in patients with unresectable hepatocellular carcinoma (HCC). However, progressive disease (PD) occurs in approximately 20% of HCC patients treated with atezolizumab plus bevacizumab, resulting in a poor prognosis. Thus, the prediction and early detection of HCC is crucial.MethodsPatients with unresectable HCC treated with atezolizumab plus bevacizumab and had baseline preserved serum (n = 68) were screened and classified according to their PD, 6 weeks after treatment initiation (early PD; n = 13). Of these, 4 patients each with and without early PD were selected for cytokine array and genetic analyses. The identified factors were validated in the validated cohort (n = 60) and evaluated in patients treated with lenvatinib.ResultsNo significant differences were observed in the genetic alterations in circulating tumor DNA. Cytokine array data revealed that baseline MIG (CXCL9), ENA-78, and RANTES differed substantially between patients with and without early PD. Subsequent analysis in the validation cohort revealed that baseline CXCL9 was significantly lower in patients with early PD than that in patients without early PD, and the best cut-off value of serum CXCL9 to predict early PD was 333 pg/mL (sensitivity: 0.600, specificity: 0.923, AUC = 0.75). In patients with lower serum CXCL9 (<333 pg/mL), 35.3% (12/34) experienced early PD with atezolizumab plus bevacizumab, while progression-free survival (PFS) was significantly shorter relative to that in patients without (median PFS, 126 days vs. 227 days; HR: 2.41, 95% CI: 1.22-4.80, p = 0.0084). While patients with objective response to lenvatinib had significantly lower CXCL9 levels compared with those of patients without.ConclusionBaseline low serum CXCL9 (<333 pg/mL) levels may predict early PD in patients with unresectable HCC treated with atezolizumab plus bevacizumab.

Project description:We recruited 39 samples of metastatic HPV-associated carcinoma patients before and after immunotherapy (Combined Valproic acid + Avelumab). Based on the best overall response by iRECIST criteria, we stratified patients into Responders (iCCR, iCPR, iSD) and Non-responders (Clinically progressive, iCPD, iUPD). We analyzed the transcript differential expression in two groups (Rs vs. NRs) before (Baseline) and after therapy (EP) to understand the differences between the two groups in each time point and in time points (C1, C2, EP) compared to C0 for each group to check the changes related to therapy.
