We experimentally study the effectiveness of policy interventions in reducing the negative welfare effects of behavioral biases on job search. Due to quasi-hyperbolic discounting, individuals reduce their search effort and reservation wage, while the sunk-cost fallacy makes individuals decrease their reservation wage over the search spell. Project description:Umbilical cord blood is a source of hematopoietic stem cells essential to treat life-threatening diseases, such as leukemia and lymphoma. However, only a very small percentage of parents donate upon delivery. The decision to donate the cord blood occurs at a very specific time and when parents likely experience emotional, informational, and decisional overloads; these features of cord blood donation make it different from other pro-social activities. In collaboration with an OB-GYN clinic in Milan, Italy, we conducted the first randomized controlled trial that applies tools from behavioral science to foster cord blood donation, and quantified the gains that informational and behavioral "nudges" can achieve. We found that information and "soft" commitments increased donations; approaching expecting parents closer to the delivery date and providing them with multiple reminders, moreover, had the strongest impact. However, a significant portion of women who expressed consent to donate could not do so because of organizational constraints. We conclude that simple, non-invasive behavioral interventions that address information gaps and procrastination, and that increase the salience of the activity can substantially enhance altruistic donations of cord blood, especially when coupled with organizational support.

Project description:Cell-free DNA (cfDNA) is a rich source of biomarkers for various pathophysiological conditions. Preanalytical variables, such as the library preparation protocol or sequencing platform, are major confounders of cfDNA analysis. We present DAGIP, a novel data correction method that builds on optimal transport theory and deep learning, which explicitly corrects for the effect of such preanalytical variables and can infer technical biases. Our method improves cancer detection and copy number alteration analysis by alleviating the sources of variation that are not of biological origin. It also enhances fragmentomic analysis of cfDNA. DAGIP allows the integration of cohorts from different studies.

Project description:BackgroundWhile international research has examined the relationship between job characteristics and mental health, including gender differences, few studies have assessed these associations at the national level in the U.S., which has unique labor markets, health care systems, and societal structures that may exacerbate gender disparities. This study investigates gender differences in the associations between job flexibility, job security, mental health outcomes, work absenteeism, work presenteeism, and mental health care utilization among a representative sample of working U.S. adults.MethodsWe analyzed cross-sectional population-based survey data from employed adults in the 2021 National Health Interview Survey. Job characteristics included perceived job flexibility and security. Outcomes included serious psychological distress, frequency of anxiety, work absenteeism, work presenteeism, and mental health care utilization. Multivariable logistic and binomial regression analyses examined associations of interest, with statistical interaction tests conducted to assess gender differences.FindingsThe study sample included 18,112 respondents weighted to represent a population of 168,068,586 civilian, non-institutionalized working U.S. adults (47.7% female). Females with low job security had a decreased probability of serious psychological distress than males with low job security (F(3,589) = 2.79; p = 0.040). Females with the lowest job flexibility reported more days worked while ill than males over the past 3 months, while males with higher job flexibility reported more days worked while ill than females (F(3,589) = 4.1; p = 0.007). The average number of work days missed over 12 months was lower among females than males when job security was perceived as fairly low and higher among females than males as job security increased (F(3,589) = 4.3; p = 0.005).InterpretationFindings highlight the need for policies and practices that recognize and address gender-specific workforce experiences and needs. Tailored interventions that enhance job flexibility and security, support caregiving responsibilities, and provide comprehensive mental health services can address such challenges.

Project description:Background/Objectives: Antimicrobial resistance (AMR) is a global public health concern exacerbated by inappropriate antimicrobial prescribing practices, particularly in low-resource settings such as Uganda. The research aimed to develop a culturally sensitive behavioral intervention, leveraging a "nudge" strategy, to improve healthcare provider adherence to the 2016 Uganda Clinical Guidelines (UCG 2016) in five Ugandan hospitals. This intervention formed part of broader antimicrobial stewardship initiatives led by the United States Agency for International Development Medicines, Technologies, and Pharmaceutical Services Program. Methods: This study employed a mixed-methods approach, combining formative research and behavioral intervention. Guided by the Deloitte Behavioral Insights Framework, the research team conducted key informant interviews to identify prescribing barriers and motivators and developed three suitable behavioral interventions: perceived monitoring, ward leaderboards, and educational workshops. The study evaluated the interventions' impact through point prevalence surveys (PPS), using the World Health Organization PPS methodology at three stages: pre-intervention, immediate post-intervention, and one-month post-intervention. Results: Key behavioral themes across individual, social, environmental, and organizational elements informed the intervention design and implementation. The behavioral intervention package increased antimicrobial prescription compliance with the UCG 2016 from 27% at baseline to 50% immediately post-intervention, though these effects diminished at one-month post-intervention. Conclusions: Our study addresses an existing gap in behavioral nudges-based operational research on antimicrobial prescribing in low- and middle-income countries. These results showed an immediate improvement in adherence to the UCG 2016 among healthcare providers in Ugandan hospitals, though the effect was attenuated at one-month follow-up. Despite the attenuation, behavior change presents a feasible, cost-effective, and sustainable approach to improving antimicrobial prescribing practices and addressing AMR.

Project description:IntroductionRecently, the network theory of mental disorders has been used to conceptualize work addiction as a dynamic system of symptoms in direct relationships. This study aimed to extend previous work by investigating the direct relationships of work addiction symptoms with dimensions of work engagement, job burnout, and perceived stress.MethodsThese phenomena were measured with the Bergen Work Addiction Scale, the Utrecht Work Engagement Scale, the Maslach Burnout Inventory-General Survey, and the Perceived Stress Scale. The sample comprised 676 working Poles with a mean age of 36.12 years (SD = 11.23). The network analysis followed the guidelines for estimating psychological networks from cross-sectional data.ResultsWork engagement and job burnout were more closely associated with each other than with work addiction which supports the notion that engagement and burnout represent polar opposites of the same construct and that work addiction is a separate phenomenon, related to both work engagement and job burnout via specific pathways. The symptoms of work addiction were connected with other phenomena through four direct relationships: (1) mood modification-absorption, (2) mood modification-stress, (3) withdrawal-absorption, and (4) problems-exhaustion.DiscussionThese findings narrow down and specify hypotheses regarding potential mechanisms leading from engagement to addiction and from addiction to burnout. The possible mechanisms focus on the absorption component and mood modification related to efforts focused on alleviating chronic stress and negative emotional states. In turn, problems arising from work addiction may lead to exhaustion. Future studies investigating these mechanisms in detail may enable proper prevention programs and therapeutic interventions.

Project description:Americans collectively hold over $1.6 trillion in student loan debt, and over the last decade millions of borrowers have defaulted on loans, with serious consequences for their financial health. In a 13-million-person field experiment with the U.S. Department of Education, we tested the effectiveness of different email interventions to inform borrowers about alternative repayment options after a missed loan payment. Our interventions tested whether sending monthly behaviorally-informed emails, providing follow-up reminders, framing benefits in percentage (vs. dollar) terms, and providing just one recommended action step at a time (vs. two) affected borrower outcomes. We find that i) behaviorally-informed emails reduce estimated 60-d delinquencies by 0.42 pp, ii) reminders boost the efficacy of such emails by 0.57 pp, iii) describing potential savings in percentage terms is more effective than describing these benefits in dollar terms, reducing estimated delinquencies by 0.14 pp, and iv) encouraging two actions (i.e., enrollment in income-driven repayment plans and auto debit programs) repeatedly across two emails is marginally more effective than encouraging one action at-a-time across two emails, reducing estimated delinquencies by 0.05 pp. Overall, if scaled to all 13-million borrowers in our experiment, we estimate that our best-performing intervention would have averted approximately 79,800 60-d delinquencies. Our findings i) highlight the benefits of describing potential savings in percentage terms, which may magnify perceived savings for recipients, ii) underscore the risks of oversimplification, and iii) demonstrate that nudges can be an effective, low-cost complement to other policies for reducing delinquencies and supporting borrowers with student loan debt.

Project description:The Flexible Job Shop Scheduling Problem (FJSP) is a combinatorial problem that continues to be studied extensively due to its practical implications in manufacturing systems and emerging new variants, in order to model and optimize more complex situations that reflect the current needs of the industry better. This work presents a new metaheuristic algorithm called the global-local neighborhood search algorithm (GLNSA), in which the neighborhood concepts of a cellular automaton are used, so that a set of leading solutions called smart-cells generates and shares information that helps to optimize instances of the FJSP. The GLNSA algorithm is accompanied by a tabu search that implements a simplified version of the Nopt1 neighborhood defined in Mastrolilli & Gambardella (2000) to complement the optimization task. The experiments carried out show a satisfactory performance of the proposed algorithm, compared with other results published in recent algorithms, using four benchmark sets and 101 test problems.

Project description:Employees with impaired work ability might be at higher risk of remaining shorter in the job than those with adequate work ability. The aim of the study was to establish whether work ability plays a role in job survival. Four-year follow-up (2008-2012) study of 1037 employees of a hospital in São Paulo, Brazil. Work ability was categorized as "adequate" or "impaired". Employment status at the end of follow-up was categorized as active, resignation or dismissal. Survival analysis was performed using the Kaplan-Meier method and the Cox proportional-hazards model. About 78.9% of the participants had adequate and 21.1% impaired work ability. Job survival was longer for the participants with adequate work ability independently from the type of job termination (p < 0.001). The odds of job termination were higher for the participants with impaired work ability (p < 0.001) who either resigned (hazard ratio-HR = 1.58) or were dismissed (HR = 1.68). Job survival was shorter for the employees with impaired work ability independently from the type of job termination. It was also shorter for the employees who were dismissed compared to those who resigned. Duration in the job might be extended through actions to enhance work ability.

Project description:Electronic health records (EHRs) offer decision support in the form of alerts, which are often though not always interruptive. These alerts, though sometimes effective, can come at the cost of high cognitive burden and workflow disruption. Less well studied is the design of the EHR itself-the ordering provider's "choice architecture"-which "nudges" users toward alternatives, sometimes unintentionally toward waste and misuse, but ideally intentionally toward better practice. We studied 3 different workflows at our institution where the existing choice architecture was potentially nudging providers toward erroneous decisions, waste, and misuse in the form of inappropriate laboratory work, incorrectly specified computerized tomographic imaging, and excessive benzodiazepine dosing for imaging-related sedation. We changed the architecture to nudge providers toward better practice and found that the 3 nudges were successful to varying degrees in reducing erroneous decision-making and mitigating waste and misuse.

Project description:Rationale: Low and slow patient enrollment remains a barrier to critical care randomized controlled trials (RCTs). Behavioral economic insights suggest that nudges may address some enrollment challenges.Objectives: To evaluate the efficacy of a novel preconsent survey consisting of nudges on critical care RCT enrollment.Methods: We conducted an RCT in 10 intensive care units (ICUs) among surrogate decision-makers (SDMs). The novel multicomponent behavioral nudge survey was administered immediately before soliciting SDMs' informed consent for their patients' participation in a sham trial of two mechanical ventilation weaning approaches in acute respiratory failure. The primary outcome was the enrollment rate for the sham trial. Secondary outcomes included undue and unjust inducements. We also explored SDM and patient predictors of enrollment using multivariate regression.Results: Among 182 SDMs, 93 were randomized to receive the intervention survey and 89 to receive standard informed consent. There was no statistically significant difference in enrollment rates between the intervention (29%) and standard consent (34%) groups (percentage difference, 5%; 95% confidence interval [CI], -9% to 18%; P = 0.50). There was no evidence of undue or unjust inducement. White SDMs were more likely to enroll the patient compared with non-white SDMs (odds ratio, 3.7; 95% CI, 1.1 to 12.2; P = 0.03). SDMs who perceived a higher risk of participation were less likely to enroll the patient (odds ratio, 0.57; 95% CI, 0.46 to 0.71; P < 0.001).Conclusions: A preconsent behavioral nudge survey among SDMs of patients with acute respiratory failure in the ICU did not increase enrollment rates for a sham RCT compared with standard informed consent procedures.Clinical trial registered with ClinicalTrials.gov (NCT03284359).
