Project description:Objectives: We compared metabolic biomarkers in the blood and PBMC gene expression profiles among normal weight, mildly obese, and moderately obese Korean adult men. Design: Subjects were classified as normal weight (BMI, 18.5~23 kg/m2), mildly obese (BMI, 25~27.5 kg/m2), and moderately obese (BMI, 27.5~30 kg/m2). Results: High leptin, lipids (except LDL- and HDL-cholesterol), and apolipoprotein B levels and low adiponectin and HDL-cholesterol levels were present in the plasma of both mildly and moderately obese subjects. Circulating levels of inflammatory cytokines (TNF-α and IL-6) and markers of insulin resistance, oxidative stress, and liver damage were altered in moderately obese subjects but not mildly obese subjects. PBMC transcriptome data showed enrichment of pathways involved in energy metabolism, insulin resistance, bone metabolism, cancer, inflammation, and fibrosis in both mildly and moderately obese subjects. Signaling pathways involved in oxidative phosphorylation; triglyceride synthesis; carbohydrate metabolism; insulin, mTOR, FOXO, RAP1, RAS, and TGF-β signaling; and ECM–receptor interaction were enriched only in moderately obese subjects, indicating that changes in PBMC gene expression profiles according to metabolic disturbances were associated with the development and/or aggravation of obesity. In particular, upregulation of 8 genes (FLT3LG, LTB, IL23A, CD19, CPT1B, AXIN2, CACNA1L, RPRM) and downregulation of 2 genes (CCL4L1, LPAR5) were observed only in mildly obese subjects. Six genes (CXCR5, NFKBIA, BIRC3, TNFAIP3, DDIT3, PDE4B) and 4 genes (CX3CR1, HSPA1A, CACNA2D3, APAF1) were up- and down-regulated, respectively, in both mildly and moderately obese subjects. These results suggested that these genes could be used as early or stable biomarkers for diagnosing and treating obesity-associated metabolic disturbance.

Project description:The aim of study was to investigate correlation bewteen peripheral blood mononuclear cell (PBMC) transcriptome profiles and plasma lipid profiles of Korean adult with varying BMI. In Korean, BMI cut-off points are 18.5~22.9 kg/m2 (normal range), M-bM-^IM-% 23 kg/m2 (overweight), M-bM-^IM-% 25 kg/m2 (obese), M-bM-^IM-% 30 kg/m2 (severely obese). Thus, the obese group was subdivided into mildly obese (BMI 25~27 kg/m2, OA) and highly obese (BMI 27~30 kg/m2, OB). This study indicates that lipid, glucose and inflammation metabolism-related gene expressions were altered according to cahnge of varing phenotype biomarkers such as BMI, plasma total-C, TG, FFA and HDL-C. Thus, blood biomarkers and PBMC gene expression profiles identified in this study may be useful as indicative biomarkers for obese susceptibility in Korean adult as well as response to various intervention for treating obesity. Total RNA of PBMCs was obtained from normal weight, obese person and mRNA expression was measured.

Project description:The aim of this study is to design and validate a test, METAHEALTH-TEST, based on gene expression analysis in blood cells, to quickly and easily analyse metabolic health. This test will be used to analyse metabolic improvement in overweight/obese individuals and in metabolically obese normal-weight (MONW) individuals after undergoing a weight loss intervention and/or an intervention for improvement in eating habits and lifestyle. Obesity and its medical complications are a serious health problem today. Using peripheral blood mononuclear cells (PBMC) as an easily obtainable source of transcriptomic biomarkers would allow to deepen into the knowledge of adaptations in response to increased adiposity that occur in internal homeostatic tissues, without the need of using invasive biopsies. Moreover, if PBMC were able to reflect lipid metabolism gene expression pattern recovery as a result of weight loss, it would provide valuable information to know the efficacy of therapies aimed at weight loss and, in any case, it would allow to personalize them according to the evolution of obese patients until the desired metabolic recovery is achieved.

PBMCs were obtained from 12 overweight/obese boys for RNA extraction and hybridization on Affymetrix microarrays. We performed the microarray analysis at baseline and after a weight loss intervention program in a total of 24 samples. They were distributed by dietary response as high and low responders.
