Ketogenic diet versus standard high-carbohydrate nutrition in sepsis: a randomized controlled trial

Project description:Rationale: Sepsis patients suffer from severe metabolic and immunologic dysfunction that may be amplified by standard carbohydrate-based nutritional regimes. We therefore hypothesize that a ketogenic diet improves sepsis treatment. Objectives: We investigated the safety and feasibility of a ketogenic diet in sepsis patients. Methods: We conducted a monocentric open-labeled randomized controlled trial (DRKS00017710) enrolling adult sepsis patients randomly assigned to either ketogenic or standard high-carbohydrate diet for 14 days with follow-up until day 30. The primary outcome measure was β-hydroxybutyrate serum concentration on day 14. Secondary outcomes included safety, clinical and immunological changes. Measurements and Main Results: 40 critically ill septic patients were assigned to the study groups. Increase in β-hydroxybutyrate concentrations from baseline to day 14 was markedly greater under ketogenic diet (1.2 ±0.9) compared to controls (-0.3 ±0.4); estimated mean difference 1.4 (95%-CI 1.0-1.8; p<0.0001). During ketogenic diet, no patient required insulin treatment beyond day 4, whereas 35% to 60% of control patients did (p=0.0095). Metabolic side effects were not observed under ketogenic diet. Ventilation-free (IRR 1.7; 95%-CI: 1.5 to 2.1; p<0.0001), vasopressor-free (IRR 1.7; 95%-CI: 1.5 to 2.0; p<0.0001), dialysis-free (IRR 1.5; 95%-CI: 1.3 to 1.8; p<0.0001), and ICU-free days (IRR 1.7; 95%-CI: 1.4 to 2.1; p<0.0001) significantly increased in patients under ketogenic diet. There was no difference in 30-day mortality. Analyses indicated favorable changes towards immune homeostasis. Conclusions: Ketogenic diet is a feasible and safe nutritional regimen in septic patients promoting recovery from sepsis-related organ dysfunction and could become a new tool in modern treatment concepts.

Project description:In this study we conducted a randomized, double-blind, placebo-controlled trial to evaluate the effects of administering probiotic mixtures containing Lactobacillus casei Zhang, Bifidobacterium lactis V9, and Lactobacillus plantarum P9 in patients with GERD. Patients were randomized into probiotic and placebo groups . All patients were treated with rabeprazole tablets and simultaneously received either probiotic or placebo for 8 weeks, and then administrated with probiotic or placebo alone for 4 weeks. Fecal samples from patients were collected and analyzed by untargeted metabolomics at weeks 0, 4, 8, 12.

The effects of Lactobacillus plantarum on small intestinal barrier function and mucosal gene transcription; a randomized double-blind placebo controlled trial

Project description:The aim of this study was to investigate the effects of three Lactobacillus plantarum strains on in-vivo small intestinal barrier function and gene transcription in human subjects. The strains were selected for their differential effects on TLR signalling and tight junction protein rearrangement, which may lead to beneficial effects in a stressed human gut mucosa. Ten healthy volunteers participated in four different intervention periods: 7-day oral intake of either L. plantarum WCFS1, CIP48 (CIP104448), TIFN101 (CIP104450) or placebo, proceeded by a 4 weeks wash-out period. Lactulose-rhamnose ratio (an indicator of small intestinal permeability) increased after intake of indomethacin, which was given as an artificial stressor of the gut mucosal barrier (mean ratio 0.06±0.04 to 0.10±0.06, p=0.001), but was not significantly affected by the bacterial interventions. However, gene transcription pathway analysis in small intestinal biopsies, obtained by gastroduodenoscopy, demonstrated that particularly L. plantarum TIFN101 modulated cell-cell adhesion with high turnover of genes involved in tight- and adhesion junction protein synthesis and degradation (e.g. actinin alpha-4, metalloproteinase-2). These effects were less pronounced for L. plantarum WCFS1 and CIP104448. In conclusion, L. plantarum TIFN101 induced the most pronounced probiotic properties with specific effects on repair processes in the compromised intestine of healthy subjects.

The plasma proteome is favorably modified by a high protein diet but not by additional resistance training in older adults - a 17-week randomized controlled trial

Project description:Background: The age-related loss of muscle mass significantly contributes to the development of chronic diseases, loss of mobility and dependency on others, yet could be improved by an optimized lifestyle. Objective: The goal of this randomized controlled trial was to compare the influence of a habitual diet (CON) with either a diet containing the recommended protein intake (RP) or a high protein intake (HP), both with and without strength training, on the plasma proteome in older adults. Methods: 136 women and men (65-85 years) were randomly assigned to three intervention groups. CON continued their habitual diet; participants of the HP and RP group consumed either high protein or standard foods. After 6 weeks of dietary intervention, HP and RP groups additionally started a strength training intervention twice per week for 8 weeks. 24h dietary recalls were performed every 7 to 10 days. Body composition was assessed and blood taken. Plasma proteomics were assessed with LC-MS. Results: Participants of the HP group doubled their baseline protein intake from 0.80±0.31 g/kg BW/d to 1.63±0.36 g/kg BW/d; RP increased the protein intake from 0.89±0.28 g/kg BW/d to 1.06±0.26 g/kg BW/d. The CON group kept the protein intake stable throughout the study. Combined exercise and HP initiated notable changes, resulting in a reduction in bodyfat and increased muscle mass. Proteomics analyses revealed 14 significantly affected proteins by HP diet, regulating innate immune system, lipid transport and blood coagulation, yet the additional strength training did not elicit further changes. Conclusions: Combined HP and resistance exercise in healthy older adults seem to induce favorable changes in the body composition. Changes in the plasma proteome due to the high protein diet point to a beneficial impact for the innate immune system, lipid transport and blood coagulation system, all of which are involved in chronic disease development.

Effects of Lactobacillus curvatus HY7601 and Lactobacillus plantarum KY1032 on Overweight and Gut Microbiota in Human: Randomized, Double-Blinded, Placebo-Controlled Clinical Trial

Project description:Obesity and overweight are closely related to diet, and gut microbiota play an important role in body weight and human health. The aim of this study was to explore how Lactobacillus curvatus HY7601 and Lactobacillus plantarum KY1032 supplementation alleviate obesity by modulating the human gut microbiome. A randomized, double-blind, placebo-controlled study was conducted on 72 overweight individuals. Over a 12-week period, probiotic groups consumed 5×10^9 colony-forming units of HY7601 and KY1032), whereas the placebo group consumed the same product without probiotics. After treatment, the probiotic group displayed a reduction in body weight (p <0.001), visceral fat mass (p <0.025), and waist circumference (p <0.007), and an increase in adiponectin (p <0.046), compared with the placebo group. Additionally, HY7601 and KY1032 supplementation modulated bacterial gut microbiota characteristics and beta diversity by increasing Bifidobacteriaceae and Akkermansiaceae, and decreasing Prevotellaceae and Selenomonadaceae. In summary, HY7601 and KY1032 probiotics exert anti-obesity effects by regulating the gut microbiota; hence, they have therapeutic potential for preventing or alleviating obesity and overweight.

Diet Composition Shapes Sleep, Quality of Life, and Gene Expression in Obesity: A Randomized Trial.

Project description:Obesity affects quality of life and sleep, making effective weight loss an attractive intervention. The role of diet composition on outcomes has not been determined. Our aim is to compare a ketogenic diet against an isocaloric balanced diet for effects on quality of life, sleep, circadian rhythm and gene expression in obesity. Twenty-four people affected by obesity completed the study. Volunteers were randomized to a 1200 Kcal ketogenic diet or balanced diet for 30 days, and all were encouraged to walk 30 minutes every day. Questionnaires for assessing quality of life and sleep were administered at baseline and at the end of the study. In a subgroup of 20 people actigraphy was performed to objectively capture sleep parameters, and circulating whole blood gene expression analysis was undertaken. Quality of life assessed with the IWQOL-Lite significantly improved after both diets (p<0.001), with the ketogenic diet showing a greater effect (p=0.006). The ketogenic diet also resulted in greater improvement in subjective sleep quality (p=0.023). Both diets improved daytime sleepiness (balanced diet p=0.012; ketogenic diet p=0.021). The ketogenic diet induced a greater weight loss (p=0.026). Gene expression analysis revealed greater changes in lipid metabolism with ketogenic diet. No significant changes in circadian genes were seen. Weight loss with diet intervention significantly improved quality of life of people with obesity, already after 30 days. The ketogenic diet resulted in greater improvements, in parallel with greater weight loss. Both diets influenced the expression of many metabolic genes in whole blood, with greater changes in lipid metabolism seen with ketogenic diet. Weight loss with diet intervention is effective to improve quality of life, sleep, and daytime sleepiness symptoms.
